A ROOF FALL WAS FOUND ON 10-25-91.THE FALL WAS LOACTED APPROX 2400' FROM THE FACE AT SURVEY STATION #58 BETWEEN THE NO 6 TO 7 HEADINGS.THIS AREA HAD BEEN DETERMINED TO BE ADVERSE DURING MINING AND ADDITIONAL LENGTH BOLTS AND POINT ANCHOR BOLTS HAD BEEN INSTALLED.THE ROOF CONTINUED TO DETERIORATE AND WAS CRIBBED AND DANGERED OFF PRIOR TO FALLING.THE ROOF WAS IN SLIP FORM APPROX 11'H
