Showing all 39A non-injury roof fall occurred at approx 8:50 AM. 2nd cross cut on Ginsong Mains. 18' wide x 5' thick x 38' long.
Employee was using cutting torch to cut metal jacks. He laid torches down to turn pipe and torch rolled over and flame struck left hand.
Roof Fall occurred that pulled roof support above bolt anchorage.
Drill steel fell out of hole and bounced off drill pot and struck operator in forehead.

Operator was holding drill steel while wearing a rubber glove. Glove stuck to steel and twisted his left hand resulting in breaking his left pinky finger.
Employee had loaded concrete block in scoop bucket during the trip to face a block fell out of bucket and scoop ran over it, causing it to drop down suddenly.

N6W face up/slip of rock. Broke from high wall. Fall was 20' in Width, 15' in height & 8' in depth.

WAS OPERATING A SCOOP WAS TRAMMING AROUND A PILLAR 4 BREAKS FROM OUTSIDE SOMEHOW HAD LEFT R AND ON PINCH POINT BLACK, SWIVELD SCOOP AND MASHED LEFT THUMB AND 3-FINGERS.
WAS RUNNING MINER TURNING A RIGHT BREAK OUT OF #3 ENTRY. PIECE OF DRAW ROCK FELL FROM RIB TO FIRST BOLT APP. 2-3' WIDE, 1-1 1/2" THICK, STRUCK VICTIM IN UPPER PART OF BACK AND RODE HIM TO MINE FLOOR.
WAS INSTALLING ROOF BOLT & BOLT HIT RIGHT WRIST
A PIECE OF DRAW ROCK FELL FROM ROOF WHILE OPERATOR WAS BOLTING, PUSHING HIM INTO THE BOLTER & BRUISING HIS RIBS.
EE WAS SITTING NEXT TO A CORNER IN A INTERSECTION PILLAR LINE PART OF A RIB ROLLED OFF, ROAD HIMTO THE GROUND UPPER PART OF NOSE HIT A PIECE OF COAL OR ROCK RESULTING IN A BROKE NOSE
ROCK FALL WAS IN RETURN ONE BREAK INBY SPAD # 176 AND DID NOT INTERFEAR WITH VENTILATION AND ACTIVE SECTION. FALL WAS 42" THICK AND 20 FT LONG, 20 FT WIDE. NO PEOPLE OR EQUIPMENT OR EQUIPMENT INVOLVED.
TWISTED KNEE WHILE BOLTING, AND TORE LIGAMENT OR CARTILAGE.
WAS SHOVELING UNDER STACKER BELT AND OUTSIDE HEAD DRIVE AND HURT BACK.
