INJURED HAD LOADED A COAL TRUCK AND THEN STOPPED THE LOADER AND WAS STEPPING OFF THE LOADER STEPS AND STEPPED ON A LUMP OF COAL ABOUT 10" DIAMETER AND TWISTED HIS RIGHT ANKLE.

A ROOF FALL OCCURRED ON THE 001 SECTION AND SPAD 101 INTERSECTION ONE CROSS CUT INBY THE FEEDER THE INTERSECTION FELL APPROX. 5 FT TO 61/2 HIGH 18 FT WIDE AND 18 FT LONG THE AREA WAS SUPPORDED WITH 5 FT AND 6 FT 3/4 INCH FULL GROUTED ROOF BOLTS RESIN A CLAY VEIN MAY HAVE WEAKED THE AREA.
