He continued working without being off or seeing a doctor until he was laid off on December 1, 2015. EE had to seek medical attention for his neck February 11, 2016. He reported the injury to mine management again on February 17, 2016
The employee was checking a shuttle car tire in the #3 entry 2 crosscuts outby the face when a piece of rib coal rolled off knocking him off balance causing pushing him into the shuttle car and catching him between the car and the coal rib.
Roof Bolting Machine operator was installing 8 foot rope bolts. As he was placing the bolt in the hole and pushing it into place he accidently hit the rotation lever on the drill. As the bolt begin to spin a strand of wire or metal broke away from the bolt and entered the right hand of the machine operator.
The employee is a roof bolt machine operator and was working in the number 4 entry of the 001 section. EE was installing the inside roof bolt when a piece of roof rock broke loose and struck EE's left foot. The rock was approximately 2'X 1'and from 0 to 5 inches thick.
A roof fall occurred in the number 4 portal (fan) intake air course entry approximately 18 feet wide, 10 feet high and 60 to 70 feet in length. The cause of the fall was due to the extremely harsh winter weather of late where the roof froze and thawed several. This fall was immediately report to MSHA via the 800 number.
Found rock fall 13 breaks outby section in return between spad # 1336 & 1327. Had cribs set & 12' rope bolts. Fall was approximately 60' long, 18' wide, 12' 13' thick. No major ventilation change. No escapeways blocked. Reported to MSHA by phone on 7/9/07 after getting to surface.
Employee was bolting top and a small piece of draw rock fell & struck him on left arm.
Had rock fall one break inby spad #1334 in #2 entry on 004 section. Fall approx 6-10' thick X 40' long X 18' wide. Area was bolted with 6' resin bolts & 12' rope bolt over 14' - 16'. Area cribbed off on all sides with crib block. Fall was inspected by MSHA Inspectors on 6/13/07.
EE twisted his knee in April. Continued to work - didn't miss any work. Went to doctor in Knoxville & had it checked out. Went on vacation week of the 4th. We had lay off on the 9th. He informed us then that he was going to have surgery on his knee that day. (He went on workers comp starting the 9th of July 2007).
Employee was bringing scoop outside,went down a hill & hit pole in bottom & it bounced him up in deck of scoop & he hit his head on canopy. Went to hospital next day at 12:30pm on 8/9/06 complaining of neck pain.
Loose rock fell from roof between bolt and rib. He was taken to hospital by amulance and then released to go home after being checked out.
Employee was using a hydraulic spreader with a porta power jack. The spreader slipped out & struck him in the nose. Went to emergency room, did x-ray, said he had a hairline fracture on bridge of nose.
Employee was bolting top in #6 entry. He had put up bolt and then walked back to get a box of glue from back of pinner. When he started back to front of roof bolter, a piece of draw rock fell from between rib and bolt and struck him in the head and back and knocked him to the ground.
Employee was bending 6'roof bolt to install into hole & he pulled something in his left shoulder.
ROOF FALL AT INTERSECTION OF #2 ENTRY, ON 003 SECTION 6' RESIN BOLTS WERE BEING USED ALSO 10' ROPE BOLTS. WATER COMING THROUGH TOP. FALL WAS APPROXIMATELY 20 X 20 X 10' THICK. AREA WAS DANGERED OFF & CRIBS SET.
EE was making up roof bolts & helping on roof bolter and a piece of draw rock fell from bolt over to rib striking him in his back.
Employee was cutting coal with Joy miner and a piece of metal broke off of bit when it hit sandstone in the coal, metal flew back about 50' and struck him in the right eye (safety glasses were provided but he was not wearing them).
Employee was removing crib block and got hit in the right pointer finger and it broke finger between knuckle & 1st joint.
Employee fell and struck left elbow on bottom. He didn't go to doctor until 8-12-05 and had x-ray done, nothing broken just bruised. Went to doctor because elbow was still a little sore. Didn't miss any work.
Working on loader bucket, he mashed left thumb inbetween bucket and bar.
PIN HAD COME OUT OF PULL CAR BEHIND SCOOP. EE WAS IN THE PROCESS OF PUTTING PIN BACK IN THE HOLE. HE HIT PIN WITH HAMMER AND SOMETHING FLEW OFF PIN AND HIT EE IN RIGHT EYE.

