EMPLOYEE STATED: The mechanic was drilling out rivets when a small piece of metal landed on the mechanic's cheek and subsequently slid under the mechanic's goggles. Mechanic was treated by a local eye doctor with a prescription.

EMPLOYEE STATED: The lab technician was preparing a lime sample for the pulverizing mill when EE cut EE's thumb on one of the mill's blades, which resulted in stitches.

A mechanic was injured when EE and another employee attempted to move a steel plate covering a below ground water valve. While sliding the heavy plate, it hung up and then moved suddenly pinching EE's left middle finger between the floor and plate. EE was treated for the injury and received four stitches on the tip of the finger.

An electrician and co-worker were lifting an empty pallet into the back of a truck when the pallet struck the truck body. The sudden stop resulted in the employee suffering a strain on bicep. Employee received surgery on 3-30-2022 which resulted in one day of lost time and was reportable.

Employee stated was looking into the top of #10 mixing tank in LCH, when employee opened up the hatch the pressure caused lch to blow into employee's face. Employee was wearing goggles, but the LCH is watery and ran under employee's goggles.

The employee stated the pin was removed from the arm so the actuator could be rotated up and out of the way to access the arm bushing. The mechanic had a hand on top of arm lowering it into place to reinstall the pin to actuator when weight of arm went over the center and pinched left index finger between the arm and housing. Broken finger was discovered on 2-14-2022

The employee was operating a fork truck, emptying coal dumpsters into the waste pile next to the Glass Stone Plant. As the employee stepped out of the fork truck to dump the dumpster, EE twisted knee. After twisting their knee, the employee realized that the ground was uneven due to frozen material. The employee was taken to Occ Med and received an x-ray.

EMPLOYEE STATED: Was super sucking a rail car out and the hose clogged up. EE took the hose apart from the whip on the back of the truck to clear the clog. As EE was attaching the hose back to the truck, it puffed material into EE's face. EE was wearing proper eye protection, ie I force slim goggles. The employee went to the eye doctor and was prescribed a prescription.

Removing the product valve to change out the air sweeps located in the bottom hopper of a hydrate product bulk trailer, the employee was flushed with a small amount of hydrate. Hydrate got in both eyes causing irritation. Right cornea received abrasion. Employee was prescribed antibiotic eye drops.

Loadout employee received lime in eye injury when lime fell on EE while walking back to the scale house. Employee initially declined medical treatment opting for onsite first aid treatment. On day three, the eye was swollen and worse.

The employee stated Employee was replacing a broken rear leaf spring on a pickup. The spring slipped while placing on the differential and they attempted to catch it. While the employee did not get on restricted duties until 7/27/2021, so that is when became reportable.

Employee stated, Employee was unloading a tire from the service truck with one foot in the bed and stepped on the bumper with the other. EE had stepped on an impact gun on the bumper and lost EE's balance. EE then fell to the ground.

The employee stated was tramming a JLG when EE rolled over a rock with the tire of the machine. When EE rolled over the rock, the basket jerked upward quickly and the employee hit EE's finger on one of the railings in the cage. EE's left ring finger was X-rayed and found to be fractured.

While using rail car puller to push three cars down track 4, the puller derailed between the 4/5 switch and crossover road. As it derailed, the tire blew which added to the rocking motion lifting the employee out of the seat employee was buckled in to & slinging employee around. Employee immediately felt pain in the right side of neck, right shoulder and right elbow.

Employee 1 was struck by employee 2's personal vehicle while crossing the road to the parking lot. Employee 1 fell to the ground and was taken to the local hospital for evaluation. Employee 1 suffered no broken bones and was treated and released for soreness and contusions.

The employee stated they injured right thumb while grinding a piece of metal using a bench grinder in the maintenance shop. The thumb came in contact with the metal that employee was grinding causing a laceration.

EMPLOYEE STATED EE went to the bulk bagger to check on orders. While there a piece of pallet that someone had leaned up against the shelf had fallen over onto the bagger. EE turned to walk away and tripped over the wood and fell on EE's knee and hand. WITH DOCTOR EVALUATION, FOUND THAT WRIST IS BROKEN.

Operator sprained left ankle when descending a ladder and stepping onto a hose rolling ankle. Supervisor began investigation at 7:00 AM. The incident occurred in the Rework Silo. The vacuum hose was removed from the area and stored.

The operator failed to put the bed of the truck down after unloading in the mine. The collision between the truck bed and the roof caused EE to strike the steering wheel and roof of the truck cab. EE suffered multiple cuts, abrasions and fractures and was transported to a hospital where EE remains for treatment.

Employee stated EE was working on cleaning the windshield of a mechanical scaling machine and slipped on the tracks. The employee slipped from atop the tracks striking both hands on the tracks. The employee suffered a laceration to one of EE's hands and stitches were used to close the wound.

The injured employee was working on 3/19/20 working to replace a return idler. The return idler became unsecured and fell downward. As it fell the idler struck the injured employee's left wrist between the handrail and the piece of equipment. The injured employee was diagnosed with a fractured left wrist on 4/9/20 with an MRI.

EMPLOYEE STATES THAT WHILE WALKING THROUGH THE LOADOUT TO THE SOUTH TRUCK BAY, AND BOTH EYES BECAME IRRITATED. EMPLOYEE DID NOT REALIZE IT WAS LIME AT FIRST, EMPLOYEE HAD GOGGLES ON. EMPLOYEE WENT TO WOLK EYE CARE ON 12-5-2020 AND WAS PRESCRIBED EYE DROPS TO HEAL THE EYES.

Employee fell while cleaning the top of a rail car. Fell into car and fall protection was worn and used. Employee was rescued and seen by the doctor and was returned to work full duty. Later appt on 2/17/2020 it was determined EE would need surgery and was given modified duty and recommended off.

THE EMPLOYEE STATED WHILE CONDUCTING A PRESHIFT OPERATION, THE EMPLOYEE WAS OVER THE BATTERY COMPARTMENT WHEN THE BATTERY EXPLODED AND PROPELLED THE BATTERY COMPARTMENT DOOR TOWARDS THE EMPLOYEE'S LEFT KNEE AND LEFT HAND. THIS CAUSED A FRACTURE TO EE'S LEFT THUMB.

EMPLOYEE STATED THEY WERE cutting piece of 1 1/2 inch angle with a 4 inch die grinder when cutting disk caught on metal, kicked back and cut top of EE's right thumb. Employee was wearing proper PPE when issue occurred. EMPLOYEE RECEIVED 4 STITCHES AT OCCMED.
