THE INJURED EE STEPPED ON THE THROW OF 1 SOUTH TRACK SWITCH WHEN HE FELT PAIN IN HIS RT. KNEE. *NOTE: HE RETURNED TO HIS DR. ON 3-18-02 AT WHICH TIME HE WAS KEPT OFF WORK.
THE INJURIED EE WAS MINING COAL REMOTELY ON OUR 4 LEFT 1 NORTH RETREAT SECTION, THE THIRD LIFT OF BLOCK 79, WHEN A PORTION OF THE RIGHT RIB BEHIND HIM ROLLED OFF STRIKING HIM ON THE NECK AND B ACK.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED AT SURVEY STATION 8022, 50 & 38 CROSS OUT OF #2 ENTRY RETURN OF WEST MAINS. THE FALL HAD NO EFFECTS ON VENTILATION THE AREA WAS DANGERED OFF AND SUPPO RTED AT ALL APPROACHES.
THE INJURED EMPLOYEE WAS WALKING ON THE WALKWAY OF THE SECOND DECK OF OUR TOW BOAT, WHEN HE STRUCK HIS HEAD AGAINST THE WALKWAY SUPPORT FOR THE THIRD DECK.
EE WAS PULLING ON WASHDOWN HOSE TO CLEAN OFF CYCLONE SUMPS, WHEN HE FELT PAIN IN HIS RIGHT WRIST. *EE SOUGHT MEDICAL TRMT. ON 3-4-02 WITH LIMITED USE OF RT. HAND.
EE WAS OPERATING #10 CRICKETT, AS HE WAS TRAVEL DOWN WEST MAINS TRACK HAULAGE HE SEEN A PARKED JEEP JUST INBY 1 SOUTH SWITCH, HE DID NOT THINK HE COULD STOP HE JUMP OUT STRIKING THE LEFT COAL RIB, INJURYING HIS LEFT KNEE.
EE WAS RIDING THE #10 CRICKET (W/ COWORKER AS THE OPERATOR). AS IT APPEARED THAT HE WASN'T GOING TO STOP , PRIOR TO HITTING THE PARKED JEEP (#5) THE OPERATOR JUMPED, AND THEN CRICKET STRUCK TH E JEEP. EE'S LEFT SHOULDER, RIBS AND BACK STRUCK THE CONTROL PANEL AND CONTROLLER.
AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #45 BLOCK OF 4 LEFT 1 NORTH RETREAT SECTION. THE FALL EXTENDED FROM THE GOB INTO #11 ENTRY OF OLD 3 LEFT 1 NORTH PARTIALLY COVERING TWO MOBILE ROOF SUPPORTS. SIZE OF FALL COULD NOT BE DETERMINED. BUT IT WAS HIGHER THAN THE EXISTING ROOF BOLTS PRODUCTION WAS INTERUPTED FOR APPROX 5 HOURS.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THEBARRIER CUT-THRU FROM 4 LEFT 1 NORTH SECTION INTO THE OLD 3 LEFT 1 NORTH BLEEDER ENTRY (SURVEY STATION 7842- 7140.) SIZE OF FALL COULD NOT BE DETERMINED, BUT THE HEIGHT WAS ABOVE THE ROOF BOLT ANCHORAGE. THE APPROACH HAD BEED SUPPORTED AND DANGERED OFF. AGE OF FALL COULD NOT BE DETERMINED BECAUSE FALL WAS NOT REPORTED TO MINE.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN #9 ENTRY, SURVEY STATION 7764, 4 LEFT 1 NORTH SECTION. THE FALL WAS APPROX. 18'WIDE X 25'LONG X 8' HIGH. AREA WAS SUPPORTED WITH 6' CONVENTIONAL RO OF BOLTS ON A 4' CENTER PATTERN. ALL APPROACHES SUPPORTED AND DANGERED OFF. IT COULD NOT BE DETERMINED HOW OLD THE FALL WAS AS IT WAS NOT REPORTED TO MINE FOREMAN OR SAFETY DEPARTMENT.
AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #27 BLOCK OF 4 LEFT 1 NORTH RETREAT SECTION. THE FALL EXTENDED FROM THE GOB INTO THE #3 ENTRY PARTIALLY COVERING TWO MOBILE ROOF SUPPORTS. SIZE OF FALL COULD NOT BE DETERMINED, BUT APPEARED TO BE25'-35'HIGH. AREA WAS SUPPORTED WITH 6' CONVENTIONAL ROOF BOLTS ON A 4' CENTER PATTERN. PRODUCTION WAS INTERRUPTED FOR APPROX. 14 HOURS.
THE EE WAS CUTTING OFF THE BELT WINDER CAR FROM THE RAIL CAR, AS THE MOTORMAN MOVED THE RAIL CAR OUTBY, THE BELT WINDE CAR DRIFTED OUTBY, STRIKING HIS LEFT FOOT CATCHING IT BETWEEN THE CAR AND THE BOTTOM.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED AT SURVEY STATION 8021 OF WEST MAINS. THE FALL APPROACHES WERE SUPPORTED AND DANGERED OFF. AREA OF FALL APPROX. 16' X 16' & 7' HIGH.
THE EE WAS HOISTING A VESSEL FLIGHT, AS HE GRABBED THE FLIGHT, THE NYLON CHOKER SLIPPED PINCHING THE INDEX, MIDDLE, AND RING FINGERS OF HIS LT. HAND, CAUSING AN AVULSION TYPE INJURY.
THE EE WAS TRAMMING HIS SHUTTLE CAR BACK TO THE FACE WHEN HE HIT A POT HOLE, CAUSING HIM TO JAM HIS HEAD AGAINST THE CANOPY. *EE SOUGHT CHIROPRACTIC TREATMENT ON 1-28-02.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED AT SURVEY STATION 352 OF NORTH MAINS INTAKE ESCAPEWAY THE FALL WAS DANGERED OFF AND ADDITIONAL SUPPORTS INSTALLED. 4 LEFT 1 NORTH PRIMARY ESCAPEWAY WA S EFFECTED UNTIL THE FALL WAS TIMBERED. AREA OF FALL APPROX. 20' WIDE, 35' LONG, 10' HIGH.
EMPLOYEE WAS ROOF BOLTING WHEN A PIECE OF ROOF SLATE FELL STRIKING HIM ON THE RIGHT SHOULDER AND ARM. EMPLOYEE HAS RETURNED TO MODIFIED DUTY ON 1-21-02.
THE EE WAS USING A SLABE BAR TO PRY DOWN LOOSE MATERIAL FROM THE ROOF, THE BAR SLIPPED AND THE EE FELT PAIN IN HIS LOWER BACK. THE EE SOUGHT MEDICAL TREATMENT ON 1-30-02. HIS LAST DAY AT WORK.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED AT SURVEY STATION 7887, 46 X 68 X CUT OF 2 SOUTH BELTLINE, HAD NO EFFECTS ON VENTILATION BUT STOP PRODUCTION. THE AREA WAS DANGERED OFF AND SUPPORTED AT ALL APPRACHES.
THE EMPLOYEE INDICATED THAT WHEN HE WAS HANDLING VENTILATION TUBING (16" X 10') HE FELT A PAIN IN HIS RIGHT HIP AND BACK.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED AT SURVEY STATION 7890, #4 ENTRY OF 4 LEFT 1 NORTH SECTION, THERE WERE NOT WITNESSES TO THE ACCIDENT. NO EFFECT ON VENTILATION OR TRAVEL. THE AREA WAS DANGERED OFF WITH ALL APPROACHES SUPPORTED. NO DANGER OR HAZARD EXIST AT THIS TIME.
