Taking a drive shaft out of a haul truck when drive shaft fell smashing left thumb between shaft and rear stabilizer.

EE said he was moving the kiln gun away from the kiln after shooting a ring out and slipped off diamond plate and hurt his pinky.

Employee was using a 4 in grinder

Putting a conveyor belt splice in the 3/4 conveyor @ the finish plant.

A breaking switch located in the portable plant control room shorted out causing a flash where the breaker was turned on causing minor burn to employee's hand.
