DECENDING FROM A DOZER. WHEN EMPLOYEE RELEASED HIS HAND HOLD AND PUT WEIGHT ON HIS RIGHT FOOT, HIS FOOT WAS ON A STONE, APPROX. 4" DIAMETER, THE STONE MOVED CAUSING HIS ANKLE TO TWIST.

EMPLOYEE WAS LOOSENING A BOLT ON CAT D-9 DOZER AND THE HEAD BROKE OFF. EMPLOYEE THEN PROCEEDED TO USE A PUNCH AND HAMMER TO REMOVE THE BOLT. EMPLOYEE WAS STRIKING THE PUNCH WITH LIGHT HAMMER TAPS AND SMALL PIECE OF METAL BROKE FROM THE BOLT AND STRUCK THE EMPLOYEE.
