High winds blew the doors on the back of the Drag Line house open. The employee and two others decided to close the doors. They successfully closed the right door and were attempting to close the left door when a gust of wind hit the door and flung it open blowing all three of them back with the door. The employee was pinched between the door and the guarding around the Gen-set.

The 103 Dragline at Ramp 53 contacted the cable handler #768 and was struck by the left rear house of the dragline while the dragline was in full swing. As a result, the cable handler was tipped onto its side. The cable handler operator was transported to the Dr. Office for further medical evaluation. On 10/13/25 the MRI confirmed that the employee would need surgery.

Employee was climbing off the dozer that was located on the deck of the lowboy trailer while stepping backwards when they fell off the trailer approximately 3 to 4 feet to the ground onto their back. This resulted in pain to the right elbow, and the wind was knocked out of them. The employee was transported to a higher level of care for further evaluation.

While positioning a barrel of solvent in the lube room of the 102. The employee's finger got caught between the barrel and the air compressor bank pinching the Left hand pinky finger crushing it between the barrel and the compressor bank.

An employee stopped for lunch break at the 2 high wall rock run. Employee climbed down the ladder grabbed both chock blocks, was walking to set one chock block on the left front wheel. The employee slipped on the ice and landed on their arm.

Employee noticed a hose blew apart down in Ramp 58. EE went down to the pump, shut the pump off and started to disassemble. The hose was not repairable, so EE went to get a different hose to replace it. When starting to pull the hose apart EE felt a poke through the glove. After removing the glove, the employee found a laceration on their left middle finger.

Employee was performing an oil change on 1774 after removing the first oil filter. Employee began to remove the second filter the employee felt 3 pops in their right arm between the bicep and forearm. Employee stated that the filter could have been stuck or had been put on too tight. MRI was done and a torn muscle was found.

Employee was carrying a receiver plate and tripped on a mixed gas hose and fell with the plate and the plate contacted the employee on the left eye brow. This caused a laceration over the left eye brow needing stitches.

Employee was opening the back doors of the dragline and placed hand on the lower rail of the door. When the door swung open, EE's left hand was located in a pinch point between the door and the back house fan.

Employee was doing audio test on 2-17-2022 and has showed employee had sustained a STS in left ear. Employee was scheduled to re test on the 2/23/22 and it confirmed that the employee has sustained a permanent STS in left ear, from the baseline in 2017.

Employee was working on 308 drill outside the shop. EE was standing on the mast loosening a hydraulic hose, after they broke the hose loose, they stepped down with right foot, left foot was caught under another hydraulic hose and right foot landed on uneven footing causing them to roll the ankle. Resulting in a small fracture to the right foot.

Employee was walking from the tailgate towards the head-gate when employee was stepping from shield 105 to shield 104 a rock fell from in-between the shields and landed on employee's foot. This caused a laceration between the two small toes on right foot and fractured the small toe.

Employee was changing out a transfer chute wear plate. The employee was directing the crane when the wear plate became hung up. The employee freed the wear plate and as the plate settled the employee's right wrist was caught and bent backwards. The employee was referred to University of Utah. A diagnosis from the employee was received on 03/29/2021 after EE's surgery.

Employee was removing the tin off of a portable building. When EE went to move EE's lanyard to a different place the tin EE was standing on gave way. The employee fell approximately 12.5ft.

Employee was stocking the trays on the roof bolter, when EE's hand was pinched between the mesh rack and the bolt pod. The mesh rack had been set down on an open bundle of bolts. When the plates were removed the bolts shifted allowing the mesh rack to drop down about 1 inch which was enough to pinch the employee's hand.

Employee was walking down the belt line when EE stepped into a water hole and twisted EE's knee

The Skid Steer bucket slipped off the attachment shoe edge and fell approximately 1.5 foot to 2 feet to the floor striking the employees foot behind the hard toe.

D-rings left on hoses got tangled up while pulling down road, Employee went to untangle, lack of communication between driver of vehicle and employee watching cable. Employee got tangled up and drug down road way.

Injured employee was working on the longwall face, pulling in shields. As the shearer passed near EE's location the tailgate side drum picked up a large piece of coal, the piece of coal hit the pan and then struck employee on shoulder and foot. Employee has not been able to return to regular job duties.

Employee had re-set electrical breaker on a belt winder. After re-setting the electrical breaker, employee reached across the control box to turn the pump on when they received an electrical shock. Employee was originally fully released to full duty but recent medical evaluation has placed them on days away from work, therefore, late reporting of this incident.

Contractor was trying to lift a bolter cable in order for a skid-steer to place a pallet of plates. In doing so,the skid-steer ran over the contractors foot. Contractor was placed on restricted duty as of 04/09/2020. Contractor terminated employment as of 04/14/2020.

Injured miner was giving directions to another miner as they were using shield #3 to free pan blockage. Injured miner was standing under shield #4 when a piece of rock fell and hit foot. The wound required stitches to close.

Employee arrived at R1 drill pad work area. Exited 2470 pickup, went around to get lunch box out of the passenger side. Grabbed lunch bucket with left hand, holding the door with right hand. The wind pushed the door shut and right middle finger was struck between the door and the frame. The finger was lacerated resulting in sutures.

