Employee was walking in an area of low head room, between 2 screens, and walked into a lighting fixture striking EE's head. The employee continued to work until 1-25-2016, when EE saw a doctor and was diagnosed with a compressed nerve.

Employee was pulling on chain to move hoist. The employee encountered heavy resistance while pulling chain, causing left shoulder strain. Employee has continued to work with no restrictions. He had his 3rd physical therapy treatment on 10/22/2014 making this a reportable accident.

While moving a piece of pipe the employee felt a pull in his shoulder. The employee saw a doctor and received a prescription for Physical Therapy. The 1st PT session was on 8-19-14. His 3rd PT session was on 8-26-14. This accident is now report able. The employee has continued to work full time, full duty.

Employee was stepping down off a 1 foot high platform and tripped over a hose. Employee landed on floor striking his left knee, left hip, and left shoulder. The employee has been diagnosed with a tear in his labrum and a partial rotator cuff injury.

Employees was removing wire ties that were wrapped around a conduit with a knife.When the knife cut through the wire tie the employee's hand pulled back and he cut the thumb on his left hand. The thumb was cut by a sharp edge on the next wire tie.

Employee slipped and fell while walking on snow/ice covered walkway. Employee complained of left shoulder, and left knee pain. Employee notified employer of fall on 2-1-2013. Employee declined medical attention at that time. Employee sought medical advice in April of 2014. Employee had 3rd physical therapy appointment on May 14, 2014. This accident is now reportable.

After MRI on 02/13/2013 employee diagnos ed partial tear of rotor cuff. Physical therapy prescribed. Employee had 3rd pt on 03/04/2013. Accident now reportable.

EE was climbing down off of a coal screen. He extended his legs over side and was unable to reach floor. His feet were approx.6" from touching. He let himself drop the 6". His left leg touched first. When this occurred he felt a sharp pain and a pop in his left knee. He stated there was no slip/trip hazard. An examination by a physician found he had a medial meniscus tear.

Employee was driving a refuse truck, on loan from R&L development. This truck rides rough. The rough ride and rough conditions on refuse pile combined to give employee a bouncy ride. Employee drove the truck all shift. He completed his shift and left. He woke the next am to lower back pain that radiated down his legs making walking hard.

Employee was helping to lift a small flat cart from the 2nd floor to the 2-1/4 floor when he experienced pain in his right side. Employee worked his next scheduled shift, reported the accident on 12/1/2009, saw a doctor on 12/16/2009, and missed 12/18/2009 for a CT scan. He reported back to work for his next regular shift on 12/21/2009.

GSI Geo-synthetics (sub contractor) EE was traveling across jobsite on ATV. EE attempted to cross small ditch, when ATV flipped over backwards. The ATV was loaded with sandbags on the rear rack. This offset the balance of the machine, contributing to the turn over.

While walking across the fourth floor level, employee hit a pipe support with his head, causing a cervical sprain and a headache.**Accident was not reported until May 27, 2009. Employee was seen at hospital ER on May 28, 2009. He missed May 29th & June 1st and returned June 2nd at full capacity.**

EE was on site to clean underground piping with a water jet via an open manhole. A hose reel guide was installed overtop of the open manhole to allow the hose to move freely. EE was standing on top of the hose reel guide when it moved him and the guide fell into the open manhole. EE caught himself on the sides of the manhole, causing contusions to both arms and left shoulder.

Employee was lying on the floor, on his back, using an electric angle grinder to grind the end of a piece of pipe. A piece of foreign material entered his right eye causing a corneal abrasion. Employee was wearing bi-focal safety glasses with side shields and a full face shield. The abrasion required prescription eye drops.

Employee lifted one-half of the drive sprocket from a magnetic separator drum to move it. The magnet inside of the drum pulled the sprocket half to it, catching his right hand between the drum and the sprocket. Employee received a laceration of the right thumb requiring sutures, and contusions of the right index finger.

Employee was walking across the ground floor of the Preparation Plant underneath an elevated conveyor belt. While going under the belt, he struck his head on a piece of angle iron support structure. Employee was operating a Volvo A40D articulated hauler (2001). He was climbing the steps of the machine in order to clean the windows. Employee grabbed onto the handrail, while climbing, when the handrail broke off at the base, causing employee to fall to the ground.

Employee was bending a piece of 1/4" x 2" steel strap around a piece of pipe. He lodged one end of the strap between the pipe and a pipe hanger, and was heating the strap with a torch and hitting it with a hammer. The strap came out of the area between the pipe and pipe hanger and struck the employee in the mouth.

Employee was operating a Volvo A35C articulated hauler, and was bitten by an unknown insect on the left elbow area. Three days later, he requested to see a doctor, and was diagnosed with a localized reaction, and an infection. He has been seen by a doctor four times to date, and received three injections for infection and swelling, as well as a prescription to be taken daily.

EMPLOYEE WAS OPERATING A BOBCAT SKID LOADER, CLEANING THE FLOOR IN THE VEHICLE MAINTENANCE BUILDING. UPON COMPLETION OF THIS TASK, HE PARKED THE BOBCAT. WHEN HE EXITED THE MACHINE, HE FELT SEVERE PAIN IN HIS LEFT KNEE.

EE WAS SUPERVISING 2 EMPLOYEES DISASSEMBLE THE FEED PIPE TO 50750 FINE COAL CENTRIFUGE IN ORDER TO UNPLUG IT. THE PIPE IS CONSTRUCTED OF URETHANE & HELD TOGETHER WITH SLIP FIT COLLARS & CLAMPS. THE PIPE WAS BEING TAKEN APART AT A COLLAR. THE BLOCKED SECTION PULLED OUT OF THE COLLAR AFTER THE CLAMP WAS REMOVED. THE PIPE FELL STRIKING EE ON HIS RIGHT SHOULDER.
