ROOF FALL WAS DISCOVERED ON 1-20-03 IN NUMBER 2 BELT ENTRY AT 0800. SIZED OF FALL IS 20' WIDE X 30' LONG X 6-8' HIGH LOCATED AT SS #6. ROOF FALL WILL BE CLEANED UP AND DOES NOT AFFECT VENTILAT ION OR TRAVEL.
A ROOF FALL OCCURRED MEASURING 20' LONG X 10' WIDE X 10' HIGH IN 6 RIGHT CUT THROUGH A 3 BREAKS INBY SPAD NUMBER 1761. NO EQUIPMENT WAS INVOLVED, NO VENTILATION WAS DISTURBED NO INJURY WAS REP ORTED, AND THE FALL WILL NOT BE CLEANED UP.
EE STATED THAT WHILE SETTING BREAKER TIMBERS ON A PILLAR SECTION HE WAS STRUCK BY LOOSE RIB. WEARING RUBBER METATERSEL FOOTS

THE RIGHT MINER WAS CAUGHT IN A PILLAR FALL THE FALL OCCURRED AT BLOCK 140 IN #6 ENTRY. THE FALL MEASURED 20' HIGHX 20' WIDE X 60' LONG. THE FALL OCCURRED DUE TO SUB NORMAL ROOF CONECTION NOT DETECTABLE OR VISIBLE. NO INJURIES RESULTED FROMTHE FALL.
A ROOF FALL OCCURRED MEASURING 25' LONG X 20' WIDE X 10' HIGH IN THE INTERSECTION AT SPAD #80. NO EQUIPMENT WAS INVOLVED, NO VENTILATION DISTURBED, NO INJURY OCCURRED AND THE FALL WILL NOT BE CLEANED UP. NOTE: THIS MINE WAS IDLE FROM 12/21/02 THRU 01/01/03 SO THE FALL WAS NOT DISCOVERED UNTIL THE FIRST DAY BACK 01/02/03.
WHILE UNLOADING BOLTS AND PLATES OUT OF A SCOOP BUCKET ONTO THE BOLTER, EMPLOYEE PICKED UP 5 TO 6 BOLTS TOOK ONE STEP BACK INTO A HOLE FORCING HIS LEFT KNEE BACKWARD. NOTE: OPERATIONS WERE IDL E FROM 12/21/02 THRU 1/2/03. REPORT IS WITHIN 10 DAY LIMIT.

WHILE PILLARING, A PARTIAL PILLAR FALL OCCURRED WHILE MINING THE SECOND LIFT OUT OF THE NUMBER 2 BLOCK, IT FELL BESIDE THE MINER WEDGING IT TO THE RIB. THE EQUIPMENT INVOLVED WAS A JOY 14-15 M INER. NO VENTILATION WAS DISTURBED. NO INJURY OCCURRED AND THE FALL WILL NOT BE CLEANED UP.

