The employee was tasked with helping fill water tank. EE was found about 30 minutes later laying face down saying EE was hurt and needed help. At this point it appears that the weight of the trailer shifted off the cribbing under the landing gears and caused the trailer to overturn, either striking against or rolling over the miner.

Showing all 46Boulder was lodged in Jaw Crusher of plant. Plant Operator looked into Jaw to get a better view of the obstruction when the boulder popped hard against an iron plate lining the jaw opening. Injured party was wearing proper eye protection, but what ended up being a sliver of metal went underneath bottom rim of eye protection and become lodged in left eye.
Employee was letting down an electrical cord hanger brace. Brace slipped out of employee's hands and the whole contraption fell on EE's leg, right above the ankle. Accident resulted in a bone fracture in lower right leg.
Employee was helping repair a conveyor belt that was torn in two different areas. After the first area was repaired, the employee was rolling the V-Belts to advance the belt to a better position for repair when employee's left hand was pulled into the V-Belt sheave, almost completely severing the thumb.
Miner was shoveling on the ground under a cone crusher unit when a rock fell out of the cone crusher and hit the miner on top of EE's hard hat. The miner was brought to the hospital to be examined in worries of a Concussion. Miner was examined and released back to work the same day.
Employee was working under the crusher feeder and a rock fell off the feeder and hit employee on top of hard hat. Employee had some pain in neck so ee went to the clinic to have to check out.
Employee was helping tear down a conveyor during mobilization of the plant. The telescoping conveyor leg tubing hit employee's hand against the frame of the conveyor. This in turn smashed the employee's right pinky.
Employee was tightening a bolt on screen with a socket wrench. The Wrench slipped and slammed hand into the side of the screen. Employee seeked medical attention to ensure it was not broke.
Employee was helping set a conveyor. When the leg of the conveyor was set down by the crane it was set on employee's foot. Employee has sought medical attention.
Employee was helping free a boulder caught in the jaw crusher and smashed hand while using a sledge hammer causing an open wound on hand. Employee went to hospital to get it dressed.
Employee was helping install a D9N Cat dozer blade when they felt their back "pop" while preforming the task. Employee is planning on seeing a doctor to get it looked at.

Employee was helping to unclog a cone crusher when a drive belt slipped and cut the tops of the middle and ring fingers of left hand. The crusher unit was de-energized and locked out at the time of the accident. Tops of both fingers were later amputated.

Employee was adjusting steel brace of a conveyor when part of the brace fell and smashed the individuals hand. The Employee was wearing gloves at the time of the accident, however he suffered some broken bones on his fingers.
Employee was cutting rock conveyor belting with a utility knife when the knife slipped resulting in a laceration between the thumb & index finger of EE's left hand.
Employee was releasing a chain binder from a load of crusher equipment. The binder twisted causing his right index finger to be crushed against the trailer frame and also resulting in a laceration that required sutures.
EE was assisting other crew members string a rock crusher conveyor belt when his left ring finger got caught in a v-belt sheave resulting in partial amputation.
Driving haul truck down a road, lost control of truck and rolled it 1 time down the road. Rolled over 1 time. Bed landed on wheels, cab rolled 1 1/4 times and came to rest w/driver's side on ground. Driver was wearing seatbelt and sustained very minor injuries.

EE NWAS DRIVING ON OFF HIGHWAY TRUCK STOCKPILING GRAVEL. HE GO TOO CLOSE TO THE EDGE AND TIPPED THE TRUCK OVER. HE WAS THROWN AROUND IN THE CAB PRETTY HARD. HE STARTED COUGHING UP BLOOD AND TA STING BLOOD IN HIS MOUTH SINCE THEN. HE QUIT WORKING FOR US THE FOLLOWING WEEK.
EE WAS HELPING TEAR DOWN JAW CRUSHER AND THE PIN ON THE HYDRAULIC JACK STUCK SO HE WAS HOLDING IT ON IT WOULD OPERATE CORRECTLY AND THE JAW SLIPPED AND SMASHED HIS LEFT THUMB BETWEEN THE JAW F RAME AND WINGWALL. HIS THUMB WAS REATTACHED AND HAS MORE SURGERIES TO HAVE DONE IT.
A ROCK SHOOK OFF THE CONVEYOR AND HIT HIM IN THE BACK WHILE HE WAS SHOVELING ROCK UNDER THE CRUSHER.
EE WAS WELDING ON EQUIPMENT AND THE AUTOMATIC SHIELD ON THE WELDING HELMET DIDN'T WORK, THE BATTERIES WERE DEAD. HE THOUGHT IT WAS THE SUN FLASHING ON THE HELMET. THE RESULT WAS WELDER BURN TO HIS EYES.
HE WAS STEAM CLEANING EQUIPMENT AND THE H OSE ON THE STEAM CLEANER BROKE AND SPRAYED HOT WATER ON HIS STOMACH.
HAD A BAD WIND STORM DURING THE DURING THE SHIFT AND EE GOT SAND, DIRT AND ROCKS EMBEDDED IN HIS EYES.
HE WAS USING A WRENCH AND A PRY BAR TOGETHER TO LOOSEN A BOLT AND PULLED TOO HARD AND SNAPPED HIS BACK.
HE WAS GRINDING ON A CRUSHER ROLLS SCREEN WHERE IT HAD BEEN WELDED AND A CHIP OF METAL SLAG GOT IN HIS EYE.
