Conditions were wet and muddy. Injured was attempting to get on swivel truck and right foot slipped while left foot was on non-skid surface. Lost balance, weight shifted, twisting left leg and resulting in fall from truck. Was taken to nearby clinic and preliminary diagnosis is possible torn quad tendon / muscle.

Before work started, employee was petting watch dog in front of another employee's vehicle. The employee driving the jeep did not see the employee and bumped his leg with jeep.
