Showing all 11Driver was on the reach of the pup trailer when load was topped off. Rock bounced off the bucket of the loader onto truck box and struck him.
Rock rolled down shot, hit on another rock and broke apart, hit employee in the eye. The employee waited a few days to seek medical attention because he thought it was fine.
Plastic handle screw driver was hit with hammer and the end of handle exploded and struck EE in the left eye.
The employee was installing final insulated hangers on high voltage cable without power being turned off. Contacted old splice area, received flash burn to right hand, finger, wrist.

Removing broken screen deck, pinched right middle finger, swollen and bruised finger tip.

WAS WORKING IN A CONFINED AREA GRINDING. EMPLOYEE WAS WEARING SAFETY GLASSES BUT OBJECT MANAGED TO GET IN AROUND GLASSES.