Employee was carrying old scraper parts, another miner was cutting a lid from a bucket using a knife. The knife slipped as employee walked by, cutting employee's leg. Stitches were required to close the cut.

Employee was climbing up the off side boarding ladder on 222 track hoe when, they slipped hitting right shin on a rung of the ladder causing a laceration on shin which required sutures.

While building crib, employee got hand caught between 2 crib blocks.

Employee was using an index bar to pry with while removing a stab jack from a piece of equipment & felt burning sensation in both arms. Later, medical examination was sought & showed that employee needed surgery for the repair of the condition, surgery on 9/23/2019, therefore, employee is on days away from work. Late reporting due to the late medical evaluation.

Employee was lifting hood off of D11 CAT Dozer, Hood shifted and pinched left index finger between the hood and the holding bracket for the hood. Broke index finger.

While fueling 442 Haul Truck at Ramp 53 fuel island, and was putting chock blocks in holder and jammed left middle finger resulting in an extensor tendon tear.

Employee was installing wedges into fiberglass rib pins. While performing this task they felt their eye becoming irritated and red. Medical personnel removed a piece of fiberglass and prescribed medication for the eye injury.

While fueling 4117 pump in ramp 60, employee went to grab EE's radio out of 1771 lube truck. While climbing up steps employee slipped and hit EE's left side on steps.

Contractor employee was loading belt structure onto a trailer. As ee stepped off of the back of the trailer onto some uneven floor, ee rolled ankle. Contractor employee is placed on restricted duty.

Roof Fall in the #3 entry, crosscut #10,from crosscut going inby, of the 5th Left longwall section. Approximate dimensions of the roof fall: 60 ft (length) 8 to 15 ft. (high) and 18 ft (wide). Exact time of the roof fall is unknown.

Employee was sitting in the cab of Company #443 haul truck a 785 Caterpillar haul truck, being loaded with dirt. An unseen rock in the bucket struck the truck bed jolting the employee.

Employee was lifting a belt roller, the roller slipped and while catching the roller they felt pain in the shoulder and arm. Employee was referred to an orthopedic doctor and following an MRI review (09/18/2018) it was determined they required surgery (11/01/2018) for the injury.

Employee was removing wires from a communication box, the box was sitting on a table. The employee used a razor knife to cut a couple of wires, the knife slipped and cut employees wrist area. The wound required stitches and employee was placed on restricted duty.

A roof fall was discovered during weekly examination on November 27, 2017. The fall is located in the Main North section, crosscut #14 between entries #3 and #4. Approximate dimensions are 12 to 22 feet long, 16 feet wide and 6 feet high. The area was bolted with 6 foot resin bolts. The area was developed March, 2005.

Contract employee was pulling feeder cable slack to the feeder breaker. As employee was pulling employee fell towards the rib and land on employee's SCSR unit. The fall resulted in a fractured rib.

Employee was crossing pan line and employee's foot slipped causing an injury to employee's penis when groin area hit pan line.

Employee was drilling a 5/8" holes in 3"x3" angle iron with a Wilton drill press. The piece pulled up out of the vice and grabbed EE's welding glove on EE's left hand. The drill twisted EE's hand and arm into the bit until it pulled off the glove and the bit broke. This accident resulted in a hairline distal fracture of the left ulna.

Employee was working at a surface PC room. While working on the PC they decided to open a garage door, as they attempted to open the door they bent over to grab the middle of the door, as they bent over, left foot caught on some grating causing them to fall forward hurting shoulder. Recent medical evaluation has placed employee on Lost Time from the work place.

A roof fall was discovered in the intersection of Main West #4, crosscut #27. Fall was approximately 23.5' x 23' and 11 feet high. Exact time of roof fall is unknown.

Welder was standing on a platform, driving a wedge setting the sidewall on a dragline bucket in the bucket shop. The employee lost balance and put left hand on the dog while swinging the 6 lb. Sledge. The sledge glanced off the wedge and struck the employee's left ring finger.

A roof fall was discovered in the 17th Right Section, entry #3, outby crosscut #5. Approximate dimensions of fall are: 18.5 feet long, 7.7 feet high and 15 feet wide.

Employee dropped a beltline down to splice the belt. Employee pushed the belt slack with left hand, while pushing employee stepped backwards onto a 4" lip in the mine floor. Employee lost balance and fell onto the left hand, this resulted in a dislocated finger.

Employee was removing a Kennedy stopping, the stopping had a mandoor in it. After removing the majority of the stopping, the mandoor rolled off of the remaining few panels and struck the employee on top of the foot. Late reporting due to recent medical evaluation placing employee on days away from work starting on 6-5-2017.

Employee had been performing several tasks during working shift, tasks included carrying belt rollers, picking up/carrying crib blocks and rolling cable. During the shift employee felt pain in groin area and on March 10, 2017, medical professionals diagnosed employee with a hernia. Late reporting due to late medical evaluation.

The roof bolting machine was moving to another entry, there was approximately 15 feet of cable slack behind the machine. The cable reel retracted the slack and the machine began to tram forward, the miner was struck by the cable hanging from the mine roof when the cable was pulled down from the hangers. After medical evaluation, the miner has been placed on restricted duty.
