Injured was helping welder line up pipe to weld to plate. Injured stuck right hand between pipe and plate when welder pushed on pipe to to line up for welding. Injured was wearing gloves but still fractured three fingers on right hand.

Showing all 44Injured was helping remove wear plates off loader bucket. Injured tried to lift a plate and throw in loader bucket. Injured states they felt lower back strain.
Employee failed to unplug chop saw prior to changing gout blade. While holding wrench on nut, they activated the saw which twisted wrist back. Injured saw doctor for strain but lost no time. On 3/10/2021 injured is now going to physical therapy but still able to work with restrictions.
Injured employee states they fell into gravel chute when they stepped out beyond walkway to weld edge strap on shaker lip. Injured state they bruised back and side of torso. EE was sent to Doctor who released them to return back without limitations. 4/01/2020 Injured went to first PT visit. Still not losing time.

Injured was struck by 4" dia. x 4'long pipe as it was being suspended with crane. Chain rigging slid off pipe about 6' from ground. Pipe hit structure before striking Injured on Left foot. Injured was wearing steel toe boots. Need to be on crutches for two weeks for minor bone fracture to top of foot.
Injured ee removed screen guard on Dewater conveyor stacker to examine roller while conveyor was turning. As ee leaned in to get closer look adjacent top roller caught a tool in ee hand and ran forearm through roller. No fractured bones, no open wound, injured suffered contusion to right forearm.
Employee was greasing bearings with pneumatic grease gun when ee accidently shot chest with gun while putting gun away. Abrasion to chest wall from impact. Employee was released back to work with no loss time. Additional visits were later needed due to slow healing of wound. Employee is diabetic and restrictions are keep area clean and dry.
Injured employee was shoveling material in finish tunnel when Injured felt sharp pain to lower back. Injured did report to Foreman but refused to go to doctor. Five days later Injured wanted to get back checked out by doctor. Treated and released with restrictions. No lost time from work.
The hydraulic hose on boom truck crane malfunctioned causing the tire on the crane to swing and strike employee on the right leg/knee area.
Injured struck back of left hand against control house metal door frame. Injured quickly turned to go into control house when EE struck and fractured 3rd. metacarpal bone on hand. Employee loss no time and is working light duty.
Injured jumped from one level to another while on work boat. Injured slipped twisting his left ankle. Injured loss no time from work and is working normal hours but in light duty area.
Injured employee was cleaning plant floor with shovel when he slipped as he was pushing shovel with left foot. Injured fell and landed on buttock with right leg underneath him. Injured did not report until next day. Injured suffered groin pull and is on light duty.
Injured employee was doing repairs on holes in vibrating screen deck. When Injured reached for torch, he stepped back into hole of screen causing laceration to Right lower leg. Injured was taken to doctor and given stitches to close up wound around calf. Injured was told to go home for two days and return back to work for light duty.
Employee was lifting heavy gears to install at plant. Injured states he felt L. shoulder pain but he failed to report. Injured later saw his personal doctor and just recently reported to Supervisor. Not loosing time, but going to therapy.
While working on cone crusher feed plate, Plant welder swung sledge hammer, head of hammer came off, ricocheted off feed plate and hit injured on top of left hand. Injured suffered hair line fracture to small bone on hand.
Equip. Operator was helping mechanic in shop when he went to get container from haul truck. Due to it raining Oper. fell when he reached for last ladder rung and landed on arm/shoulder. He refused medical attention. He recently decided to have doctor do MRI. Never lost time but now may need rotator cuff surgery.
Injured was helping change conveyor through rollers and cut his left hand on sharp edge of roller as roller slipped from his grasp. Injured was wearing gloves and still cut palm of hand. He needed stitches to close laceration. He lost no time.
Injured was helping startup 6" waterline from Ground Well. After repairs to hose connection, plugged line burst open and rubber water line struck injured in Left leg below his knee. Injured was diagnosed with contusion to leg with no fractures. Injured was sent home for two days. Back to work on Monday.
While hammering holes in conveyor belt, IE struck his left pinky finger on tip, with hammer. Tip of finger need stitches to close wound. IE went back to work without any loss time or loss wages and is doing his regular work duties of monitoring his plant.
On 04/30/2012, Injured Superintendent contacted me that he needs a MRI on Right Knee and he needs to report injury. I contested but Workmans comp will cover injury. Injured states he stepped on rock causing discomfort to knee. He didnt immediately report it thinking pain would stop. Orthopedic Dr. says he needs surgery for meniscus tear. At this time he has not lost any time.
Rather than getting a nearby ladder, Injured Employee (IE) was using a 55 gallon drum to stand on to reach a chain on cherry picker. IE fell off drum when trying to get down. IE struck his lower left side rib on edge of drum suffering mild rib fracture.
EE was opening a metal container of welding rods, when he took his glove off to get a better grip he cut his finger on the edge of the container. EE needed four stitches to close the laceration. Working light duty, working the same hours.