THE EE WAS INSTALLING HIS OUTSIDE BOLT WHEN A ROCK FELL FROM BETWEEN THE ATRS JACK AND THE HEAD OF THE ROOF BOLT MACHINE, STRIKING THE EE LEFTLEG. EX-RAY NEGATIVE , BRUISE
EMPLOYEE WAS SAWING A TIMBER WHEN OTHER TIMBERS FELL CATCHING THE JOSE OF THE HYDRAULIC SAW CAUSING IT TO BUCK UPWARD STRIKING EMPLOYEE ON THE LEFT SIDE OF HIS FACE.
NORTH MAINS: THE EE WAS PERFORMING MAINTENANCE ON THE REMOTE CONTROL MINER AND A RIB FELL WITHOUT WARNING, STRIKING THE EE'S BACK RIGHT LEG, THE X-RAYS WERE ALL NEGATIVE.
A ROOF FALL OCCURRED MEASURING 15'X20'X10' HIGH BEGINNING AT APPROX. 250' INSIDE, IN INTERSECTION, INTAKE #6 ENTRY. APPROX. 14000' OUTBY WORKING SECTION. NO EQUIPMENT WAS INVOLVED, VENTILATION WAS NOT DISTURBED, NO INJURY OCCURRED AND THE FALL WILL NOT BE CLEANED UP.
MOVE CREW WAS SPLICING BELT, AND HE CUT HIS LEFT HAND, BETWEEN THUMB AND INDEX FINGER REQUIRING STITCHES.
EE STATES THAT HE & (2) TWO CO-WORKERS WERE ATTEMPTING TO FREE A WEDGED CONVEYOR CHAIN ON A CONTINUOUS MINER WITH A SLEDGE HAMMER & BAR. THE BAR WHICH THE INJURED WAS HOLDING SLID UNDER THE CH AIN & THE INJURED'S LEFT HAND WAS CAUGHT BETWEEN THE FRAME OF THE MINER AND THE BAR.
EE REPORTED THAT WHILE TRAMMING THE ROOF BOLTER HE BUMPED HIS RIGHT ELBOW ON THE CANOPY JACK IN THE DECK.
A ROOF FALL OCCURRED MEASURING 17'LONG X 10' WIDE AND 8' HIGH BEGINNING AT 2 BREAKS IN SPAD #1737 IN #3 ENTRY IN THE INTERSECTION . NO EQUIPMENT WAS INVOLVED, NO VENTILATION WAS DISTURBED, NO INJURY OCCURRED AND THE FALL WILL BE CLEANED UP.
EE STATES THAT WHILE PERFROMING NORMAL DUTIES HE FELT PAIN IN THE RIGHT KNEE AND LOSS OF MOVEMENT. THERE WAS NO REPORTED EVENT TO CAUSE THE SYMPTOMS AND APPEARS THAT THE CAUSE WAS DUE TO REPEA TED TRAUMA OVER AN EXTENDED PERIOD OF TIME CAUSING THE OCCUPATIONAL ILLNESS.
LEFT FRONT TIRE BLEW OUT. IT'S BELEIVED EXPLOSION BLEW OIL FILTER BASE INTO STARTER WITH A SHORT CIRCUIT OCCURRING.
EE REPROTED THAT ON 11-23-02 HE FELT PAIN IN HISLEFT ARM AFTER RELEASING PRESSURE ON COME ALONG.EE WERE MAKING A BELT MOVE AT THE TIME. EE NOW REPORTS THAT A MEDICAL PHYSICIAN DIAGNOSED THE PA IN AS A HAIR LINE FRACTURE. EE STATES THAT THE ON SET OF THE PAIN OCCURRED AFTER RELEASING PRESSURE ON A COME ALONG WORKED UNTIL 1-6-03
EE DID NOT HAVE HIS DRILL STEELS CONNECTED PROPERLY, THEY BENT, SNAPPED BACK AND STRUCK HIM ONT HE RIGHT INDEX FINGER. FRACTURE TO INDEX FINGER.
EE HAS BEEN DIAGNOSED WITH CARPAL TUNNEL SYNDROME IN THE RIGHT AND LEFT WRISTS.
SLOPE TRACK: THERE WAS AN UNDETECTED ROCK THAT ROLLED ON THE SLOPE TRACK AND LODGED UNDER THE HOIST CAR. THE TRUCKS WERE DAMAGED AND REPAIR WAS NECESSARY. TWO EMPTY FLATS WERE BEING BROUGHT OU TSIDE AT TIME OF INCIDENT. REPAIR COMPLETED 730 AM
EAST MAINS LEFT SIDE MMU 007 MINER #5 ENTRY. ONT HE EAST MAINS SECTION, APPROX. 50 FT IN THE #5 ENTRY THERE WAS A FRICTIONAL IGNITION THAT WAS LIMITED TO THE PAN AREA OF THE MINER AND LASTED 1 -2 SECONDS. YELLOW ORANGE FLAME SPARKS FROM BITSSTIKING SANDSTONE-ROCK BINDER.
EE FELL WHILE CLEANING COAL CHUTE AND CUT HIS LEFT KNEE. WOUND REQUIRED SUTURES. MEDICAL REPORT.
EMPLOYEE WAS HOLDING ROOF BOLT IN PLACE WITH HIS LEFT HAND WHEN A ROCK FELL AND STRUCK HIS THUMB ON HIS LEFT HAND.
EE WAS GETTING OUT OF SHUTTLE CAR AND SPRAINED HIS RIGHT ANKLE.
WHEN WORKING BY THE ROOF BOLT MACHINE, THE EE CAUGHT HIS HAND ON A ROOF BOLT STRAP THAT WAS HANGING FROM THE SIDE OF THE MACHINE.
EAST MAINS #9 ENTRY IN BY CROSS CUT. THE MMU 008 MINER FINISHED THE A CUT (CURTAINSIDE) AND THE B-CUT. THE OPERATOR WAS CLEANING UP THE A CUT AND JUST STARTED THE C CUT (CURTAINSIDE) WHEN SPAR KS FROM THE MINER BITS STRIKING THE SANDSTONE CAUSED A FRICTIONAL IGNITION THAT LASTED APPROX ONE TO TWO SECONDS.
EAST MAINS, THE 008 MINER WAS APPROX 100 FT IN THE #9 ENTRY AND ENCOUNTERED A SAND STONE ROLL. SPARKS FROM THE BITS CAUSED A FRICTIONAL IGNITION( ORANGE BALL OF FLAME IN RIGHT COENER) THAT LAS TED 1-2 SECONDS. METHANE MONITOR WAS READING. SECTION IDLED UNTIL INVESTIGATION COMPLETE DETERMINED TO BE DUST IGNITION.
EAST MAINS, THE 008 MINER WAS APPROX 42 FT IN THE #6 ENTRY, AND THE SPARKS FROM THE BITS CAUSED A FRICTIONAL IGNITION IN THE RIGHT CORNER. THERE WAS ARANGE BALL OF FLAME FOR 1-2 SECONDS. SECTI ON IDLE UNTIL INVESTIGATION COMPLETED.
A ROOF FALL OCCURRED MEASURING 20' WIDE X 25' LONG X 6' HIGH, BEGINNING AT 1 CROSSCUT TO LEFT OF SS# 1555 AND 1 CROSSCUT OUTBY SS# 1552 #3 ENTRY. NO EQUIPMENT WAS INVOLVED, NO VENTILATION DIST URBED, NO INJURY OCCURRED AND THE FALL WILL NOT BE CLEANED UP.
A ROOF FALL OCCURRED MEASURING 20' WIDE X 25' LONG X 6' HIGH BEGINNING AT 1 CROSSCUT INBY SS# 1555 #4 ENTRY. NO EQUIPMENT WAS INVOLVED, NO VENTILATION WAS DISTURBED, AND NO INJURY WERE REPORTE D, AND THE FALL WILL NOT BE CLEANED UP.
THE EE WAS USING A BLOCK HAMMER TO LOOSEN A CLAMP ON A CHAIN. WHEN HE SWUNG THE HAMMER, IT GLANCED OFF THE CLAMP AND STRUCK HIS RIGHT KNEE.

