On 3/17/15 employee was operating a Wagner scoop. While driving the scoop he ran over some debris which caused him to strike his head against the cab of the machine injuring his neck. Employee was seen by a doctor on 3/18/15 and was released to come back to work. He again sought medical attention on 3/23/15 and was then taken off of work at this time.

Employee was using a stationary grinder to smooth off a piece of metal that had been cut. The piece of metal kicked and the employee's right thumb contacted the moving grinding wheel causing a laceration. The laceration required two stitches to close.

On 2-3-15 employee was descending a ladder when he inadvertently missed the last rung. As he stepped past the last rung and made contact with the ground he twisted his left knee. Employee continued to work while medical diagnostics and reviews were being done. He was taken off of work 4-30-15 for surgery to repair his left knee.

On 2/26/15 employee reported an injury to his neck that he claims happened on 12/17/14, when he struck a door frame with his head. He has missed several days between the reporting date & the alleged accident date. His first day missed following the date reported was 3/2/15. Investigations continue to determine if an accident actually occurred and if there is a legitimate injury.

EE injured his back on 12-11-14 as he was loading a ¾ chain onto a piece of underground diesel equipment. He had been scheduled to be seen by a physician on 12-17-14 for a separate non-related incident. During this visit he was taken off of work until follow up diagnostics could be performed. EE first day of work missed was 12-17-2014.

EE was pulling rock props down the longwall face and lifting them over the cable trays into the panline. As he was placing the rock props in the panline he felt pain in his lower back. EE continued to work until 11/06/2014 when he went off for surgery. His first day missed due to this incident was 11/06/2014.

Employee was in the process of re-setting a small set of ventilation doors on the Mill Fork compressor station. As he let go of the inby door, it shut on his left index finger causing a laceration and fracture to the finger. He returned to work on his next scheduled shift.

On 8-18-14 employee and a co-worker was lifting a portable pump onto a flat-bed truck. As they were lifting employee felt a pain in his right shoulder. Employee was seen by a physician and was scheduled for an MRI. The MRI revealed that he has injured ligaments in his shoulder that are going to require surgery to repair. His first day missed from this injury was 8-22-14.

Employee was watching the V-Belts in operation on a motor that operates the shaker for the Main West bin. After de-energizing the power he felt the V-Belts to determine their tension. Immediately after making contact with the V-Belt the shaft rotated and his finger was caught between the V-Belt and the pulley. The shaft then immediately returned releasing his finger.

Employee was walking to the tailgate to reset a faulted shield during the mining process. When he reached 125 shield a bounce occurred. The bounce threw material toward the shield employee was standing on. The material hit the bounce guard which resulted in employee being knocked over striking his ribs against the shield. This caused a small laceration to his spleen.

Employee was attempting to cut some holes in a plastic barrel using a reciprocating saw. As he was cutting one of the holes the saw kicked back. The kick back of the saw resulted in his pointer finger on his right hand contacting the saw blade causing a laceration that required five sutures to close.

EE was at his desk in his office and started to feel ill. As the signs and symptoms progressed and with his knowledge of medicine as a certified Paramedic he felt that he was experiencing a possible heart attack. EE initiated the emergency response system where he was taken to a hospital and it was confirmed that he did in fact have a heart attack.

Employee injured his shoulder on 2/26/14. While operating a piece of equipment he ran over a cement spill that had dried. This caused him to strike the cab injuring his shoulder. Employee continued to work up until 12/30/14 where he went off for surgery to repair the shoulder. Employee subsequently retired on 12/31/14.

On 2/26/14, employee filled out an incident report for straining his right shoulder. He continued to work until 7/30/14 at which time he had surgery to correct his shoulder problem, after receiving confirmation that his claim was accepted as a work related injury. The difference between the injury date, surgery date and reporting date was pending acceptance of this claim.

EE was in the process of tightening a take-up on the surface sampler belt using a wrench. As he was pulling on the wrench he felt pain in his left shoulder. EE continued to work up until 6-17-2014 when he had surgery to repair his shoulder. His first day of work missed due to this incident was 6-17-2014.

We received notification at the mine on April 3, 2014 that employee had exercised his Part 90 Miner Rights, from the miner. This was diagnosed in November of 2013 which is the date noted above for illness date. The difference between reporting date and date of illness is related to the date we were made aware of the option to exercise his Part 90 Miners Rights.

On 9-17-13 employee was attempting to lift a belt roller to release a chain and felt a pain in his right groin area. On 10-3 he was sent by his doctor and diagnosed with a hernia that would require surgery to repair it. Employee continued to work until 10-9-13, on 10-10-13 he had surgery. First day from work was 10-10-13

On 8-21-13 employee struck a piece of coal causing the steering wheel to spin straining his shoulder. On 8-26 he was seen by a Dr and released for work. On 9-4 he was taken off work pending an MRI. EE has previous neck and shoulder injuries and this case was under review by industrial to determine if it was a new or old injury. On 10-10 it was accepted as a new injury

Employee was removing a bolt from a filter housing using an 18 inch extension on a breaker-bar. As torque was applied to the breaker-bar the extension broke lacerating his palm against the broken extension. The laceration required two stitches to close.

On 6-18-13 EE claimed he slipped on loose coal straining his knee. He's been off work pending a medical evaluation. There was no witness to incident. There were implications this may not have been work related. On July 9 this claim was accepted as an industrial accident. Time difference between accident date and reporting date was pending investigation and acceptance.

