Showing all 44Two employees were horseplaying. The first employee had the other in a head lock then fell down, hitting the other's head on the ground causing a mild concussion.
EE was standing in mud which the rain caused. He turned his body in order to take a step. EE felt his foot pop and he was in pain. The doctor said that he broke his right ankle.
Strained elbow on right arm while installing head pulley. EE was given medication and was placed on limited use of right elbow restriction for 2 weeks.
Pain in left shoulder. This incident happened on 3/6/07 according to EE, however he did not report the incident to anyone until 3/15/07. **Dr prescribed physical therapy for 3 weeks.

Removing a bolt with a crescent wrench. Wrench slipped off the nut a struck EE in the nose. He had just had surgery for a dievated septum and was healing when he hit his nose with wrench. He had to have his nose re-set again.
A large rock fell off conveyor belt striking hard hat on head, causing a laceration to top of head.
An employee was using a blow torch to cut a cable. He crossed the torch over the left wrist of injured employee causing a small burn to inj ee wrist.
Cutting a piece of rubber on the bobcat skid steer with a razor knife. EE slipped with knife cutting his left hand requiring stitches in little finger.
Employee was changing oil in the cone. As he removed plug the drain valve also unscrewed and hot oil splashed onto two fingers of the left hand burning him.
A LARGE ROCK WAS STUCK ON CONVEYOR. EE WAS PUSHING ON ROCK & FELT PAIN IN RIGHT SHOULDER FROM STRAINING.
EE was helping to remove screen, when it slipped sideways striking ee right forearm causing a contuison he reported the lake clinic. He was treated with splint pain meds and motrid and returned to work on light duty restriction for 7 days.
Employee was standing near another employee welding and a small spark was carried by the wind and blew into his left eye. He went to Lake Clinic the next day when he finally reported the incident to his supervisor.
Changing a tooth on a loader, using a hammer to drive it on when a piece of metal flaked off and punctured his skin on his chest. He went to a doctor, they removed the metal and put two stitches and allowed employee to return to work full duty.
EE WAS CLEANING MUD OUT OF THE SCREEN WITH A HAMMER. HE STRUCK HIS ELBOW ON SIDE OF PLANT CAUSING A CONTUSION. HE WAS STILL HURTING ON 6/02/05 SO HE WENT TO DOCTOR TO GET IT CHECKED OUT. IT WAS JUST A CONTUSION, THE DOCTOR PLACED HIM ON RESTRICTION UNTIL 6/09/05
On cat walk of 6 by 20 screen. He backed up thinking he was against railing, but was in front of ladder opening and he stepped off cat walk falling to ground, landing on his feet. He didn't go to Dr until 4-8-05, and was then put on stay home rest for three days.
Grinding on piece of metal he had just completed welding together. A piece of metal flipped up under safety glasses and he had to go to eye Dr. to have small piece of metal removed from eyelid.
EE WAS CUTTING A BELT USING A KNIFE WHEN IT SLIPPED AND CUT HIS THUMB. REQUIRED STITCHES TO CLOSE WOUND.
EE WAS CLIMBING ONTO A CONVEYOR TO MAKE A REPAIR WHEN HE STRUCK HIS KNEE ON THE IRON FRAME, LACERATING HIS KNEE. IT TOOK STITCHES TO CLOSE THE WOUND. NO LOST TIME.
THE EE WAS WORKING NEAR A CREW THAT WAS WELDING. EE DID NOT PROTECT HIS EYES BY WEARING EYE PROTECTION. THAT NIGHT HIS EYES BEGAN TO BURN. HE WENT TO AN EMERGENCY ROOM OF HOSPITAL. THEY GA VE HIM PAIN MEDICATION AND TOLD HIM TO STAY HOME THE NEXT DAY.
EE WAS WALKING OUT TO SHOVEL DIRT WHEN HE SLIPPED ON SOME ICE & STRAINED HIS BACK
EE WAS MOVING A SCRAP PILE OF METAL WHEN A PIECE OF METAL FELL ON HIS FOOT, CAUSING A CONTUSION. DOCTOR X-RAYED & SAID FOOT WAS ONLY BRUISED AND EE COULD RETURN TO WORK. EE STAYED HOME ANYWAY. THEN WENT TO ANOTHER DOCTOR A WEEK LATER AND WAS GIVEN TIME OFF.
LIFTING THE EE STEPS MOVING CRUSHER TO NEW LOCATION.
