THE EE WAS HOLDING UP THE MINER CABLE AND WATER LINE FOR A SHUTTLE CAR TO PASS THRU A CROSSCUT WHEN THE ACCIDENT OCCURRED. EE RIGHT HAND WAS CAUGHT BETWEEN THE SHUTTLE CAR CANOPY AND THE MINER CABLE WHEN THE SHUTTLE CAR PASSED UNDERNEATH. THIS RESULTED IN A LACERATION TO HIS RIGHT INDEX FINGER. SUTURES WERE USED TO CLOSE THE WOUND. THE EE WAS WEARING METACARPAL GLOVES. NO LOST TIME
THE EMPLOYEE WAS TRAINING THE CONVEYOR BELT WHEN THE ACCIDENT OCCURRED. HE WAS LIFTING AN IDLER WHEN HE EXPERIENCED PAIN IN HIS LOWER ABDOMEN. EMPLOYEE DIAGNOSED WITH A HERNIA. SURGERY REQUIRE D TO REPAIR THE HERNIA.

EE WAS UNLOADING A CAR OF GARBAGE FROM A MINE CAR INTO THE BUCKET OF A 988D FRONT-END-LOADER. WHILE PERFORMING THIS TASK, A BONDING STRAP (METAL BAND) CONTACTED THE EE'S RIGHT ARM AND CAUSED A LACERATION REQUIRING SUTURES. EE REUTRNED TO WORK ON THE SAME DAY AND COMPLETED HIS SHIFT.
EE WAS OPERATING A JOY 10SC SHUTTLE CAR HAULING COAL FROM THE FACE TO THE FEEDER & RETURN. WHILE TRAMMING THE SHUTTLE CAR IN THE R CROSSCUT OFF #5 ENTRY [STATION SPAD #14884], THE EE REPORTED THAT THE SHUTTLE CAR STRUCK A DEPRESSION IN THE ROADWAY & CAUSED HIM TO BE JOLTED IN HIS SEAT. EE AND A COWORKER WERE ENGAGED IN PLACING TROUGHING IDLERS ALONG THE BELT ENTRY IN PREPARATION FOR A BELT MOVE. THE EE REPORTED EXPERIENCING PAIN IN HIS UPPER BACK. WHILE ASSISTING TO LIFT ON A TROUGHING IDLER. HE WAS UNABLE TO WORK ON 4-23-97 AS A RESULT OF THIS INJURY.
EE WAS HAULING COAL FROM #10 ENTRY TO BELT FEEDER(5TH SHUTTLE CAR IN THIS CUT). AS HE WAS TRAMMING SHUTTLE LOADED FROM FACE TO FEEDER, SHUTTLE STRUCK A DEPRESSION IN ROADWAY(#7 ENTRY) AND CAUS ED HIM TO BE THROWN AGAINST UNDERSIDE OF CANOPY OF OPERATORS COMPARTMENT. EE REPORTED THAT HE EXPERIENCED PAIN IN HIS NECK AS A RESULT.
GPF-THE EMPLOYEE ALLEGES TO HAVE COAL WORKERS PNEUMOCONIOSIS. AS A RESULT AN OP AWARD WAS GRANTED.
EE AND A COWORKER WERE ATTEMPTING TO MANUALLY EXTEND SLIDING EXTENSIONS ON ATRS T-BAR JUST PRIORTO BEGINNING ROOF BOLTING OPERATIONS. EE REPORTED EXPERIENCING A SUDDEN PAIN IN HIS LOWER ABDOME N AS HE EXERTED UPWARD PRESSURE. HE COMPLETED SHIFT BUT FAILED TO REPORT ON 10-15-96. THE INJURYWAS DIAGNOSED AS A HERNIA.

EMP REPORTED THAT HE STRUCK HIS HARD HAT AGAINST THE MINE ROOF WHEN HE STOODUP WHILE LIFTING THE CABLE AND WATER LINE. REPORTED PAIN IN HIS NECK.

EEREPORTED HE WAS JOLTED AND THAT HIS HARD HAT STRUCK THE UNDERSIDE OF THE OPERATOR'S CANOPY RESULTING IN A SUDDEN PAIN IN HIS NECK.
UNPLANNED FALLS OF ROOF OCCURRED AT 3 LOCATIONS (STATION SPAD REFERENCES #13621, #5390, AND FALLWERE IN OLD WORKINGS AND NEAR THEA REA WHERE PILLAR MINING WAS CONDUCTED. NO INJURIES AND NO DAMAGE TO MINING EQUIPMENT OCCURRED. MSHA NOTIFIED BY PHONE WHEN FALLS WERE DISCOVERED.
THE EMPLOYEE AND CO-WORKER WERE RECOVERING 60 POUND RAILS FROM AN ABADONEL PANEL. A WHEEL OF A BATTEYR-POWERED SCOOP STRUCK ON OF TH ERAILS INADVERTENTLY CAUSIN GHT ERIAL TO SWING AORUND AND STRIKE THE EMPLOYEE ON THE OUTSIDE OF THE LEFT LEG RESULTIG IN A CONTUSION. THE EMPLOYEE WAS WALKING APPRXO 12' INBY THE SCOOP AT THE TIME OF THE ACCIDENT.