On 6-3-13 employee filed a claim of repetitive fatigue to his right hand from using a dust hose attached to a bulk dusting system while dusting belts. Employee continued to work with this condition. Over time the condition worsened and on 10-28-13 he had surgery to correct the condition.

Employee continued to work with this condition while seeking a medical opinion about the symptoms that persisted. On 12-27-13 he had surgery to correct the condition. The first day of work missed due to this condition was 12-30-13.

Employee was in the process of replacing a top belt roller. As the roller was being slide into place it came in contact with a metal shim that flipped up and pinched his left little finger between the shim and the roller frame causing a tip fracture and a laceration that required stitches.

It was undetermined at this time if his condition was work related, on 8-29-13 it was accepted as an industrial accident, he is schedule for surgery 0n 9-12-13. The difference of the accident date and reporting date was pending acceptance.

On 1-3-13 employee was closing a metal door on a feeder chute at the mines tipple. While closing the door he strained his abdomen. On 1-18-13 he was seen by a Dr. and was diagnosed with a hernia that would require surgery to repair. On 2-12-13 he had surgery.

On 1-2-13 EE filed a claim of accumulative trauma from operating a jack hammer; he stated there was no specific event only that his elbows hurt. 2-4-13 he went to Dr. and was diagnosed with tendonitis. 4-3-13 he return to his Dr. and was taken off work. 6-17-13 this claim was accepted as an illness. Time off and reporting difference was pending a medical report.

Employee was placing a section of wire mesh on the atrs of a roof bolter and as he went to step onto the driller's platform, he missed the platform and his leg dropped approx. 2 feet below the platform. This caused him to strain his right knee. He was scheduled to see an orthopedic Dr. for an evaluation. The Dr. has now taken him off work.

A roof fall occurred just off the tailgate of the 25th West longwall face. The fall was approx. 2.5 feet thick, from 5 feet to 20 feet wide for a distance of approx. 60 feet. The fall blocked access off the tailgate entry. This fall was found at the start of the weekly examination at approx. 12:35 a.m.

On 9-21-2012 EE was installing cable truss bolts and as he went to install the cable in the truss harness he overextended his arm straining his shoulder. EE continued to work with this condition; on 4-25-2013 he had surgery to correct the condition. Last day he worked was 4-24-2013.

Employee was in the process of installing a bolt in the mine floor to secure a belt drive. As he was rotating the bolt, his left index finger got pinched between an air hose and the frame of the drill causing a laceration that required stitches.

Employee was in the process of rock dusting a belt line. The duster stop for a minute so he sat on a water line that was held up by chains attached to a rib bolt plate. The plate that the water line was hooked to broke and the water line fell landing on his ankle causing a fracture to his lower leg.

On 8-6-12 EE was moving a stopper and felt a pain in his knee. He continued to work; on 9/10/12 he was seen by a doctor and was taken off work pending a MRI. On 9-17 he had a MRI. On 11/15/12 he had surgery. The time difference between reporting and submitting report was pending a medical report that determined that this was accepted as an industrial injury.

On 8-3-12 employee was opening a material door and suffered a torn Achilles tendon. After a medical exam it was determined that sometime prior to opening the door he had injured his tendon, this injury is being submitted as an accumulative trauma type of injury. The time difference between when the injury occurred and the reporting date was pending a medical report.

The injured was recovering a screw that had fallen to the bottom of an electrical cabinet. The cabinet was top heavy while on his hands and knees attempting to get upright the cabinet was falling forward. He tried to stop the cabinets forward movement. His left hand was caught between the cabinet and the metal decking. He developed a contusion.

On 7/20/2012 employee was in the process of cutting down some old mesh. While cutting down the mesh he was struck in the head with a loose piece of top. He was seen by a Dr. on 7/27/2012 and was released to work that day. He continued to work while waiting for industrial to accept his claim up until his surgery date of 5/6/2014.

A roof fall was found off the tailgate entry of the (22nd West) of the 23rd West longwall. The roof fall was approximately 16 feet from the tailgate and was approximately thirty feet in length, twenty feet in width and approximately five feet in height. The roof fall block access off the tailgate.

On 4-23-2012 employee was traveling down the longwall face when he tripped and fell injuring his shoulder. He continued to work until March 28th 2013 at which time he had surgery to repair his shoulder. First day of work missed due to this injury was March 28th.

IEE and three other employees were in the process of lifting a metal side board into place on a feeder breaker. As one of the employees' s started to lift the side board it slipped and fell striking EE on top of his left foot causing a fracture.

EE was helping replace a section of ventilation tubing. As he was holding up the tube another miner was attempting to attach the tube hanger to the mines roof mesh using a bar. The bar slipped off the tube hanger and struck EE across the nose fracturing his nose. the fist day of work missed due to this accident was 2-27-2012.

Employee is a roof bolter, on 1-16-12 he was positioning a section of wire mesh when the mesh got hung up and he strained his shoulder. He continued to work but his shoulder never healed. Employee was seen by a doctor and it was determine he needed surgery. On 10-8-12 he had surgery to repair his shoulder.

On January 12, 2012 EE was walking and his feet got tangled up in a plastic band which caused him to fall. As he went to catch himself he injured his left shoulder. EE continued to work. On February 3rd, he went to the Dr. and was taken off work. The first day of work missed was 2-6-12.

On 10-10-2011 employee moved a shuttle car anchor. Later during the shift he started to have pain in his left shoulder. He had previous problems with his shoulder. On January 5, 2012 he had surgery on his shoulder. We received his medical report and it was determined that his injury was due to the event on 10-10-2011. The date of the report was pending a medical report.
