Electrician was working on pulling cable out of conduit when he felt a warm burning sensation in his groin area. Employee was treated and released. H & S was notified on 3/6/15 that the injury was indeed a hernia, which made this a reportable injury to MSHA.

A 24" diameter by 9' long screw section was being lifted by a picker. When lifted the screw swung unexpectedly striking a mechanic on the left side, pushing his right side into a handrail. This resulted in a broken rib on the left side.

While installing an oil header the employee was holding the header in place while another employee was threading the union, header slipped and employee caught the header and felt a pop in his right elbow. Currently the employee is on restricted duty.

Employee was moving an Acetylene bottle with a crane with the chocker slipped, EE was guiding the bottle and it jerked injuring his back. Patient was treated and released from the hospital. The company was not notified until 11/4/14 that the injury was an MSHA recordable injury.

Employee was assisting in moving a piece of equipment, through a door. equipment was being moved with a cherry picker, equipment hung up on a door jam, employee tried to free it and his hand got pinched between the equipment and the door jam.

employee that was fixing a fence that had been blown over behind our building on the FMC property. As the employees were lifting the top horizontal bar and repositioning this bar into a fence post coupling, the top horizontal bar pinched his left hand at the thumb between the post and the fence post coupling resulting in a laceration on his left hand thumb.

EE bent down felt a slight pain in lower back, while moving valve. EE reported to the onsite medical facility the next day. Treated and released without restriction. EE was later treated by Specialist, prescribed medication for pain relief which made this recordable on 7/24/14. Company was not informed that it had become reportable until a later. (Date Unknown)

Employee was carrying water hose, while crossing a roof bolter cable it took up slack causing the cable to whip, catching the EE causing him to trip and roll his ankle. Injury was not determined to be recordable until the employee returned to his regular scheduled duty, and was put on restricted duty because of his injury.

EE had been using a die grinder throughout the day, later in the day noticed an irritation in his left eye. Metal shaving was found in his eye the next day after reporting to the onsite medical facility to have it examined. Employee received antibiotic drops for treatment after the metal shaving was removed.

EE was cutting the linkage on the cage of the crusher when a rock came down the chute bouncing off the rotor and through the opening striking the EE on the head, causing a laceration which required stitches, and/or staples.

Employee was lifting steel weights for a gravity belt take up to remove them. At the end of the shift he reported pain in the abdomen. He was referred to a doctor, FMC received the medical information on 10/22/2013 to show this hernia was work related.

Operator strung out water hose that was twisted. Hose slipped out of his hands and sprayed his leg with scalding hot water. Injured area became blistered and resulted in a 2nd Degree burn on upper right thigh.

While installing roof bolt,the employee reached to grab the steel an put his other hand on the top of the drill box, other steel came out of the roof landing and landed on his hand smashing his finger between the steel and the box. Employee was transferred to ER and received stitches. Returned to work with no restrictions.

On 1/24 a.m. EE had been welding/cutting/grinding on sections of the ore dist. building installing unistrat. He was wearing all required PPE but at some point around 9:00 a.m. he felt a foreign object in his eye. Assuming it was dust he attempted to wash it out, when that failed he notified the safety dept.

While hanging a cable up, he stepped on the first rung of a two step ladder and felt the ladder tip backwards causing him to fall onto the belt line hitting his head on the belt line rail. The fall caused a head wound requiring 8 staples, to the back of his head.

Mechanic was welding a baffle plate inside Mono 1 dissolver, while making an up hill weld the mechanic came into contact with a sharp edge of the baffle cutting his right index finger. The mechanic was sent to Medcor where his finger was evaluated and the laceration was medically glued.

Mechanics were removing a pipe, when they cut the bolts supporting it the temporary support allowed the pipe to twist and fall striking the employee on the head, shoulder and back. The employee received a slight concussion.

On the bagging line a bag was pushed off the loading spout and began spilling. The operator reached to his left to push the e stop and strained his shoulder. On February 7 the employee sought medical treatment and was put on restricted duty.

A miner was barring down some top that was over a disconnect box. When the rock hit the box it bounced and struck the miner's right foot, fracturing one bone in the right foot. The miner had X-rays taken on 12/19/2011 and that is when the broken bone was discovered. Prior to the 19th the injury was not thought to be serious.

Employee was aligning the load spout to the Railcar. He was aligning the spout with his right leg. He was supporting his posture and gripping the hand rail. As he pushed with his right leg. He felt a sharp pain run through the back side of his right upper leg which resulted in a torn hamstring muscle.

While the employee was traveling to the mining section to begin work the nut retaining the pitman arm fell off causing the jeep to veer to the left climbing the rib and rolling over. Initial examination at the hospital showed no injury. Reexamination on 7/8/11 showed a fractured vertebrae. At that time the doctor put him off work.

Employee was using a 1/2 inch drill to drill a hole in sheet metal. The hole saw grabbed, causing the drill to spin out of his hand. The drill had a side handle attached and the twisting of the drill caused the side handle to strain his left thumb. Injury became reportable when the employee had surgical intervention on 7/19/2011.