HE DISCHARGED A LOAD OF COAL AT THE FEEDER AND WAS TRAMMING THE SHUTTLE CAR TOWARD THE FACE ARE A EMPTY. THE SHUTTLE CAR STRUCK A DEPRESSION IN THE ROADWAY (#7 ENTRY) AND THE EE WAS JOLTED IN THE OPERATOR'S COMPARTMENT. HE REPORTED EXPERIENCING PAIN IN HIS LOWER BACK.

THE EMPLOYEE STATES THAT SEVERAL UNRELATED EVENTS DURING THE PREVIOUS 26 YEARS AS A COAL MINER AREA THE CAUSE OF HIS DISCOMFORT. THIS CONDITION IS BEING REPORTED AS AN OCCUPATION ILLNESS.
EMPLOYEE WAS ATTEMPTING TO SPLIT A HALF-HEADER WOOD BLOCK BY STRIKING IT AGAINST A RAIL.THIS CAUSED THE HALF-HEADER BLOCK TO RICOCHET,STRIKING THE EMPLOYEE'S MIDDLE AND RING FINGER OF HIS LEFT HAND RESULTING IN A FRACTURE TO HIS RING FINGER.THE EMPLOYEE DID NOT MISS WORK AND THE EVENT SHOULD BE CLASSIFIED AS A DEGREE 6 AND CLASS 20.

EE REPORTABLE HE TRIPPED AND FELL WHILE ATTEMPTING TO DISMOUNT FROM THE OPERATOR S COMPARTMENT OF A DIESEL POWERED LOCOMOTIVE EQUIPMENT WAS STATIONARY AT TIME OF INCIDENT. EE REPORTED AN INJUR Y TO HIS BACK AFTER HE SLIPPED AND FELL AGAINST UA RAIL LAYING ON THE MINE FLOOR. EE FAILED TO REPORT TO WORK ON NEXT REGULAR SHIFT. EMPLOYEE REPORTED AN INJURY TO HIS LEFT KNEE. HE STATED HE KNEELED TO THE MINE FLOOR AND THAT A SMALL LUMP OF COAL BETWEEN HIS LEFT KNEE AND LEFT KNEE PAD RESULTED IN AN INJURY. NO VISIBLE INJ URY WAS DETECTED. THIS EVENT SHOULD BE CLASSIFIED AS A DEGREE 3, CLASSIFICATION 19 INJURY.

EMPLOYEE STATED HE WAS LOADING SUPPLIES ONTO TH T TOP OF A ROOF BOLTING MACHINE WHEN THE ALLEGED ACCIDENT OCCURRED. THIS EVENT SHOULD BE CLASSIFIED A DEGREE 3, CLASSIFICATION 09-HANDLING MATERIAL ACCIDENT.

EE REPORTED HE STRUCK HIS HARDHAT HEAD AGAINST THE UNDERSIDE OF THE OPERATOR S CANOPY OF A SHUTTLE CAR. THIS OCCURRED WHEN THE SHUTTLE CAR STRUCK A DEPRESSION IN THE ROADWAY. EE CONTINUED TO W ORK UNTIL 2-10-95 FIRST DAY MISSED WAS 2-13-95 EE DID NOT INFORM THE OPERATOR THAT THE ACCIDENT WAS THE CAUSE OF HIS ABSENCE FROM WORK UNTIL 3-3-95.

THE EMPLOYEE REPORTED PAIN IN HIS NECK AS A RESULT OF THE COLLISION.

EMPLOYEE BENT OVER TO PICK UPA HYDRAULIC JACK WHEN HE EXPERIENCED A SUDDEN PAIN IN HI LOWER BACK. THE HYDRAULIC CYLINDER WAS APPROX 6' IN DIAMETER BY 3' IN LENGTH AND WAS LAYIN GON THE MINE LFOOR. THIS ACCIDENT SHOULD BE CLASSIFIED AS A DEGREE 3, ACCIDNET CLASSIFICATION 09. HANDLING MATERIAL. THIS EMPLOYEE HAS A HISTORY OF BACK PROBLEMS.
