Employee was performing maintenance on the belt press. He attempted to lift the end of a roll with a come-a-long when he felt a sharp pain in the right shoulder. The pain persisted the rest of the shift with some tingling in his right arm and hand. He went to the chiropractic doctor the next day.

Mechanic was hammering on a drill bit on a Drill Tech DK45 highwall drill with a sledge hammer when a metal fragment struck the mechanic and lodged in the left side of his jaw. Employee went to the hospital and the object was removed. Employee reported to work the next day.

EMPLOYEE WAS LIFTING A TABLE BUSHING OUT OF THE MECHANIC'S TRUCK. BUSHING SLIPPED & CAUGHT HIS FINGER BETWEEN THE BUSHING & THE TRUCK BUMPER.

Employee was working on dozer and felt a pop in his right shoulder. Went to doctor next day and diagnosis was rotator cuff injury.

Employee was helping lift a piece of feeder chain when he felt a sharp pain in chest and lower back. Employee had suffered a severe burn injury of the upper chest from an earlier accident.

UNPLANNED IGNITION AT 2 LOCATIONS CONSUMING 1/4 & 1/2 ACRE OF GRASS NEAR ABANDONED PREP. PLANT. APPEARS TO BE CAUSED BY ELECTRICAL SPARK THAT FELL ONTO DRY GRASS.

Employee was pushing refuse with a cat D8 dozer. He stopped to spot the refuse truck and started to get a burning, watering sensation in his left eye. He finished his shift and went to hospital and had a metallic foreign object removed from his eye.

Injured was shoveling on the belts and began experiencing severe lower back pain

Unintentional roof fall in H Mains, No. 7 entry, inby spad 10398. The fall was discovered by General Mine Forman, at 7:15 AM. Fall measured approximately 80'L X 7' H X 18' W. The entry was previously supported with 5' torque tension roof bolts when developed 08/03. There is 400' of cover in this area. All approaches to the fall were protected with posts

Injured was standing on the belt rail of the lowlow when he lost his footing and fell, striking his shoulder on the low low rail. The injury was not reportable until he had surgery on the shoulder on 9/16/05.

EMPLOYEE WAS INSTALLING A ROOF BOLT IN AN OVERCAST AREA WHEN A PIECE OF ROCK (RIB) FELL & STRUCK EMPLOYEE ON THE HEEL OF HIS RT. FOOT. ACCIDENT OCCURED 45' INBY SPAD #3928 ON MAINS SECTION.

An unintentional roof fall was discovered 05/19/2005. Fall measured approximately 6' high X 100' Long X 18' Wide. The intersection was supported with 6' fully resin grouted tension bolts during development on 06/2001.

Injured was assisting in unrolling 2" plastic pipe for installation. The end of the pipe had been secured to the mine roof by tying it secure. The pipe came loose and struck the injured in mouth causing injury to his right upper lip. Five sutures were required.

Employee was carrying a receiver for a rail runner mantrip and stepped on a large piece of stone in pit area with right foot. Sharp pain and "pop" was felt in arch of right foot.

AN UNINTENTIONAL ROOF FALL WAS DISCOVERED ON THE 2 RIGHT PANEL IN THE #3 X #2 CROSSCUT. 1 BREAK INBY SPAD #3865. FALL MEASURED 10'H X 17 1/2'W X 25'L. THIS MINER UNIT (003-0) WAS PUT INTO NON- PRODUCING STATUS AND MOVED FROM THAT AREA.

EMPLOYEE WAS SITTING IN A STOPPED MANTRIP WHEN ANOTHER MANTRIP FAILED TO STOP, SLIDING INTO THE STOPPED TRIP. THIS CAUSED THE EMPLOYEE TO STRIKE HIS HEAD AGAINST THE CANOPY OF THE MANTRIP, CAUSING PAIN IN HIS UPPER BACK & NECK AREA.

An unintentional roof fall has occured on the B2 East section, MMU 017-0 at Sta. 8+41. The area was supported with 5' resin bolts through out and 10' cable bolts in the intersection. H- 10' W- 20' L-50' No injuries or equipment damage to report

The weekly examiner discovered an unitentional roof fall in the #1 entry of C-Mains panel. The measurements are: H-6' W-24' L-18' Area was supported by 5' resin Area has been dangered off

WAS LIFTING A SCOOP TIRE FROM A MAINTENANCE JEEP WHEN HE FELT A PAIN IN HIS LOWER BACK.

An unintentional roof fall occurred on the 2nd right panel, #6 entry, 1 block inby spad #3810. The fall measured 8'H x 17-1/2'W x 55'L. The fall occurred inby the loading point of section.

Injured was struck on left foot by falling roof rock when he swung the head in to install the inside bolt on pattern.