THE EMPLOYEE ARRIVED AT WORK 11-1-02 WITH SWELLING TO HIS RIGHT KNEE. THERE WAS NO ACCIDENT, BUT HE WAS ADVISED BY MANAGEMENT TO GO TO THE ER FOR AN EXAMINATION. IT IS OUR OPINION THAT THIS IS AN OCCUPATIONAL ILLNESS CAUSED FROM REPETITIVE MOTION (BENDING & WORKING ON KNEES.)

THIS FALL WAS IN THE 2 NORTH MAIN AREA THAT WE HAD PULLED OUT OF ABOUT 6 MO.'S AGO. THIS FALL WILL PROBABLY NOT BE CLEANED UP AND DOES NOT INTERFERE WITH ANY AIRWAYS, ESCAPEWAYS OR TRAVELWAY S TO OR FROM THE ACTIVE 01/04 SECTION. THIS FALL IS ONE BREAK ABOVE SPAD #514 IN THE #6 NEUTRAL ENTRY IN A FAULTY AREA THAT WAS MINED IN THE 9THMONTH OF 2001/THE FALL IS 60-70'L X 3-8'H X 18'W
FALL WAS IN 2 NORTH MAIN AREA THAT WE HAD PULLED OUT OF ABOUT 6 MONTHS AGO. FALL PROBABLY WILL NOT BE CLEANED UP & DOES NOT INTERFERE W/ANY AIRWAYS, ESCAPEWAYS OR TRAVELWAYS TO OR FROM THE ACT IVE 01/04 SECTION. FALL IS AT SPAD #674 NEAR BULL CREEK AREA. IT FELL IN THE #5 TRACK NETURAL ENTRY & OVER TO THE BELT ENTRY. MEASURED 5-10'H, 18'W. DANGERED OFF W/CRIBS & TIMBERS.
