EMPLOYEE HAD TRAVELED A STAIRWAY TO THE SECOND LEVEL SEVERAL TIMES KNOWING A CROWBAR HAD BEEN PLACED ON ONE OF THE STEPS WHILE ASCENDING THE STAIRWAY HE REQUESTED ANOTHER EMPLOYEE TO REMOVE I T THE OTHER EMPLOYEE DID NOT REMOVE IT AND THE INJURED EMPLOYEE SLIPPED WHILE DESCENDING THE STAIRWAY
