Employee was attempting to hook a pump to the back of a truck. The pump rocked back and forth and the tongue fell, striking the employee's left foot. EE continued to work until EE went to the Dr. on 8-11-18. EE's first day missed was 8-13-18.

EE was walking to the work area when he stepped in a hole of water. He worked with wet socks & boots, this may have caused an abrasion on his foot. Mgt. wasn't notified until 9-25-2014 of this event. EE has a med. Cond. (diabetes) that may contributed to this alleged event. Mgt. is protesting the validity of the event with workers comp.

Employee was dragging de-energized high voltage cable to the 5b belt head kva. As he was walking with the cable over his shoulder, he slipped and tripped causing him to fall and fracture his right lower leg.

Employee was assisting in moving the bolter to load supplies, the operator engaged the reel and shut the bolter off but did not disengaged the reel. when they were ready to move the bolter employee picked up the cable, the slack took up and jerked his arm. he completed his shift and did not miss work until 3/18/2014.

Employee states that he and his bolt partner were backing the roof bolter out of the #5 entry. As he pulled up the cable slack back, the bolter turned and began to roll up the cable to the spool. The cable had a loop in it and as it tightened up, his foot was in the loop. This caused a fracture to his (L) ankle.

Employee was helping disassemble a prefab overcast, as they removed the side panels the remaining structure shifted and collapsed. Employee was struck by the structure causing him to be pinned against the scoop bucket and panels of the overcast.

Employee states that while operating a 785b, he was backing under the loader when the 992d bucket struck his cab protector. He states that this occurred several more times during this shift. Employee continued to work until 12/04/2013 when he began to miss. He is complaining of low back pain.

Employee states that while making a belt splice he was using channel locks to pull on the belt, he felt pain in his right shoulder. After a couple of days, the pain and discomfort continued and he went to the Dr. and he was taken off work.

As employee was exiting the bonnie car he was crossing at the front end of the car, he dropped his tools. As he turned to pick them up, he tripped on the bonnie car rope falling, landing on his left elbow.

Employee states that a piece of draw rock broke out between bolts and struck him in the head. He completed his shift and went to the ER afterwards. He was given 1 day off. On 8-22-2013 he went back to the Dr. where he was taken off work until further notice.

Employee was unloading supplies from a flat car with a chain and pallet puller. As the materials were being removed the chain came loose and pinched employee's hand between the chain and scoop bucket.

Employee was walking across the section when he slipped in the #4 entry twisting his (R) knee. The area was damp but no tripping hazards were present. He reported this incident to his supervisor after his knee began to swell. He completed this shift and continued to work until 06/05/2013 when he began to miss work.

Employee states that while adjusting the leveling chain on the pot he had placed his (R) hand on the boom. His (R) middle finger got caught between short arm and pot arm. This caused a laceration to his (R) middle finger. Employee was wearing leather gloves at the time of the incident.

Employee states that he was hanging the miner cable and as he was lifting the cable he felt a pop in his left shoulder and neck. This report was filed by the contractor P177 on 05/21/2013. It was determined thru a Part 50 audit the operator, 4608763 was required to also report this incident. Citation #7166777

EE was unhooking a beam from the crane when he slipped and fell about 8-9 feet hitting his arm on a beam that was on the ground. EE was transported to the hospital for treatment and x-rays were done showing no broken bones and a dislocated elbow was placed on light duty.

An unintentional roof fall occurred on 3/19/2013 on the 1st Southeast Mains at the #5 entry, at or near spad #2425, 720' outby the face on 5A belt. The fall was approximately 36' long x 34' wide x 10' high.
