EE was at section feeder running rock through feeder when he attempted to hit steel bar to activate e-stop. The steel bar slid out by and did not hit e-stop. EE hit e-stop cable which caused a rock to flip over and pinned his hand and cut it.

Hanging face curtain in #2 entry on 4ft ladder. Ladder leg buckled and employee fell from the 3rd rung on the ladder landing on elbow forcing shoulder upward.

Bolt man went to walk past the bolter after hanging his curtain and tripped and fell into bolt machine.

Welding on a Joy 14-9CM when a piece of hot material went into boot causing a burn on the inside of the left calf area causing a burn. Was not reported until 8/18/13.
EE was running #3 shuttle car ran over ledge in bottom turning right handed toward feeder. His head jerked back and rammed head into canopy. Has pain in neck and down right side of body.
Employee & co-worker were pulling the cable of a s-car when employee felt a tightening in his back. He finished the shift and didn't stop work until 3-18-13.

Around 10:20 AM EE came outside - had crawled nearly 4000 ft - could not tell management why or how he had gotten outside. In the absence of any apparent injury, he was sent home for the remainder of the day. Later that evening we learned he was kept overnight at hospital for observation. He made the statement he had bumped his head earlier.
Bolting top caught tip of finger on roof plate.

While cutting miner cable to make a splice, blade slipped, cutting the inside of his left middle and ring finger.
While pulling a cable, strained left lower back - did not seek medical attention until 10/24/11.

Employee stated that he was helping to make a belt splice when he was struck by the hammer used to drive the belt nails. Co-worker was wielding the hammer and accidentally hit EE's right middle finger.
Laying track tightening up bolts in fish plates pulled muscle in lower part of stomach.
Was loading oxygen tank - oxygen slipped and smashed little finger on left hand between tand and frame of mantrip.
During a weekly examination by ee of the right return it was discovered that a roof fall had occurred on Panel 6 at spad #1525. Stopping were installed and removed to allow the return to be safely traveled in its entirety. All accessible entries leading into the fall area have been supported and dangered off.
The victim received fatal injuries and was found in the #4 belt head chute. There were no eyewitnesses. The circumstances surrounding and the cause of the accident are under investigation.
EE was running s/c in #1 entry, rib rolled onto his hip area.
Employee had been shoveling belt, hit his head on uneven mine roof, hurting the back of his neck. Stated his neck was already hurting due to a previous non-work related injury.
