Employee was exiting loader, using steps, one step was frosty, and EE slipped and fell 8-9 feet. EE's ankle is broken in 3 places, and EE received several stitches in EE's head. EE will be seeing a surgeon on Monday Dec. 12, 2022, to determine if surgery is necessary.

Storage Trailer was set on fire by unknown vandals. Storage trailer and contents were completely destroyed. No injury or illness ocurred.

Employee was cleaning around swingers on dragline with loose rag. Loose rag caught and was pulled through the guard. This action also pulled employee's hand up to guard.

Employee was welding on an R-90 Rock Truck. Spark from torch went down back and ignited clothing. Employee was burned on back and under left arm.

Machine was parked in parking lot idle at the time of the accident. Operator was cleaning window on drill rig cab. Wind caught door and blew it shut on his right hand severing tip of index finger to first knuckle.