PULLING ON A CHAIN BEING USED TO LIFT A CONVEYOR. THE CHAIN BECAME STUCK. THE EMPLOYEE PULLED HARDER ON THE CHAIN WHEN IT FREED UP IT STRUCK EMPLOYEE IN MOUTH. TWO TEETH (MIDDLE BOTTOM) WERE K NOCKED LOOSE. EE WILL BE CHECKED IN ABOUT A WEEK TO SEE IF ANY TREATMENT WILL BE NEEDED.
LOADING EQUIPMENT TO MOVE CRUSHER, WHEN EE STEPPED ON A CONVEYOR & LOST HIS FOOTING DUE TO RAIN & SNOW ON EQUIPMENT. INJURED HIS LEFT KNEE AS HESTRUCK THE FRAME ON CONVEYOR
EE WAS LOADING SOME 8X8 BOARDS ONTO A TRANSPORT TRUCK PREPARING TO MOVE CRUSHER TO A NEW PIT. HE DROPPED THE BOARD ON HIS FINGER, HE NEEDED 8 STITCHES TO CLOSE THE WOUND.
EE WAS COLLECTING SAMPLES. WHEN SHE TRIPPED AND FELL BREAKING HER RIGHT ARM.
EE WAS REMOVING THE SCREENS ON THE CRUSHER AND PULLED THE SCREEN OVER HIS ARM CAUSING A LACERATION.
EMPLOYEE WAS CUTTING WITH A TORCH WHEN HE TURNED, RUNNING THE FLAME ACROSS THE RIGHT HAND OF ANOTHER EMPLOYEE CAUSING A BURN TO HIS HAND.
EMPLOYEE WAS PULLING WITH A WRENCH ON A STUCK FITTING WHEN THE WRENCH SLIPPED CAUSING HAND TO CONTACT A SHARP EDGE. EMPLOYEE LACERATED HIS LEFT THUMB REQUIRING STITCHES.
EE WAS WORKING BY ANOTHER EE THAT WAS WELDING,HEWATCHED WITHOUT A WELDING HELMET AND BURNED HIS EYES.
EMPLOYEE WAS CUTTING A CONVEYER BELT. HE SLIPPED AND CUT HIS RIGHT THIGH.
EE WAS HITTING A CHISEL WITH A HAMMER WHEN THE HEAD FELL OFF AND STRUCK EE ON HIS PINKY FINGER OF L. HAND CAUSING HIM U LOOSE NAIL AND REQUIRED STITCHES TO REPAIR WOUND.
UNKNOWN EMPLOYEE SAID WIND WAS BLOWING HARD THAT DAY AND SOMETHING BLEW INTO HIS EYE. HE THOUGHTIT WOULD COME OUT. A FEW DAYS LATER HE WENT TO DOCTOR AND A SMALL PIECE OF METAL WAS REMOVED.
ROCK WAS STUCK IN THE GRIZZLY. EMPLOYEE PUSHED ON THE ROCK TO FREE IT. WHEN IT CAME LOOSE HE SMASHED HIS FINGER ON HIS LEFT HAND.
LABORER WAS BREAKING OUT A STUCK ROCK IN FEED TRAP WHEN THE LOADER OPERATOR DUMPED A LOAD OF ROCK AND MISSING THE FEED HOPPER, DUMPING THE LOAD ONTO THE LABORER.
EE GRABBED THE HAND RAIL & SWUNG HIS LEGS OUT DOWN 4 STEPS TO THE GROUND THIS CAUSED THE HAND RAIL TO BE PULLED OUTO F ITS FRAME WORK MAKING' EE TO FALL TO GROUND EE HAS STRAINED LEFT SHOULD ER
EMPLOYEE WAS LIFTING AN IRON FEED PLATE FOR THE BARMAC, UP ON A CAT WALK, WHEN HE FELT PAIN IN HIS LOWER BACK.
EE WAS OBSERVING CRUSHER AFTER THE POWER WAS SHUT OFF. AS THE CRUSHER WAS SLOWING TO A STOP A LARGE ROCK BOUNCED UP HITTING GUARD. THE GUARD BROKE LOOSE AND STRUCK EE IN FACE, CAUSED LACERATIO N.
AS EE WAS PLACING THE RUBBER GASKET FOR THE CHUTE LIP THE LOAD SHIFTED AND CAUGHT EE'S L RING FINGER BETWEEN THE CHUTE AND THE FRAME OF THE CRUSHER. THE FINGER HAD AN AVULSION AND BROKEN BONE.
A LARGE CHUNK OF FROZEN DIRT & ROCK FELL FROM THE CONVEYOR ONTO EE STRIKING HIM IN THE BACK OF THE HEAD NEAR HIS LEFT EAR.
