An employee was trying to remove a skid steer bucket from the machine. The employee was using a hammer to tap the top latches to release the bucket. When the employee tapped the last latch, the bucket popped off and landed on EE's right foot causing two fractures in the 3rd metatarsal bone of the right foot.

Showing all 12Two employees were replacing I beams on the scale. The employee in front was aligning the cross beam with a sledgehammer. When the employee struck the beam, it bounced off of the beam and struck the second employee in the upper lip. Employee received 8 sutures to the upper lip.
An employee was helping change belt rollers on LC-2 belt line and felt a pop in their back.
Miner was exiting a front end loader when EE's boot got caught on the tread of the access step resulting in the miner falling approximately 6 feet landing on their left side. Miner sustained injury to the left hip & knee, head and neck.
While performing maintenance on the dewatering wheel, miner struck his left thumb with a hammer.

EE was removing electric wiring from a conveyor. EE was using a pocket knife to remove a zip tie that was holding the wire to the conveyor. The knife slipped and struck ee on the left bicep resulting in a puncture wound that required 1 stitch to close.
EE was dismounting loader, stepped on bottom step of loader and slipped off twisting knee. EE states the ladder was slippery and frosted. Claims a little bit of pain but it went away. EE said he noticed swelling Friday night and was painful and could not get in to see his doctor until Monday morning.
Grinding I beam, slag bounced off safety glasses into eye. Employee was wearing safety glasses.
2 EMPLOYEES WERE IN THE PROCESS OF INSTALLING A NEW CUTTING EDGE ON ML53 988 CAT LOADER BUCKET. ONE OF THE CUTTING EDGES FELL OFF THE FLOOR JACK WE USE TO LIFT THE EDGE INTO PLACE AND CAUGHT INJURED'S FINGER BETWEEN EDGE AND BLACKTOP.
Fell while fueling loader, claimed injury to ribs, saw Dr on 5-7-04, returned to work 5-10-04, worked no problem until 5-19-04, went to Dr again, and off until 5-20-04.
