Employee was walking beside a scoop, he tripped and fell into the blade of a bow saw lying on the scoop. The fall resulted in a laceration of the top part of his left hand requiring stitches.
A roof fall occurred in the last open x-cut in #5 entry, measuring 120'x18'x10' thick. Area was bolted with 3/4"x72" tension rebar with T3 roof channels on 42" ctr's. 2-10' long cable bolts are between each row of roof bolts. X-cut had been centered jacked with 100 ton jacks. Poor immediate roof with over mining at 60' and 1000' of overburden were contributing factors.
EE was pulling drill steel out of top and fractured his right little finger between drill steel and drill pot.
Employee was changing crawler take-up on Joy 14CM15 miner. The gathering head pan was chained up. He placed himself under the gathering head pan to push take-up in place with his feet. The chain broke causing him to be caught between the gathering pan and mine floor. He had fracture of nasal bone & 3 ribs, sprain of sacroiliac & pelvis, pneumothorax trauma & liver laceration.
EE was backing the miner out of the cut of coal when the cable went under the miner. He went up to remove cable from under the miner and accidentally bumped the remote control box against the cable stand-off, activating the tram lever and then the panic bar. He was caught between the coal rib and the stand-off of the machine.
EE was hanging a ventilation curtain and cut his left middle finger, requiring stitches, on a CAB brattice hanger.
Employee was stepping into scoop deck when he slipped causing him to twist his back. He was at #9 break on the #3 left roadway when the accident occurred.

A roof fall occured in a 3-way intersection in #1 entry at spad no. 11248, measuring 150' x 20' x 12' thick. The area was roof bolted with 3/4" x 6' tension rebar roof bolts with several 10' long cable bolts with T3 channel.
EE was cutting metal band on a pallet of Heintzmann jacks. When the band was cut, ajack rolled off the pile and struck his left foot.
A roof fall occured in a 4-way intersection measuring 70' x 20' x 12' thick. The area was roof bolted with 3/4" x 6' tension rebar roof bolts with a few 8' to 12' long combination roof bolts supplemented in.
A roof fall occurred in a 4-way intersection at spad no. 11278 measuring 60' x 20' x 12' in thickness. The area was bolted with 3/4" x 6' tension roof bolts. At least 8 - 10' long cable bolts were installed in the area also. No injuries occurred.

EE was pushing/installing a 16 foot cable roof bolt when he felt pain in his right shoulder.
EE was moving a set of cable crossovers. He felt pain in his lower back and the left side of his middle and upper back
EE was operating roof the bolter on the operator side, standing on ground. The operator man-up platform, which was chained up, fell striking his left knee.

A roof fall measuring 40 feet by 18 feet by 4 thick was discovered in the no. 7 entry at 83 break. Jacks and timbers were set and the fall will not be cleaned up.
A roof fall occurred 50 feet inby spad no. 11201 in the #5 entry in a four way intersection. The roof fall measured 40' x 20' x 10' to 12' in thickness. The area was roof bolted with 3/4" dia. by 72" long tension rebar roof bolts and a few 10' to 12' long cable roof bolts. No injuries occurred.
EE alleged experiencing stomach muscle strain after lifting on roof bolter T-Bar. Did not start missing work until 8/18/11.
A roof fall occurred 120' inby spad #11035 in crosscut between #5 & #6 intake entries. The fall measured 50' x 20' x 6' thick and was bolted with 6' long by 3/4" dia. tension rebar roof bolts. The area had previously been breakered off with 100 ton Heitzman jacks due to deteriorating roof conditions.
EE was making a cable splice, knife slipped and cut palm of left hand, requiring sutures.
A roof fall occurred 300' outby spad No. 10952 in the return air course. The fall measured 30'x 20'x 10' thick and was roof bolted with 3/4" dia., 6' long tension rebar roof bolts with 8"x8" plates with rock shields and supplemented with 10' long cable bolts. The fall may have occurred due to over mining approximately 70' above.
Employee was repairing the high voltage cable after a copper theft at the surface substation. He dropped the PLM coupler and mashed both thumbs.
Employee slipped and fell on wet slick bottom and injured his back.
A roof fall occurred in an intake aircourse near spad No. 10819. The fall measured 120 ft. in length, 20 ft. in width and 9 ft. in thickness. The area was roof bolted with 3/4" diameter 6 ft. and 8 ft. tension rebar roof bolts. The fall was probably caused by overmining abutment stress. No injuries were involved.
EE was cutting timber with a hydraulic chain saw. His hand slipped and landed on the chain requiring stitches.
A rock fell from the mine roof, striking the ATRS of the Fletcher roof bolter. When the rock hit the ATRS, employee was positioned near the rear of the bolter. The rock falling onto the machine, bounced him off the machine onto the mine floor with possible rib fractures.

EE was replacing a bottom belt roller. When he bent over to take it out he felt pain in his back. The pain caused him to slip in the mud and fall, straining his back, left hip and knee.
EE picked up a bag of rock dust, twisted to put on Boss Hoss. Alleged pain in his lower back. E-Doc #14116925 that was submitted 5/10/11 for alleged back injury on 4/28/11 was determined by Workers Compensation (BrickStreet) to have sufficient inconsistencies and contradictions to concluded that he was not injured in the course of and result of employment.
EE bent over to pick up his miner cable and felt pain in his lower back.

EE was moving roof bolt resin when a rock fell and struck him on his head and back. He returned to work on 3/31/11 but did not return to work on 4/4/11 due to injury.

A roof fall measured 12'long x 16' wide x 6' thick occurred in the #6 entry in the escapeway between cuts 188 & 189, 3600' outby the face area. The area was bolted with 6' tension rebar bolts and supplemented with 12' cable bolts on 16' centers. This area is under stress from over mining. Some water was present in the roof.
EE was cutting nylon rope cable hangers with a utility knife, when the knife slipped from his right hand it struck his left hand, causing a thumb laceration.
EE was loading supplies on the roof bolter, when a rock fell, striking him on top of head and shoulder.

Employee "ALLEGES" that while hanging a miner cable some foreign material (rock dust) fell into his right eye. Subsequent investigation revealed the employee actually had rust in his eye from an off the job accident. The employee's workers compensation claim was denied based on this fact.
