Showing all 21EMPLOYEE WAS UNPLUGGING A FLY ASH SILO. HE LATER LOOKED INTO THE SILO TO SEE IF ANY MATERIAL WAS LEFT. WHILE LOOKING THROUGH THE INSPECTION OPENING SOME FLY ASH DISLODGED AND HIT HIM IN THE FA CE. HIS EYES WERE FILLED WITH ASH. HIS EYES HAD TO BE WASHED OUT.
EMPLOYEE WAS WASHING DOWN THE LOAD OUT HE WAS GOING DOWN THE STEPS FROM THE 5TH FLOOR TO THE 4TH & HIS FOOT SLIPPED ON THE STEP HE FELL AGAINST THE HAND RAIL & SUFFERED 3 BROKE RIB HE DID NOT SEEK MEDICAL ATTENTION UNTIL 04-20-95 THAT IS ALSO WHEN HE STARTED MISSING WORK DAYS
EE HAD BEEN MEASURING TRACK ON A DOZER HE WENT INTO THE CAB TO GET THE HOURMETER READING HE START ED DOWN OFF THE DOZER WHICH HAD NO HANDR AILS THE DOZER WAS WET & DIRTY HE PUT HIS LEFT HAND O N THE FENDER & HIS LEFT LEG ON THE PUSHARM WHICHWAS COVERED WITH COAL HE ST EPPED OFF WITH HIS RT FOOT WHEN THE LEFT FOOT SLIPPED OFF THE PUSHARM & HE FELL & LANDED ON THE GROUND ON HIS KNEES
EMPLOYEE WAS ATTEMPTING TO ALIGN A LINKAGE ARM TO A BEARING. A PRY BAR WAS BEING USED TO LIFT THE ARM INTO PLACE. THE ARM SLIPPED OFF OF THE BAR AND HIT THE MIDDLE AND RING FINGER TIPS ON HIS RIGHT HAND. THE FINGER TIPS WERE CUT AND REQUIRED TWO STITCHES ON EACH FINGER.
EE WAS HELPING EE (CONTRACTOR )REMOVE A SECTION OF METAL GRATING AT THE COAL ASH BUILDING AFTER THEY HAD LIFTED THE MET AL UP EE STUMBLED & EE LOST HIS BALANCE & FELL HE STRUCK THE METAL & CUT HIS HEAD BROKE HIS NOSE & BRUISED HIS SHOULDER THE CUT ON HIS HEAD REQUIRED STITCHES
EMPLOYEE WAS UNLOADING COAL ASH FROM A RAIL ROAD CAR. THE ASH WAS IN LUMP FORM AND HAND THE DOOR FOULED OPEN. WHILE TRYING TO GET THE DOOR OVR THE ASH THE DOOR MOVED ANDMASHED THE EMPLOYEE'S RIGHT INDEX FINGER CAUSING A LACERATION. STICHES WERE REQUIRED TO CLOSE THE WOUND.
EMPLOYEE WAS PLACING A LARGE WOODEN CRIB BLOCK UNDER A D9 BULLDOZER PUSHARM. WHEN PLACING A SECOND CRIB BLOCK IT SLIPPED FROM HIS GRASP, FALLING ON HIS LEFT HAND, CAUSING FINGER TO BE CUT.
REPAIRMAN AND OPERATOR WERE INSTLALING NEW TORQUE CONVERTER FILTERS ON FRONT END LOADER. REPAIRMAN WAS TIGHTENING FILTER CANNISTER WHEN THE ADJUSTABLE WRENCH HE WAS USING SLIPPED OFF NUT SURA FACE STRIKING OPERATOR IN HEAD.
SLIPPED ON PIPE THAT HAD MUD ON IT AT THE SCALE HOUSE.
EMPLOYEE WAS ATTEMPTING TO CLOSE A RAILROAD CAR DOOR WHILE USING A SLATE BAR AS A PRY.THE BAR SLIPPED AND THE ANGLED END OF THE BACK STRUCK THE EMPLOYEE'S RIGHT ELBOW.BRUISING IT.
EMPLOYEE WAS MAKING A FUEL STAND TO HOLD FUEL HOSE SO OPERATOR WOULDN'T HAVE TO GET OFF LOADER TO FUEL.EMPLOYEE WAS STANDING UNDER FUEL STAND AND THE HOSE CAME LOOSE FROM TOP OF STAND AND STRU CK HIM IN THE TOP OF THE HEAD;CUTTING THE TOP OF THE HEAD AND REQUIRING 9 STITCHES.
I.NAME DRIVING TRUCK OFF MOUNTAIN AT EXCESSIVE SPEED.TRIED TO DOWNSHIFT NEVER DID GET IT BACK IN GEAR AND BRAKES MALFUNCTIONED ,LOST CONTROL AND WENT OVER 20' EMBANKMENT BROKE LEG AND HAD 3" C UT ON FOREHEAD.
EMPLOYEE WAS LIFTING CONVEYOR PARTS FROM GROUND LEVEL TO CONVEYOR ABOVE WITH ROPE. STRAINED LOWER BACK WITH PULLING MOTION.
EMPLOYEE WAS STANDING ON 18" RAISED PLATFORM TO CUT HOLE ABOVE HEAD TO PASS CONDUIT THROUGH. WHEN HE STEPPED DOWN FROM THE PLATFORM, HIS LEFT FOOT SLIPPED ON A LUMP OF COAL, SPRAINING HIS LEFT ANKLE.
EMPLOYEE WAS ASSTISTING IN POURING CONCRETE WALL. WET CONCRETE SPASHED INTO HIS EYES. RESULTING IN CORNEOL ABRASION AND CASUTIC BURN TO RIGHT EYE.
EMPLOYEE WAS ASSISTING WELDER.FACE AND EYES WERE EXPOSED TO U.V. RADIATION RESULTING IN U.V. BURNS.
MOVNG CONCRETE FORMS AN D STRAINED LIGAMENTS IN L HAND.
EMP LIFTED A RA3LROAD TIE AND INJURED HIS BACK
THE EMPLOYEE SA3D HE WAS OPENING DOORS ON RAILROAD CARS IN ORDER TO CLEAN OUT FOREIGN MATERIAL FROM WITHIN. THE SLATE BAR, BEING USED AS A LEVER TO CLOSE DOOR,SLIPPED FROM CAR DOOR STRIKING EM PLOYEE IN LEFT 7OLLAR AREA. NO INJURY WAS REPORTED TO MANAGEMENT. ON 10/3/86, EMMPLOYEE WAS ABSENT FROM WORK. ON 10/7/86, HE REPORTED HE HAD BROKEN COLLAR BONE, BUT, DID NOT KNOW HOW OR WHERE
WHILE HAULING C2AL TO A STOCKPILE, DRIVER DROVE OFF ROAD INTO SHALLOW DRAINAGE DITCH ALONG RAILROAD TRACKS. TRUCK S LEFT FRONT TIRE HIT RAMP FOR TRACK CROSSING AND TRUCK BOUNED TO RIGHT CAME B ACK AND TURNED 2VER ON LEFT SIDE ON TOP OF CROSSING. DRIVER BROKE LEFT WRIST.
