Miner was lifting a roll of rubber hose into box of ORV and felt something pop in their arm. A co-worker helped them remove their coveralls, they were in a great deal of pain so the supervisor transported them to the local ER. Miner was examined, and an MRI was performed confirming a torn bicep muscle. Surgery is required to repair.

Miner was using a come-a-long to raise a take up pulley, the hook came loose and rebounded up between conveyor and toe-kick and glanced off miners cheek causing a laceration.

Miner was attempting to blow debris out of a flywheel assemble on the cone crusher through a small access whole in the cover. The nozzle either became clogged or EE inadvertently extended the tip into the opening making contact with the flywheel. This caused the nozzle to recoil cutting the palm of the miners hand resulting in a dr visit & stitches.

The Quality Control Technician was taking samples of finished material and strained her shoulder. The initial report was classified as non-reportable and has since been re-classified as reportable.

In the normal course of doing his daily duties, EE strained his left knee. Unknown exact cause of injury.

Fell from top of staircase of the crusher control house. Did three rolls down the steel staircase scraping both knees, bruising back and neck. No stitches required, employee released by doctor to regular duties as a supervisor.

Picked up a piece of metal to put in the metal bin. Smashed finger against the end of the metal bin. Received a laceration to the first digit of the left hand.

Cutting a piece of conveyor belt with a razor knife. The knife slipped and he stabbed his left hand. Employee received 4 stitches to close the wound.

Pouring concrete into an above ground form. 12 inches off the ground leaning on the mixer chute, when it moved he fell down on some form iron supports and lacerated his left lower leg. Employee received steri-strips to close and help heal wound.

Cutting beam with track burner. Quickly reached to shut off machine as waste steel fell on foot. Received bruised left foot. No time loss.

Welding without face shield, later found out employee had burned his eyes. Safety violation!

Removing the wheels from the screen trailer with the 1" impact gun. The air discharge from the gun blew the materials in the rim out and something made it past the safety glasses and into the left eye. Foreign body had to be removed from the left eye by a medical professional. No permanent damage or time loss due to this incident.

Hearing loss due to years working on portable crusher.

Removing pump impeller from barge pump with a Pettibone crane. Impeller came loose and rotated causing the chain hook to come off the vane striking him in the sternum and throwing him 6 ft and landing on top of fellow employee knocking them both to the ground. As a result, the employee received a fractured sternum.

Driving haul truck up to the load out hopper, stopped the truck and reached out the window to start the conveyor. When the start/stop pendant was contacted, employee received an electrical shock.

Employee was lifting a set of troughing rollers when he discovered a bees nest on the rollers. He attempted to throw the nest away from himself and in the process of throwing felt a sharp pain in his back on the right side. After this incident, the pain was constant.

Changing iron in a crushing unit. Picking heavy iron with cables and cable rolled and caught finger. Cable pulled fingernail and end of finger off in the glove. Unable to save end of middle finger on the left hand.

Attempting to reach top of a steel plate to attach clamp, in effort to reach higher on plate he climbed up on an ecology block retaining wall & lost balance falling over the wall 80" to ground. The wall was 1 block high on the working side 3 blocks on the wall side. Wall was designed to interlock not flat on top & not intended to be used as work platform or walkway.

Standing by controls, rock came out of jaw plant and hit him under chin.

GETTING READY TO CHANGE MOTOR ON CONVEYOR. HAD FOOT ON CHAIN SPROCKET, LEFT ARM LAYING ON CONVEYOR. FOOT SLIPPED OFF SPROCKET PUTTING EXCESS PRESSURE ON LEFT ARM AND SHOULDER.

CLEANING DIRT OFF FLYWHEEL OF WASH PLANT, SCREEN #76-050. THE CLEANING BAR SLIPPED OFF THE FLYWHEEL AND STRUCK WORKER IN LEFT EYE.

WHILE REMOVING A 7/8" LIFTING CABLE FROM THE HOOK OF A CRANE, PULLED ON CABLE AND IT CAME LOOSE SUDDENLY. IT STRUCK WORKER ON THE BRIDGE OF THE NOSE.

WASHING CONCRETE SLAB WITH FIRE HOSE. LOST FOOTING SLIPPED AND FELL BACKWARDS HITTING RIGHT HAND ON STEEL BEAM.

GREASING FEEDER UNIT ROCK FLEW OUT OF MASABI SCREEN UNIT. FLYWHEEL ON SAME UNIT CAUGHT ROCK & THREW IT. ROCK STRUCK EE IN THE MOUTH

HE PULLED LOCK BOLTS OUT OF A CONVEYOR BEFORE ITWAS HOOKED UP TO THE CRANE. WITHOUT SUPPORTS, ITFELL BACKWARD HITTING HIM IN THE HEAD.

INJURY REPORTED TO EMPLOYER ON 11/14/00. LIFTING SCREENS AT WASH PLANT THROUGHOUT NIGHT. RE-INJURED BACK. ORIGINAL INJURY OCCURRED IN 1991.

OPERATING TRUCK IN PIT, BACK WAS AGGRAVATED. PREVIOUS BACK INJURY FROM 6-7 YRS. AGO WAS AGGRAVATED.

EE ACCIDENTALLY SHIFTED FROM REVERSE TO FORWARD INSTEAD OF TO REVERSE 2ND GEAR.