WAS WORKING ON A PUMP, WENT TO MOVE IT & WAS PULLING ON IT & TURNED TO THE LEFT. HIS LEFT LEG GAVE WAY & HE HURT SOMETHING IN HIS LEFT HIP.
Employee was walking to truck, fell on the way and cut his left hand when he fell on a piece of metal. It was dark; he said he could not see.
EMPLOYEE WAS CHANGING BIT IN PIECE OF DRILL STEEL WHEN A SMALL PIECE OF SHARP ROCK FELL & STRUCK HIM IN THE POINTER FINGER ON HIS RT. HAND, CUTTING THE TIP OF THAT FINGER UP AROUND THE FINGERNAIL. IT TOOK 6 STITCHES & THEY WANT HIM TO SEE ANOTHER DR. FOR ANY OTHER DAMAGE.
EMPLOYEE WAS BOLTING TOP IN #1 ENTRY WHEN A PIECE OF DRAW ROCK FELL & HIT TOP OF CANOPY & HE RAN OUT FROM UNDERNEATH THE CANOPY & A PIECE OF THE ROCK SLID OFF & HIT HIM IN THE HEAD. HAD TO HAVE 10 STITCHES TO CLOSE THE WOUND.
EE was tightening fitting on water line with a pipe wrench, the pipe wrench slipped and struck him in the mouth and broke his front tooth. Was making a belt move at time.
ROCK FALL WAS 4 1/2' THICK 18 X 18 FEET WIDE IN INTERSECTION OF #5 ENTRY 5 BREAKS UP ON #2 BELT INTAKE. SHOWED NO SIGNS OF ACTIVITY BEFOR IT FELL DANGERED OFF AND BUILT CRIBS AROUND IT DOES NOT AFFECT VENT OR TRAVEL WAY
A rock pinned his hand against canopy.
EE was changing tire on 10SC shuttle car, had new tire on and was finishing tightening bolts, when the tire exploded and the 2 safety rings struck the victim in the face, left hand and chest area.

EE WAS WALKING AROUND ROOF BOLTER & STRUCK HIS RIGHT LEG AGAINST SIDE OF ROOF BOLTER THAT HAD A STRAP STICKING OUT & IT CUT HIS LEG JUST ABOVE HIS KNEE. HE WORKED THE SHIFT & WENT TO THE E.R. AT MIDDLESBORO AFTER WORK. THE PUT 4 STAPLES & BANDAGE & T-SHOT
EE WAS BOLTING TOP IN #4 HEADING AND A PIECE OF DRAW ROCK FELL AND STRUCK HIM IN THE RIGHT FOOT. ROCK SLID OFF TOP OF CANOPY AND STRUCK HIS FOOT. WENT TO THE DOCTOR THE NEXT DAY - NO BONES BROKEN, JUST BADLY BRUISED.

EMPLOYEE WAS TAKING SCOOP OFF CHARGE AND WHEN HE PULLED ON BATTERY BELL HE PULLED SOMETHING IN HIS BACK.
EE WAS BOLTING TOP IN #1 ENTRY AND PIECE OF ROCK SLID OFF OF CANOPY AND STRUCK HIM IN HIS RIGHT FOOT. TOOK HIM TO PINEVILL COMMUNITY HOSPITAL EMERGENCY ROOM. THEY ADMITTED HIM FOR OBSERVATION AND TO CHECK FOOT OUT. ROCK CUT A PLACE ON TOP OF HIS FOOT & POSSIBLY BROKE BONE IN TOP OF FOOT.
ROOF STARTED WORKING 6 BREAKS IN BY SECTION HEAD DRIVE (001 SECTION) ONE BREAK IN BY SPAD 536. CRIBS WERE SET BEFORE THE FALL. BELT AND STRUCTURE WAS REMOVED AND BELT LINE WAS MOVED OVER ONE ENTRY AND RESET. FALL WAS 40' LONG 19' WIDE AND 10' THICK.

UNTI L THE 19TH AND BACK TO DR.ON 9-22-03 RETURNED TOWORK ON 9-23-03 SAID HE WOULD HAVE TO GO TO DR. AGAIN LATER.

EE WAS BOLTING TOP IN #5 HEADING & SMALL PIECE OF ROCK FELL BETWEEN CANOPY & ATRS AND STRUCK HIM IN THE LEFT HAND. CUT HIS MIDDLE FINGER & HIS RING FINGER.
STARTED A #2 LEFT BREAK CUT ABOUT 15' DEEP. TOP STARTED TO WORK BEFORE ROOF BOLTER GOT TO IT. WE LET IT WORK AWHILE. IT FELL ABOUT 4' THICK AND PULLED ROCK FROM AROUND A COUPLE ROLLS OF BOLTS IN THE INTERSECTION. AREA HAS BIG SLIPS SO WE TIMBERED AND DANGERED IT OFF AND CUT AROUND THE AREA.
EE WAS CHANGING WHEEL UNIT ON SHUTTLE CAR AND INJURED HIS BACK. WAS OFF FOR THANKSGIVING DIDNOT COME BACK TO WORK ON THE 1ST POSSIBLE SLIPPED DISC NOTIFIED ME ON THE 29TH OF NOV.
DROPPED PIECE OF DRILL STEEL WHICH STRUCK HIS RIGHT RING FINGER (POSSIBLE FRACTURE).