THE INJURED EE WAS SWEEPING THE BELT LINE, WHEN SHE STEPPED ON A PIECE OF COAL TWISTING HER LEFT ANKLE.
EE WAS STEPPING OFF TRUCK SLIPPED & FELL DISLOCATING SHOULDER.
THE EMPLOYEE HAD JUST MOVED THE MINER CABLE WHENA PIECE OF ROOF SLATE FELL STRIKING HIM ON THE LEFT LOWER BACK. THE PIECE WAS APPROX. 18" X 18" X 1".
THE EMPLOYEE WAS TAKING THE SIDE PANEL OF CABLE REEL COMPARTMENT OFF THE SHUTTLE CAR WHEN THE PANEL DROPPED AND STRUCK THE TOP OF HIS LEFT FOOT.
EMPLOYEE WAS BUILDING A CRIB AND CRIB FELL STRIKING HIM ON THE RIGHT THUMB. HE DID NOT SEEK MEDICAL TREATMENT UNTIL THE BEGINNING OF HIS NEXT SHIFT.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED BY THE SHIFT FOREMAN ON WEST MAINS, #3 ENTRY STATION 7941, 47+47. IT FELL APPX 12" ABOVE THE ANCHORAGE OF THE 6'BOLT USED TO BOLT THE PLACE. HAD NO EF FECTS ON VENTILATION OR HAULAGE. IT WAS DANGERED OFF AND SUPPORTED AT ALL APPROACHES.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED BY SHIFT FOREMAN ON THE WEST MAINS SECTION. THE FALL WAS JUST OUT BY STATION 8044, 51 AND 43 OF #4 ENTRY THE FALL COVERED THE AUX FAN CABLE, HAD NO EF FECTS ON VENTILATION. THE AREA WAS DANGERED OFF AND SUPPORTED ON BOTH APPROACHES. SEE ATTACAHED MAP AND SKETCH.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED BY THE WEEKLY EXAMINER APPROX 6:30 PM RIGHT RETURN OF WEST MAINS #10 ENTRY JUST OUT BY STATION 7912 46 AND 39 IT HAD NO EFFECTS ON VENTILATION, THE AR EA WAS DANGERED OFF AND SUPPORT AT BOTH APPROACHES.
EE TRIPPED ON WATERLINE, AND FELL,M INJURING HIS LEFT KNEE. NOTE: EE SOUGHT MEDICAL TREATMENT PRIOR TO THE BEGINNING OF HIS WORK SHIFT ON 11-20-01.
AN EE WAS DRILLING A HOLE FOR A ROOF BOLT, WHEN A PIECE OF ROOF SLATE FELL STRIKING HIM ON THE TOP OF HIS RIGHT THUMB. NOTE: THE EE WAITED UNTIL HIS NEXT SHIFT TO SEEK MEDICAL TREATMENT.
THE INJURED EMPLOYEE WAS HANDLING SUPPLIES FROM THE SCOOP BUCKET, WHEN PIECE OF ROOF COAL 2' X 2" X 3" FELL STRIKING HIM ON THE RIGHT LOWER BACK.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED BY PRE-SHIFT EXAMINER ON THE IDLE WEST MAINS SECTION MMU 074, 46 ROOM #1 ENTRY, STATION 8023 WHICH WAS THE LAST OPEN XCUT. THE ENTIRE INTERSECTION FEL L AND INTO THE XCUT TOWARD #2 ENTRY APPX 35'. THE FALL WAS ABOUT 8'HIGH. ABOVE THE 6'TENSION AND 8' TO 10 SUPPLEMENTAL ROOF BOLTS BEING USED. THE FALL DANGER OFF, NO ONE INJURED, NO EQUIP DAM
THE EMPLOYEE WAS LIFTING MINER CABLE OUT OF THE ROADWAY, WHEN HE FELT PAIN IN HIS BACK.
