Employee was exiting truck, when foot slipped off bottom step and landed awkwardly, rolling ankle. Employee continued to work until end of shift believing had just sprained ankle. When home employee removed boot and ankle began to swell and became painful. Went to clinic where it was determined employee had hairline fracture in right ankle and possible ligament damage.

Showing all 13Employee was using utility knife to cut conveyor belt when utility knife slipped and cut palm of left hand. Laceration required 4 stitches and tetanus shot. Employee was treated at local hospital and released with no restrictions.
EE had raised empty box on dump truck and released tailgate. Tailgate did not property released so ee attempted to manually release tailgate when got fingers pinched between tailgate and tailgate spreader chain. Contact resulted in lacerations to left hand index and middle fingers which required stitches. EE was treated at local hospital and released.
EE was using LP torch to cut metal on inner screen box of screening plant when hot metal slag flew back & small piece went into EE's rt ear. Was taken to local clinic for exam, after follow up visit EE was referred to specialist on 4/1/16. EE has not had any lost workdays, hasn't been placed on restricted duty, nor rec'd any med. treatment till being seen by specialist
Employee was drying belt utilizing a long handled large LP torch when torch became entangled and pulled employees hand into metal guard protecting the pulley and belt. The resulting contact caused several lacerations to the left hand which required sutures. Employee was treated at local hospital and released.
EE was placing paperwork in truck that was loaded on lowboy trailer. When getting out of truck, he mis-stepped & twisted his knee. EE was able to continue to work for remainder of shift, but could not return the next morning due to swelling & pain. On 6/18/14 Dr drained fluid from knee, told EE to rest for the remainder of week & to wear knee brace until knee was completely healed.
Employee was working with 2 other ee removing head pulley wiper & mounting bracket on loading bin. When lifting mounting plate off bin employee felt sharp pain in left groin. thought he had just pulled muscle. Pain still present next morning so employee was sent to doctor and was diagnosed with hernia. Employee under went hernia repair surgery in Fri 2/7/14.
Employee was cutting plastic zip tie on hose he was replacing when utility knife slipped and cut his right hand palm & upper wrist. Injury required 5 stitches.
EE was working on loading bin, when getting down from unit he slipped on ice and fell backwards hitting his head on the tire rim of the bin he was working on. EE was taken to Clinic and relieved 6 staples. EE was allowed to RTW for remainder of shift with restriction that he would work at ground level. He did miss the following day 1/29/13 due to headache.
Employee was rolling up a cord on the floor of the shop, when he tripped over the cord falling to the floor.
EE was carrying trash cans out of generator house,he missed a step and fell forward onto the ground. He put his hands down to break the fall and broke left wrist.
Employee was digging holes for new fence, put in (hole digger) and twisted his lower back.
Employee was attempting to climb up the steps into his loader. His right foot got wedged between steps as he grabbed a hold of the handrails and pulled himself up. This put pressure on right leg & lower back.
