Employee was performing maintenance on the belt press. He attempted to lift the end of a roll with a come-a-long when he felt a sharp pain in the right shoulder. The pain persisted the rest of the shift with some tingling in his right arm and hand. He went to the chiropractic doctor the next day.

Mechanic was hammering on a drill bit on a Drill Tech DK45 highwall drill with a sledge hammer when a metal fragment struck the mechanic and lodged in the left side of his jaw. Employee went to the hospital and the object was removed. Employee reported to work the next day.

The entry was previously supported with 5' torque tension roof bolts when developed 08/03. There is 400' of cover in this area. All approaches to the fall were protected with posts

Injured was standing on the belt rail of the lowlow when he lost his footing and fell, striking his shoulder on the low low rail. The injury was not reportable until he had surgery on the shoulder on 9/16/05.

An unintentional roof fall was discovered 05/19/2005. Fall measured approximately 6' high X 100' Long X 18' Wide. The intersection was supported with 6' fully resin grouted tension bolts during development on 06/2001.

Injured was assisting in unrolling 2" plastic pipe for installation. The end of the pipe had been secured to the mine roof by tying it secure. The pipe came loose and struck the injured in mouth causing injury to his right upper lip. Five sutures were required.

EMPLOYEE WAS SITTING IN A STOPPED MANTRIP WHEN ANOTHER MANTRIP FAILED TO STOP, SLIDING INTO THE STOPPED TRIP. THIS CAUSED THE EMPLOYEE TO STRIKE HIS HEAD AGAINST THE CANOPY OF THE MANTRIP, CAUSING PAIN IN HIS UPPER BACK & NECK AREA.

An unintentional roof fall was discovered at 7:10 AM on the mains section. The fall was in the #1 entry, 1 break inby spad #3765. The fall measured 5'H x 20'W x 18'L. Fall was investigated by MSHA inspector at 7:10 AM 4-6-05, and by State inspector at 8:40 AM 4-6-05. The fall occurred in a 3-way intersection.

Injured was struck by a rock which fell out around the drill steel. The rock, which had the steel through it, spun down the steel, broke in half causing a piece of the rock measuring 8" by 8" by 3" thick to fly striking the person in the mouth.
