During a period of heavy rainfall the drains within the bagging room became overwhelmed with water. A miner working in this area was walking through the water when they stepped on some loose gravel causing the miner to slip and fall. The miner suffered right rib contusions and contusions on their right arm and right leg.

Plowed snow turned into ice. When the operator walked out of the plant the operator slipped on the ice and sprained EE's ankle.

Employee was walking away from the fuel pump when they slipped on ice and landed on a shoulder. Employee went to the doctor in December and began physical therapy at the end of January (18 visits). Employee went for a second opinion and was put on restricted duty (4/28/21).

Employee was removing a pipe from the manifold. EE felt a pop in shoulder. Employee continued to work under normal conditions with no restrictions from doctor exam on 12/10/18. On 12/26, employee had an MRI conducted. Results came in on 1/10/19 that has now diagnosed a shoulder injury and restrictions have been implemented by doctor.

One of the sand chutes was becoming clogged with material in the north crusher. The employee was using the clearing tool to clear the material. As the employee was clearing the chute, EE struck finger between the tool and the chute frame.

EE reviewed customers paperwork for a pickup. EE let the driver know to enter the center of the building. As the EE was stepping back onto the lift truck, the truck driver entered the building at an angle, allowing the trailer to catch the lift truck dragging it into and pinning lift truck at the building and the trailer. EE hit shoulder and knee on the frame of the lift truck.

At the end of the day, the employee stepped out of the loader carrying lunch box. When EE stepped out, EE slipped on the icy platform on the loader. EE was able to grab the handle but struck left side on the machine, fracturing two ribs. Cold temperatures, rain and ice was forming throughout the day.

Contractor turned portable belt sander on as they turned the sander on its side, their finger became caught in between the rotating belt guide on the sander, creating an avulsion to left hand index finger and tip.

Lifting hood on moxy equipment #1450 for AM precheck and he twisted his back.

During the repair of a pulley, right middle finger got pinched between the pulley bearing and the frame work.

Stepping down off of a haul truck and pulled a muscle in right leg.

Victom was attempting to hang a curtin on a chute at a moving conveyor tail section. He got his hand pinched between the moving conveyor and a troughing roller injuring the first two fingers on the left hand.

Was climbing down out of a truck. Twisted right ankle on contact with ground.

Removing a cover plate on carryall trailer when it slipped and came down on left hand causing a deep cut on third finger. Onsite first aid rendered and then transported to clinic where he received 3 stitches.

Employee was climbing up onto his truck and strained the calf muscle on his right leg.

Employee was pulling cable back over drag when spring load of cable caused it to snap around which caused a frayed end of cable to puncture his arm.

Employee got a foreign body in left eye. The foreign body was picked up by fan when hitting bumps in road.

Employee operating crusher and twisted his back while shoveling a bin.

PUTTING A SIGN POST IN THE GROUND, USING A LOADER BUCKET. THE SIGN POST HIT SOLID ROCK, AND THE POST BOWED AND SLIPPED AWAY FROM THE LOADER AND HIT EE'S UPPER RIGHT ARM WITH FORCE.

CLIMBING DOWN FROM EXCAVATOR WALKWAY ONTO A LADDER. LADDER FOOT SANK INTO MUD CAUSING IT TO TIP.EMPLOYEE FELL ABOUT 4 FT TO THE GROUND.

EE HAD JUST FINISHED A SAND SAMPLE ANALYSIS AND NOTICED AN IRRITATION OF HIS RT. EYE. EE TRIED TO FLUSH IT OUT AND COULDN'T. WENT TO DR. & HAD FOREIGN OBJECT REMOVED & USED ANTI-BIOTIC OINTME NT SEVERAL TIMES A DAY TO PREVENT INFECTION.

PULLING FLIGHT INTO DRUM-FELL ONTO RIGHT HAND-LACERATION OF MIDDLE FINGER-REQUIRED STITCHES.

CHECKING 480 LINE, AS TESTING LEAD WIRES ON TESTER WERE IN WRONG POSITION RESULTING IN TESTER IMPLODING CAUSING BURNS TO HAND & FINGERS.

TRACK HOE BUCKET HIT TRUCK BED. EE WAS INSIDE CAB OF TRUCK HIS NECK WAS JARRED.

SET UP LADDER TO GO INSIDE #3 BIN - LOST BALANCE AND FELL.

BOTTOM STEP ON TRUCK BROKE IN HALF AS EE EXITED TRUCK. FELL TO GROUND & HURT BACK & LEFT LEG.

FILLING 400 BIN, FOOT GOT STUCK. DID NOT BREAK FALL. HIT WALL, SPLIT LIP, LOST A TOOTH, PAIN IN NECK.

EMPLOYEE WAS BURNED FROM A PIECE OF RECENTLY CUT STEEL THAT WAS HOT. HE WAS INSTALLING STEEL IN A CHUTE AND IT GRAZED HIS RIGHT FOREARM.

DRAGGING WASH HOSE TO CONVEYOR #1. AS HOSE UNFOLDED A HOSE CLAMP PUNCTURED LEFT FOREARM.

EMPLOYEE STEPPED ON A BOARD FROM A PALLET THAT HAD A NAIL STICKING OUT/ UP.

DURING A FOLLOW UP AUDIT ON 10/22/02 WE WERE ADVISED TO SUBMIT THRESHOLD CHANGES ON THIS FORM, CONSEQUENTLY WE ARE NOTIFYING YOU OF THE THRESHOLD SHIFT THAT OCCURRED ON THE ABOVE DATE. IN THE FUTURE WE WILL NOTIFY YOU WHEN WE ARE NOTIFIED.

INSTALLING TAIL SHAFTS ON NORTH LOG WASHERS, WORKING IN VERY AWKWARD POSITIONS, PUSHING AND PULLING HEAVY OBJECTS INTO POSITION AND TIGHTEN-ING BOLTS.

FELL OFF MACHINE AMD INTO TRUCK TOOL BOX.

A LOAD OF MATERIAL WAS DROPPED INTO THE TRUCK AND CAUSED A VIOLENT SHAKE. THE SEATBELT WAS TIGHT AND CAUSED A STRAIN.

HELPING PLACE A PULLEY ON THE CRUSHER AND TWISTED KNEE, KNEE POPPED OUT.

EE WAS CHIPPING ICE BUILDUP WITH HAMMER & HIT HIS RIGHT WRIST WITH HAMMER

EE CUT FINGER ON THE VALVE WHERE THE BAG IS PLACED FOR THE MINED MATERIALS.
