I NAME WAS SETTING A PULL JACK FOR THE MINER AND HAD HIS HAND ON THE BOTTOM OF THE JACK.THE BOTTOM BUSTED UP CAUSING THE JACK TO SLIP AND IT CAUGHT HIS LITTLE FINGER ON RIGHT HAND AGAINST THE DEAD EYE AND SMASHED IT.AT THE HOSPITAL THEY X-RAYED IT AND SAID THAT HE WOULD NEED SOME SKIN GRFTED BACK ON IT.

I NAME WAS OPERTING THE MINER.THEY WERE BELTING UP AT THIS TIME.THEY HAD THE PULL ROPE HOOKED UP TO THE SOCKET OF THE MINER AND AS HE TIGHTENED UP ON THE PULL ROPE IT CAME LOOSE AND THE SOCKET STRUCK I NAME UNDER THE LOWER LIP CAUSING A LACERATION WHICH NEEDED 7 STITCHES.

UNSAFE ACT OF THE INJURED PERSON. HE HAD HIS FOOT IN A PINCH POINT. IRREGULAR BOTTOM. ONE OF THE DRUMS OF THE MINER WAS NOT WORKING PROPERLY ANDCAUSED THE MINER TO SHIFT TO ONE SIDE QUICKLY AN D CAUGHT HIS FOOT UNDER THE MINER SEAT.
