While performing his normal duties as a working Supervisor employee experienced pain/swelling in both knees. Employee returned to work his next shift however the following day he received medical attention of which he was diagnosed as having prepatella bursitis and consequently started missing work on 08/25/2010 making this reportable.

Employee was removing a block wall that was used to store mud behind it in order to be dried and later removed from the mine via Scoop. As he started to remove blocks some mud came loose thus pushing employee's leg against the blade of the Scoop.

Employee was installing pin on a bridge when he caught finger under the pin. Employee did not miss any work however he did go to a medical facility to get an x-ray done and it was determined that it was fractured therefore making this accident a reportable.

Employee was drilling hole on a motor base when the drill bit jammed in hole. He reversed the drill and while pulling back out on the drill the drill struck employee's mouth thus breaking his front teeth. Employee is getting necessary dental work done.

An unplanned roof fall occurred in the G - Butt rooms from #1 entry to #3 entry at spad #1092 (#2) entry. Fall was approximately 18' wide, 130' long, 10' high. Area was bolted with four (4) foot and six (6) foot fully grouted bolts and with ten (10) foot cable bolts throughtout. Area was timbered and dangered off.

An unplanned roof fall occurred in the No. 2 entry of G - Butt. Fall was located one (1) x-cut outby survey station #994. Fall was determined to be approximately 18' wide, 40' long, 8' to 10' high. Area bolted with four (4) foot fully grounted resin bolts with eight (8) foot to ten (10) foot cable bolts installed throughout the area. Area posted and dangered off.

ATRS and safety post was set. Rock came loose from the right rib, striking the employee on the lower back. (Rock size was approx. 5'x3'x5" thick)

While performing his normal duties as a Supervisor employee experienced pain in knee. Employee did not request report be made out until 1/6/09. Employee worked until 2/2/09 at such time he went off. Initial investigation concluded this was not work related. After further investigation it was determined on 2/23/09 it was work related and now becomes reportable.

Did not affect any ventilation but did cover about 35' of belt and structure. Will not be cleaned up. Area was bolted with 4' compression bolts and was also center posted.

An unplanned roof fall occurred in the belt entry (#3) 1 x-cut outby survey station #8. Fall was approximately ten (10') high, nineteen (19') wide and thirty five to forty feet (35' to 40') long. Area was bolted with four (4') fully grouted bolts with x-bars installed as per our approved initial roof control plan. Fall will be cleaned up. Did not affect any ventilation.

Employee had lowered the head of the roof bolter to remove drill steels. The finisher steel had stuck in the roof. As the employee was putting the pusher steel away, the finisher steel fell out of the roof, striking the employee on the ring finger of his left hand.

An unplanned roof fall was discovered in the return air course in a room panel off 10 Right Main.This fall is located at spads #5241 and 5242. It is approximately 10 feet high and 60 feet long. The area was supported with 6 foot, fully grouted resin rods. The area was dangered off. The fall did not affect the ventilation at this mine.

EE had just helped a co-worker set a "J" channel and set legs under the "J"channel.Co-worker then dropped the ATRS and the legs that supported the "J"channel apparently kicked out allowing the "J"channel to fall striking EE on side of head & shoulder.EE was placed on restricted.He returned to full duty on 04/21/2008.
