Employee was loading supplies, bent over and picked up some half headers injuring lower back.

The roof fall was 15' wide, 40' long and 6' thick. It was bolted with 4' fully grouted resin bolts. This fall is 1 break inby spad #1242 in #6 entry. This fall has been timbered off and will not be cleaned up. The fall is 4000' from the working section.

THIS FALL FELL IN INTERSECTION 12 BRK. OUTBY SECTION. AT SPAD # 1388, #6 ENTRY, WEST SECTION. TIMBER OFF, WE ARE NOT GOING TO CLEAN UP. APPROX. 4'THICK X 20'WIDE X 20'LONG. 4'RESIN WAS USED.
Employee was operating a Mac-8 mantrip when he lost the wheel and it slipped from his hand injuring his wrist.
The ee was cutting a piece of his knee pad when he cut his thumb requiring stitches.

Fall in the no. 4 entry sometime between Jan. 20 & Jan. 21, 2005. The main power was off and all the men were knocked off. Fall approximately 60' long, 5' high. Bolted with 4' glue bolts; grade 60; 19' wide. Single crack on left rib, slate top. Miner was in #4 when power knocked. Rock on the miner and the retriever was used to pull miner out Saturday.
