Injured suffered a ruptured eardrum while at work, initially injury was thought to have been caused from sinus pressure. On 8/4/2014 injured was seen by a specialist who determined the cause of the rupture to have been caused by a foreign body entering the ear.
Employee was installing split steel and didn't know his glove was caught. Employee went to Good Samaritan Hospital sustaining a fracture to his finger which requires surgery and will be off work for an uncertain period of time.

An unintentional roof fall occurred on Unit 4 (MMU 008), Crosscut 13 Entry 6-7. The roof fall measured approximately 50' in length, 18' in width, and 8-10' in thickness. A 103(k) Order (9033214) was issued June 15, 2014, by MSHA Inspector.
Employee was using a utility knife and slipped cutting into his leg. Employee immediately went to Good Samaritan Hospital receiving sutures.
An unintentional roof fall occurred on the 3 North 3, Crosscut 26 Entry 5 Beltline. The roof fall measured approximately 48' in length, 18' in width, and 6' in thickness. A 103(k) Order (8444663) was issued May 20, 2014, by MSHA Inspector.
Injured was pinned between the movable roller and the take-up frame after putting a come-along on the roller to free it. The roller broke free and struck the injured knocking him to the ground and proceeding to run over him causing severe injuries.
Employee was hammering to remove a bit from the No. 1 CM when a sliver came from the bit entering employees thumb. Employee went to hospital receiving sutures to his thumb.

Employee was directing his co-workers when a band was cut holding the prop setters together and the props rolled onto employees foot. Employee sustained a fractured foot and returned to work on restricted duty the following work day.
Employee was breaking a steel band when it sprung back cutting his arm. Employee required sutures and returned to work the following work day.
Injured employee filed accident report on 3-21-14 stating that he had been cut on the back of his left hand while getting roof bolt supplies. He received 3 stitches to close the wound and had a severed tendon to his ring finger.
Employee was in supply yard loading his equipment when he smashed his finger. Employee required sutures and returned to work the following work day.

Injured was operating a scoop, when he got out of the scoop his back begin to hurt. He does not recall a specific incident that may have caused his back to hurt.

Roof Bolter on Unit#5 was trammed up on the right foot of EE by another ee who was at the side tram controls. EE was leaning over the mid section of bolter on the right side of bolter. He was cleaning off space for some bolts and did not hear bolter start up due noise from forklift.
Employee was hanging curtain while a co-worker was striking pin steel against the bolter. Co-workers steel flew out of his hand striking employee in the face.
Injured was placing wire mesh on the roof bolter and cut tip of his right ring finger.
Employee was hanging cable when he strained his back.
Injured was bolting in the #9 entry using a split steel and went to put in second steel and missed the chuck. This caused the steel to flex and contract striking his right hand index finger.
Employee was pulling on a jumper cable when it smacked him in the mouth.
A shuttle car driver was tramming through a crosscut from Entry # 5 to Entry # 6. The car driver stopped spoke to the victim. The victim proceeded past the car, traveled through a curtain into the intersection of Entry # 6. The victim told the car driver he was clear. The car proceeded through the curtain and struck the victim between the shuttle car and the left rib.
Was guiding steel into chuck. Misalignment occurred causing a springing action of steel. Tore a laceration into left hand.
Employee was driving a mantrip when the tire blew out causing the mantrip to abruptly stop into the arches. Employee struck the steering wheel with his safety glasses causing a laceration to his nose that required sutures.
Employee was operating the shuttle car as he was moving the curtain for better visibility his arm got caught cutting his arm which required sutures.
Employee was tramming the bolter when he lost his balance falling and trammed over his foot.
Employee was a passenger in mantrip traveling out when the trip hit the rib and smashed his finger.
Employee stepped between scoop bucket and rib as it turned it pinched his foot.
