Injured employee was hit by a hammer from a distance of 4 feet. The hammer came out of the rubber protective sleeve on the handle and the metal hammer flew through the air and hit the employee on the left hand. Resulted in a contusion and a broken bone.

Showing 50 of 111EE was running the number 4 shuttle car and while turning the corner at the number 1 entry he impacted the coal rib into the canopy. He had his hand on the canopy at the time of the impact. Received cuts, contusions, and crushing of the right hand and pointer finger. Approximately 30 stitches were placed in the hand.
Employee raised up and impacted his left ear into a roof bolt and plate. Employee was walking with head bent over and turned head and ran it into the roof bolt and plate. Laceration to the left ear. Received approximately 10 Stitches in his ear.
The Injured employee said he slipped in a mud hole on the section and strained his knee.
Using Scoop and Chain and Pulling TailPiece to pull the belt back. The chain broke and the tailpiece lunged and hit EE on the lower portion of his right leg. Employee was removed from the mine and taken to the Hospital. Swelling, Bruising of lower portion of leg. Not yet known if any bones were fractured/broken.
Electrician was putting a Cat Head on a Cable. While using a Knife to cut the Shielding on the cable he cut his left hand. The cut was in the webbing between his thumb and forefinger. He had 5 stitches placed into his hand, some were below the surface of the skin and some are on the surface of the skin.
Employee was walking on Section and his upper back and lower neck impacted a roof bolt plate, causing small contusion to skin. Employee went to Hospital after work and the Laceration was "Glued" -- no stitches were made by the treating physician. The employee missed one shift of work and returned to his regular job duties.
Injured employee was operating the Scoop Machine. When Exiting, he placed his left hand on top of the canopy. A Rock fell from the Mine Roof and Crushed his hand between the Rock and Steel Canopy.
Injured employee was building a intake stopping and a cinder block fell and struck him on the right hand.
Employee was picking up a piece of waterline and slipped and turned his left knee inward.
Working on scoop - putting in carrier assembly. EE had a cheater pipe on a ratchet trying to break bolt and nut loose. Over exerted pressure on left arm and pulled muscle on inside of arm between forearm and bicep.
Rock broke out of Rt rib, sticking EE in back showing him against side of miner. Rock measured 7' long 18"x10".
Slipped on lose material and fell and twisted left knee servicing miner.
Slipped in the mud loading the belt tools
EE was getting off mantrip on section, EE tripped on something and pulled something in his back. Tried to work and pain became more severe. EE worked until 1:30a.m.
Walking belts, stepped in crack in bottom.
Trying to lift tram motor in buggy
Attempted to break loose a rusted pipe fitting on a pump using a pipe wrench. Strained lower back.
Setting timbers on 001 section. When employee was lifting timbers, he twisted his right knee.
Hit right shoulder in the roof of the mines.
Was setting timbers in #5 Entry and hurt right shoulder when lifting. NOTE: Employee began missing work on 10/31/08.
Cleaning out rock box on #5 head and smashed fingr in between 2 piece of rock.
Scoop feeder hit coal rib; rib rolled in deck and struck EE in the mid back. His right arm went numb.
In #5 entry pulling cable off shuttle car. Stepped on a lump of coal on the muddy bottome, twisted left knee.
Employee & co-worker were using a bar to pry a roller out of stand to replace roller. The bar slipped and employee fell injuring his left knee.
Slipped into hole and twisted right knee.
Walking back from lunch in #4 entry. Slipped in mud, fell and injured right hand.
Pulled muscle in rack moving miner cable.
Men were throwing block across scoop and hit EE on left thumb, cutting the thumb.
Employee was cleaning gob on off side of #3 belt and the rib rolled and covered him up. He was covered up for about 1 hour; it took 7 men to get him out.
Corner broke off striking right foot.
Buggy breaker knocked and employee went to put breaker back in when a flash occurred at receptacle. Employee jumped back and hit hand on power box and maybe rib.
Employee was pulling miner cable out of shuttle car roadway and strained his right shoulder.
Bent down to mix a batch of fast set cement and some spilled and got in eyes.
Running 488 scoop to clean #7 entry and coal from rib rolled and hit left knee and right shoulder.
Hoisting belt drive boom in the air when the chain of hoist slipped. The boom fell from about 3 feet and struck victim's left leg. Accident happened at #3 belt drive between #3 and #4 entries.
Stepped off forklift and stepped on uneven ground and right ankle twisted.
Injured was plastering stopping and plaster got in eye. He was wearing safety glasses and plaster splashed in eye from side.
During the process of installing roof bolts, operator swung drill head out to install outside bolt & pot struck neck & jammed rock into knee.
Electrician was cutting a 5/8" steel rope cable by hitting it with a hammer. Injured EE was walking by the scoop and a metal fragment struck him in the left eye. The fragment pierced the upper eye lid and required removel and a liquid stitch to close the wound.
Putting up belt hangers & ATRS hit my lower back.
Bolt crew had set ATRS preparing to bolt the face of #4 entry. Operator was starting to swing drill boom out to install 1st Roof bolt. A piece of slate 0-8" thick x 30" x 60" fell between T-bar and last permanent support. Rock hit operator on head & shoulders & hit bolter canopy. Canopy had not yet been raised. Neck was jammed up & shoulders bruised.
Employee had assisted in setting a bent for a conveyor support. He was standing on a concrete pier & stepped from the pier to a guard rail. From the guard rail he jumped to the ground landing on a piece of loose guard rail & he twisted his left ankle.
EE began to feel pain in back after entering and exiting Joy 10SC32 Shuttle Car.
Walking down hill on No. 1 belt. Fell on slick, loose mud and twisted and landed on his left knee.
EE INJURED LEFT SHOULDER WHILE REMOVING ROCK FROM BELT TRANSFER POINT.
INJURED EMPLOYEE WAS HELPING MECHANIC/ELECTRICIAN REMOVE SHIELD FROM CONT. MINER. HIS HAND SLIPPED OFF WRENCH AND HAND WAS CUT ON PIECE OF METAL.
A pall was discovered in intake airway 30'l, 20' w, 5-6' thick. Laminated shale with slight water present. Good air flow across fall and a 6' walkway was timbered across fall edges of fall were sloped with scoop to make travelable. Additional supports were installed in roadway and crosscuts between intake & roadway and between roadway and belt.
EMPLOYEE WAS SHOVELING ON BELTLINE AND FELT PAIN IN LOWER BACK.
Unloading resin from scoop bucket and putting the glue on top of pinner, he felt pain in lower back, hip and left leg.