Injured was carrying a belt return roller when he stepped on a loose rock causing him to slip, twisting his back

An unintentional roof fall was found at approximatley 6:48 pm. The fall measured 5'hx30'wx25'l. The fall was dangered off, Roof Fall Cleaning Procedures were reviewed and posted at the fall and proceded with the clean-up. The fall was in the #6 Entry, inby spad station #3756, Mains Section.

Injured was lifting on a roof bolter pump motor to turn it around in the scoop bucket when he felt a "pop" in his left abdomen area.

WHILE WALKING THE #7 BELTLINE THE INJURED STEPPED ONTO A PIECE OF BELT STRUCTURE & FELL TO THE GROUND, TWISTING HIS RT. ANKLE. HE WAS TRANSPORTED TO THE HOSP. BY AMBULANCE, HE WAS TREATED & RELEASED. HIS RT. ANKLE WAS BROKE & IT WAS IMMOBILIZED WITH A REMOVEABLE WALKING CAST.

EE WAS CARRYING DESK AT UPSHUR PROPERTIES (ANKER) FOR THE SAGO MINE'S NEW OFFICE FACILITY WHEN HIS HAND WAS CAUGHT BETWEEN OFFICE DESK AND FILING CABINET. THE LACERATION REQUIRED 8 STITCHES.

EE WAS PLASTERING A WALL WITH B-BOND AND GOT B-BOND ON HIS ARMS AND LEGS, CAUSING BURNS TO ARMS AND LEGS.

An unintentional roof fall was discovered at 7:10 AM on the mains section. The fall was in the #1 entry, 1 break inby spad #3765. The fall measured 5'H x 20'W x 18'L. Fall was investigated by MSHA inspector at 7:10 AM 4-6-05, and by State inspector at 8:40 AM 4-6-05. The fall occurred in a 3-way intersection.

Injured was struck by falling rock from between the bolts. Bolt spacing was with requirements of plan

Injured was straightening a roof bolt while installing it in the drill hole when he straiined his back

AN UNINTENTIONAL ROOF FALL OCCURRED ON B-SOUTH MAINS IN CROSSCUT 4-5 AT STA. 13278. PRIMARY ROOF SUPPORT - 5' RESIN. HEIGHT-6'. WIDTH-18'. LENGTH-25'. ADDITIONAL SUPPORT WAS ADDED IN ADJOINING ENTRIES AND ROOF CONTROL PLAN CHANGED TO ADDRESS THE CONDITION.

(2ND OF 2 ROOF FALLS DISCOVERED ON 3/15/05.) An unintentional roof fall has occurred on B-Mains East in the #2 entry at station 13196. Height - 5', Width - 16',Length - 20'. Roof Support in area - 5' resin.

(1ST OF 2 ROOF FALLS DISCOVERED ON 3/15/05.) An unintentional roof fall has occured on the old West Mains belt entry in the intersection at station #12968. Height - 6', Width - 20', Length - 18'. Roof support in area - 5' resin.

EE called supervisor 3-12-05 that he had pulled a muscle & could return to work on 3/15/05. He also told supervisor he had pulled the muscle lifting a tire.

Injured was opening door for scoop to go through when a piece of rock fell striking him on left shoulder causing a bruise.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #4 ENTRY IN THE L-MAINS, AT SPAD #10849. IT MEASURED 6' HIGH X 18' WIDE X 30' LONG.

An unintentional roof fall was found @ 12:30 am , march 4, 2005. the fall measured 22' long, 19' wide, 5' high. fallis located 1 break inby spad #3680 on mains section. fall was not inby loading point.

Injured was struck by a rock which fell out around the drill steel. The rock, which had the steel through it, spun down the steel, broke in half causing a piece of the rock measuring 8" by 8" by 3" thick to fly striking the person in the mouth.

Employee was attempting to "free" a shuttle car cable that had caught on the section power center when tension on shuttle car took up slack on cable which in turn struck him in the face causing a cut below lower lip, scraped bridge of nose and forehead.

Injured had just finished assisting in the hanging of the cm and roof bolter cables when he began to experience pain in lower back and right leg.

An unintentional roof fall occurred at the south mains spur in a x cut developed 3/31/99, height-7' with-17'-length-25'-bolts 5' resin.

An unintentional roof fall occured. The area has clay a venvin it and same unconsolidated roof material. bolts installed were 5' resin 1 10' cable bolts height 7' width 18' length 25'.

EE WAS PERFORMING SERVICE WORK ON THE JOY MINER ON THE MAIN SECTION WHEN A ROCK FELL FROM BETWEEN BOLTS STRIKING EE LOWER LEFT LEG CAUSING BRUISE ANDSCRAP AREA TO CALF