EE WAS TAKING A CHAIN OFF THE TOP OF THE MANTRIPWHEN THE HOOKED END FELL OFF, STRIKING HER RIGHTHAND.
THE EE WAS TIGHTENING 1" BOLTS ON THE CUTTER W 3/4 DRIVE RATCHET AND 6' CHEATER PIPE. AS THE BOLTS WERE GETTING TIGHT THE RATCHET AND PIPE WOULD SNAP, JARRING HIS BACK AND LEFT SHOULDER. SOUGH T CHIROPRACTIC TREATMENT ON 10-18-01.
THE INJURED EE WAS OPERATING THE LEAD SUPPLY MOTOR, TAKING SUPPLIES TO WEST MAINS SECTION. AS THEY STARTED DOWN THE STEEP GRADE, HE JUMPED FROM THE MOTOR AND STRUCK THE COAL RIB. THE MOTOR STR UCK THE #5 JEEP AT END TRACK.
AS THE INJURED EMPLOYEE WAS TAKING DOWN VENT TUBE, WHEN A WIRE BROKE ON A HANGING TUBE, THAT FELL STRIKING HIM ON THE RIGHT INSTEP. * THE EMPLOYEE HAD A MRI ON 11/21/01 THAT REVEALED A FRACTUR E.
THE EE WAS CARRYING A BELT SPLICING MACHINE, WHEN HE SLIPPED AND FELL, CAUSING PAIN TO HIS LEFT HIP AND LOWER BACK. HE SOUGHT CHIROPRACTIC TREATMENT ON 10-01-01.
THE EMPLOYEE WAS HOSING UNDER #1 NORTH DRIVE WHEN HE STOOD UP HE FELT PAIN IN HIS LOWER BACK.
THE EMPLOYEE WAS WALKING DOWN THE STEPS INTO THE MINE (AT STARTING TIME) WHEN HE TWISTED HIS RIGHT ANKLE AT THE LAST STEP.
AS THE EMPLOYEE WAS WALKING ON THE FIRST FLOOR OF PREP PLANT HE SLIPPED ON PLASTIC PIPE, TWISTING HIS RIGHT ANKLE.
THE EE WAS REBOLTING AN AREA WITH THE TRACK MOUNTED BOLTER, WHEN PIECE OF ROOF SLATE (12" X 12" X 12") FELL STRIKING HIM ON THE FOREARM.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN #7 ENTRY OF 4LT 1NORTH SECTION (MMV 072) SURVEY 11+92. THE AREA IS A SUBNORMAL ROOF AREA WITH CLAY VEIN. FALL WAS ON SECTION TRACK AND HIGH VOLTAGE CABLE CAUSING SEVERAL HOURS OF PRODUCTION LOSS. APPROACHES ARE SUPPORTED AND DANGERED OFF. FALL CAVITY 8'HIGH, 17'LONG AND 17'WIDE.
THE EE WAS STANDING ON A TIMBER (8' X 4' X 4") AND UNLOADING OTHERS, HE SLIPPED AND FELL STRIKING HIS RIGHT WRIST FOREARM AND ELBOW AGAINST THE SIDE OF THE SCOOP BUCKET.
THE EE HAD JUST COMPLETED EXAMING THE FACE, AS HE WALK OUTBY DOWNHILL HE SLIPPED AND FELL TWISTING HIS RIGHT ANKLE.
THE EMPLOYEE WAS CLEANING THE SUCTION LINE ON THE RIGHT DRILL HEAD OF THE ROOF BOLTER, WHEN A PIECE OF ROOF SLATE (1' BY 2" BY 1') FELL STRIKING HIM ON THE BACK, HAS HE MOVED HE TWISTED HIS RI GHT KNEE AND ANKLE.
THE EMPLOYEE WAS PUTTING A BOARD ON A SCAFFOLD, WHEN HE FELT PAIN IN HIS LOWER BACK. RECEIVED CHIROPRACTIC TREATMENT ON 9-14-01.
