Employee was running the dozer on grade, encountered ruts and jarred his back. Back became stiff over the weekend requiring employee to seek Chiropractic medical attention beginning 7/16/2007.

Very strong winds on that day, with gusts in excess of 60 mph. Mechanic was hooking up airlines on water tanker truck when the wind blew him off of the truck. Jarring back and neck when landing on ground. Back & neck where stiff, but didn't require immediate medical attention. Receiving Chiropractic treatments.

Employee was cutting drive blets on standard cone with a utility knife. He slipped and cut his right forearm. Cut required three stiches.

Employee strained his back. Employee was standing in chute, pulling on belting. He slipped on rock in chute and fell forward into front part of chute and twisted his back.

Employee was walking to car for lunch, scraper was headed to fill bin at same time. Both saw each other at same time. Both went right and then turned to left. Employee jumped away. Right front scraper tire ran over his right leg and left foot. Safety glasses broke, laceration on face. Bruising, swelling, abrasions, full extent of leg injuries not known at this time.

Employee was unloading metal screens from supplier's truck. He had his fingers through the openings in the screen when the supplier's employee let go of the screens. The screens slid down the truck pinching his finger and bending it sideways.

WHILE MOVING &/OR BENDING TO PICK UP A LARGE ROCK OFF OF A CONVEYOR BELT, EE HEARD A POP IN HIS UPPER BACK. HE DID NOT ACTUALLY PICK UP THE ROCK HE WAS JUST MOVING/BENDING TO REACH FOR ROCK.

WHILE CHANGING SCREENS, EMPLOYEE WAS TIGHTENING A BOLT. THE RATCHET SLIPPED CAUSING HIS RIGHT ELBOW TO HIT 2 SMALL BOLTS BRUISING IT. AT E.R DOCTOR ADVISED WORK FOR ONE WEEK, IMMOBILIZED ELBOW & PUT ARM IN A SLING. TYLENOL AS NEEDED FOR PAINONE WEEK LATER HIS HAND WAS SHAKING SO INDEFINITE RETURN TO WORK DATE.
