Rib rolled off corner of pillar striking car driver in lower back.

A non-injury roof fall occurred at approx 8:50 AM. 2nd cross cut on Ginsong Mains. 18' wide x 5' thick x 38' long.

Employee was using cutting torch to cut metal jacks. He laid torches down to turn pipe and torch rolled over and flame struck left hand.

Roof Fall occurred that pulled roof support above bolt anchorage.

Drill steel fell out of hole and bounced off drill pot and struck operator in forehead.

Cut right index finger while installing wet down hose on head drive.

6 volt battery on personnel carrier arced when being worked on resulting in a flash to the eyes and maybe some acid to the face.

There was an altercation between face foreman and miner operator. EE received two punches to the head. Note: No one received medical treatment.

Operator was holding drill steel while wearing a rubber glove. Glove stuck to steel and twisted his left hand resulting in breaking his left pinky finger.

Employee had loaded concrete block in scoop bucket during the trip to face a block fell out of bucket and scoop ran over it, causing it to drop down suddenly.

Hand slipped off handle and struck miner in chin.

Raised up in railrunner in low top and was pinned between railrunner and mine roof.

Strap was broke loose from bolt, hanging down and caught miner behind right ear, resulting in a 4" laceration.

Tire blew out on shuttle car he was operating.

Employee struck top with hard hat; dirt and rock fell into employee's left eye.

Employee was riding mantrip and got into top.

Employee was changing bottom stands on belt line. #2 was pulling stand out from under belt. Stand got caught and then as pulling hand it came loose and the frame on stand against hand hit 4" water line, resulting in hand being mashed between them.

N6W face up/slip of rock. Broke from high wall. Fall was 20' in Width, 15' in height & 8' in depth.

EMPLOYEE WAS SITTING A TIMBER, PIECE OF DRAW ROCK FELL OUT OF TOP STRIKING ANKLE.

WAS MAKING A BELT SPLICE AT #3 HEAD DRIVE, EE PICKED UP A BELT SPLICE BOARD AND GOT A SEVERE PAIN IN LOWER BACK.

WAS LAYING ON MINER, DIGGING A ROCK FROM CONV. CHAIN. LAID ON A ROCK THAT WAS PROTRUDING HIGHER THAN REST OF ROCK, AND BRUISED RIGHT RIB.

HE WAS INSTALLING ROOF BOLTS & A SHARP PIECE OF DRAW ROCK FELL FROM ROOF STRIKING LEFT FOREARM.

WAS SLIDING MINER CABLE OVER WITH RIGHT LEG, WHILE BENT OVER HOLDING BOX AND HURT BACK.

WHILE INSTALLING A ROOF BOLT THE BOLT BENT STRIKING HIS LEFT HAND.

WAS OPERATING A SCOOP WAS TRAMMING AROUND A PILLAR 4 BREAKS FROM OUTSIDE SOMEHOW HAD LEFT R AND ON PINCH POINT BLACK, SWIVELD SCOOP AND MASHED LEFT THUMB AND 3-FINGERS.

WAS RUNNING MINER TURNING A RIGHT BREAK OUT OF #3 ENTRY. PIECE OF DRAW ROCK FELL FROM RIB TO FIRST BOLT APP. 2-3' WIDE, 1-1 1/2" THICK, STRUCK VICTIM IN UPPER PART OF BACK AND RODE HIM TO MINE FLOOR.

WAS INSTALLING ROOF BOLT & BOLT HIT RIGHT WRIST

A PIECE OF DRAW ROCK FELL FROM ROOF WHILE OPERATOR WAS BOLTING, PUSHING HIM INTO THE BOLTER & BRUISING HIS RIBS.

EE WAS SITTING NEXT TO A CORNER IN A INTERSECTION PILLAR LINE PART OF A RIB ROLLED OFF, ROAD HIMTO THE GROUND UPPER PART OF NOSE HIT A PIECE OF COAL OR ROCK RESULTING IN A BROKE NOSE

ROCK FALL WAS IN RETURN ONE BREAK INBY SPAD # 176 AND DID NOT INTERFEAR WITH VENTILATION AND ACTIVE SECTION. FALL WAS 42" THICK AND 20 FT LONG, 20 FT WIDE. NO PEOPLE OR EQUIPMENT OR EQUIPMENT INVOLVED.

TWISTED KNEE WHILE BOLTING, AND TORE LIGAMENT OR CARTILAGE.

WAS SHOVELING UNDER STACKER BELT AND OUTSIDE HEAD DRIVE AND HURT BACK.

INSTALLING ROOF BOLT, SMALL PIECE OF ROCK FELL FROM ROOF STRUCK EMPLOYEE ON RIGHT SIDE OF EYE, CUT UPPER EYE LID.
