WHILE INSPECTING THE SPIRALS OF THE WASHER MACHINE THE SPIRAL WAS JAMMED WITH SAND, HE PULLED THE WATER HOSE TO FLUSH IT WHEN HIS FEET BECAME TRAPPED IN THE WATER HOSE. HE LOST HIS BALANCE AND WHILE FALLING WAS ABLE TO GRAB A SIDE LADDER. HIS LEFT FOOT GOT CAUGHT BETWEEN THE LADDER AND THE SPIRAL AND SPRAINED HIS LEFT KNEE,LEG AND INJURED HIS LOWER BACK AND HE HIT HIS LEFT ARM. HE
THE LOADER BRAKES HAD BEEN FIXED. A LEAKAGE OF BRAKE FLUID CONTINUED AND THE MECHANIC WAS CALLED TO FIX IT. THE OPERATORS WERE DOING THE INSPECTIONS AT THE BEGINNING OF EACH SHIFT AND ADDING T HE NECESSARY BRAKE FLUID. THIS OPERATOR DID NOT MAKE THE BEGINNING INSPECTION AND THE EQUIPMENT RAN OUT OF BRAKE FLUID CAUSING THE BRAKE FAILURE.