Injured ee met with foreman prior to going to plant to assist in removing shaker guard to replace drive belts injured ee told foreman that morning that his back hurt he though that it was possible his kidneys, but he would be able to work the day. just after removing guard, EE now claims he strained back in same area of initial pain. While using a steel pry bar to dislodge rock from jaw crusher, Injured employee slammed his right hand into side of crusher wall, causing fracture to tip of right index finger. Crusher and conveyors were locked out and Injured was wearing gloves.
Plant supervisor call for ee to assist others working on plant shaker bearing. EE stated that he did not feel good maybe indigestion; he walked over to operators control rm. and sat in a/c. After a few min the guy checked on him and found him on floor trying to get back to chair 911 was called as well as superintendent. CPR/first aid was given until help arrived.
Injured employee was walking on cleated conveyor walkway when he slipped on water wet surface, falling backwards. Employee fractured bone in left wrist needing surgery.
Employee was exiting Cat 345 Excavator when he slammed door on his right ring finger causing a laceration that needed stitches.
Ijured was working on floor (shop) to repair pulley. Crusher rotor tip that was sitting on table, adjacent to where he was working, fell off table top and struck injured on left hand. Fracture left middle finger.
EE states that he stood on a bucket to reach grease nipple. He stated that he suffered an umbilical hernia from falling on his rear. The report is late due to injured not reporting injury timely, and also due to the litigation between dr and insur. on compensability and past medical surguries in same abdomen area.
RELOCATING WATER PUMP. EE WAS PULLING WATER PUMP MARINE CABLE WHEN HE STEPPED BACK ON HIDDEN 4X4 WOODEN PIECE BELOW DEAD GRASS. EE STEPPED ON WHAT HE THOUGHT WAS SOLID GROUND. HIS LEFT ANKLE TWISTED, CAUSING FRACTURE TO L. ANKLE. SURGERY ON 6/6/05 TO FIX BREAK.
EE STEPPED BACK OFF OF EXCAVATOR TRACKS WHILE SERVICING UNIT. EE LANDED ON HIS FEET, BUT STATES HE HEARD POP IN RIGHT KNEE. EE DID NOT REPORT INJURY TO OFFICE UNTIL 3/29/05. EE WORKED REGULAR HOURS UNTIL 5/20/05, THE DAY HE HAD ARTHROSCOPIC SURGERY TO RIGHT KNEE.
Injured stepped down from loader to help plant operator wash at plant. Injured slipped on wet surface injuring left shoulder. Injured reported injury but refused medical or 1st aid until 1/25/05. Diagnosed with left shoulder sprain. Injured not losing time, regular work assignments w/therapy.
INJURED EMPLOYEE THREW A ROLLER CAN UP ABOVE HIM. CAN DID NOT LAND ON CONVEYOR BELT, BUT BOUNCED BACK AND STRUCK INJURED ON RIGHT HAND CUTTING MIDDLE FINGER NEAR KNUCKLE. EMPLOYEE GOT 4 STITCHES AND RETURNED BACK TO REGULAR DUTY.
EE jumped from dozer (D8) tracks, injury was caused by twisting of right knee when he landed. Diagnosis is strain to right knee.
INJURED EE WAS HELPING RIG UP MATERIAL TESTING RIG. HELPER WAS LOWERING IRON TUB FROM TURNTABLE WHEN HE GAVE TOO MUCH SLACK IN CABLE CAUSING TUB TO HIT INJURED ON HIP.
WHILE TRYING TO REPLACE CABLE ON DREDGE SPUD, INJURED GOT LEFT INDEX FINGER CAUGHT BETWEEN SPUD SLEEVE AND STEEL BAR. INJURED SMASHED TIP OF INDEX FINGER.
ON 12/30/02 INJURED REPORTED FEELING DISCOMFORT AND PAIN WHILE TRYING TO FLIP AN IRON PIPE BELL THAT HE WAS RESURFACING. DOCTOR ORIGINALLY DIAGNOSED A GROIN PULL. NEW DIAGNOSIS SHOWS INGUINIAL HERNIA TO BE REPAIRED 1/16/03.
INJURED WAS IN SAND SCREW TUB TIGHTENING BOLTS. WHEN HE PULLED BACK ON THE WRENCH HE FELL OUT OF THE TUB. INJURED FRACTURED HIS TAILBONE.
INJURED HIT FIELD CONVEYOR WITH BELTLINE TRUCK. INJURED TRIED TO PULL OUT DAMAGED FRONT FENDER ANDT RT. PINKY FINGER ON SHARP POWDER METAL. INJURED RECEIVED 4 STITCHES FROM CO. DR. AND RETURNE D BACK TO WORK ON REGULAR SHIFT.
EE WAS BENT OVER INSTALLING WATER COOLER ON EQUIPMENT. WHEN HE STOOD UP HE FELT PAIN IN LOWER BACK.
INJURED WAS HAMMERING ON DREDGE PIPE BELL, WHEN HIS HAMMER DEFLECTED OFF TOP RIM OF PIPE BELL AND HIT HIS LEFT LITTLE FINGER.
INJURED WAS ADDING PIPE COUPLING TO DREDGE DISCHARGE PIPE. COUPLING SLIPPED AND SHARP EDGE CUT RIGHT FINGER ON HAND.
