Showing all 13Glove was caught on a moving beam that was being set to support an auger at wash plant. He pulled away and injured shoulder. Employee felt like he had pulled his shoulder and needed to be seen by a doctor. He was seen and was back to work same day.
Two employees were moving steel I-beams and one smashed the tip of his right ring finger.
Was removing screens, pulling them out of the screen deck with a rope. Rope came untied from hook, causing me to lose my balance & twisted my right knee.
Welding on ladder broke while greasing crusher. Fell and strained muscle on right shoulder and ribs.
Employee complained of slight pain or discomfort in Left Wrist, did not know of any work contributing factors at that time. Employee was treated at Prescott Valley Primary and Urgent care with no confirmed diagnosis.
While removing trash bags from an elevated shelf in the office restroom, employee also pulled down a container of AjAx powdered cleaner with bleach. The container upended and emptied a portion of its contents onto her head, face, and in her eyes.
Employee was working on a wash plant material chute. When he was finished he was climbing out and a piece of dirt fell into his right eye under his safety glasses.
BEE STING TO WRIST.
Taking apart Duracell and a piece of rock fell into eye.
Employee had tub ring chained to loader bucket and elevated in air. When reaching to retrieve hammer under tub ring, chain shifted and dropped on his finger.
Cleaning permanent magnet over conveyor belt & was poked by rusty piece of wire about 1/2" deep on right arm
CUTTING SHIELD OFF OF AN ELECTRICAL WIRE & KNIFE SLIPPED & CUT ACROSS PALM OF HAND.
