EE was exiting the truck, missed a step and slipped. Hurting wrist
The EE alleges he hurt his back on 10/2 but did not report any injury. I spoke to the EE on 10/28 via telephone after trying several times to contact him. He states he was putting on a repaired hydraulic hose on his haul truck and hurt his back. He left work on 10/8 when he was told to find a water leak in his truck. The EE also filed for unemployment.
Miss stepped on grader latter causing operator to hold his entire body weight with his right arm on the entry handle so to not fall on grader blade. pulled his bicep muscle to a very painful state to which he could not curl his arm without pain level of 8 on a 1 to 10 scale. doctor ordered MRI may have torn muscle. MRI as of(5-2-2013)has not been done do to workmen's comp.
EE was swinging a sledge hammer, missed the focal point and strained his shoulder when trying to gain control of the sledge. Injury was reported late
Truck driver crawled in trailer to clean access material from trailer.
EE was working on the dragline... slipped and fell on the catwalk landing on his left elbow.
EE was breaking Flexco lacings off the conveyor belt with a Flexco chisel and a hammer. A piece of the hammer shattered as he was striking the chisel and a piece went into his abdomen.
Employee went down ladder to adjust feeder opening. After doing so, he then was using a scraper approximately 4" in length to remove build up on the sides of chute. In doing so, the material flowing from feeder grabbed the scraper pinning his thumb between the hand rail and the scraper, with enough force to cause the laceration to his thumb.
Pit was dry and maintained. Miner stated that he was bent over looking into his lunchbox and not paying attention where he was going and veered off the road into the pit. Truck dropped approximately 5ft. coming to rest on its left side, damaging the walkway, handrails. and door.
EE was replacing some lacings on his conveyor belt. He was using a hammer and chisel to split the lacings, when a small piece of the hammer broke off and struck EE in the left chest.
EE was using a chop saw to cut a piece of angle iron and his hand slipped and cut his thumb.
EE slipped when exiting the conveyor ladder, causing skin to be taken from his lower left leg. Initial first aid/wound care was given at time of reporting to his supervisor. EE refused to go to te doctor at that time.
EE fell off ladder while working on plant. EE fractured right wrist.
Service truck gas pedal was stuck causing the truck to strike light post, CAT haul truck(parked), etc before coming to a stop.
Door to tool box fell on arm.
Employee exited his Euclid. Upon walking from truck, he experienced a sharp pain in lower back that got worse.
As the dragline was backing away from the pit, (the truck driver was parked about 30 yards directly behind the dragline) he ran into the side of the Euc R40 tipping it on its side. The driver fell to the opposite side of the cab.
EE turned to get wrench and hit his elbow on the door latch.
EE was attempting to clean out feeder when the tool he was using was caught in a clay ball. As the clay fell out of the feeder, it pulled the tool with it, pinching and cutting the EE's finger against the railing.
EE slipped on a conveyor catwalk and hurt knee.
Hauling from pit to plant; the road was soft due to recent rains, and the egde of the road gave way causing the truck to roll on its top.
removing large nut with a 1" impact wrench, the torque of the impact wrench twisted hand into gear tooth.

CHIPPING RUST OFF ANGLE IRON TO REPLACE IT WITH A NEW ONE. IT BROKE LOOSE AND FELL ON HIS RIGHT FOOT BEHIND THE STEEL TOE BOOT. CUT HIS BOOT AND2 TOES. (8 STITCHES)
EE WAS INJURED WHILE WORKING ON A CONVEYOR. WE WERE ADJUSTING ROLLERS BECAUSE WE HAD JUST PUT A NEW CONVEYOR BELT ON. EE SAW SOME MUD ON THE BOARD ROLLER ON THE TENSIONER & HE REACHED INSIDE THE GUARD TO CLEAN IT & THE BELT GRABBED HIS ARM & PULLED IT BETWEEN THE ROLLER & THE BELT. I HAD JUST TAKEN THE LOCK-0UT OFF SO WE COULD TURN THE BELT ON TO ADJUST ROLLERS.
FEET WERE SOAKED FROM DAYS PRIOR AND THIS DAY WHILE WORKING IN AND AROUND RECENT FLOOR WATERS. POSSIBLE CONTAMINATION CAUSED CELLULITIS.
BACKED HAUL TRUCK TO NEAR DUMP SITE-AND TRUCK WENT OVER EDGE-TURNED TRUCK OVER ON RIGHT SIDE 19' DOWN.
WALKING DOWN CONVEYOR-FOOT SLIPPED AND KNEE POPPED.
ACCESSING DRAGLINE HE STRAINED A MUSCLE.
CLIMBING UP ON GRADER WHEN HE FELT A PAIN IN HIS STOMACH.
WORKING ON SAND PUMP-CUT TWO FINGERS ON RIGHT HAND.
HITTING METAL GEAR ON DRAGLINE. JAR HURT BACK.
REMOVING MOTOR FROM OLD CONVEYER TO INSTALL NEW ONE, CONVEYER FELL WITH EMPLOYEE ON IT. ALL SAFETY PROCEDURES WERE BEING FOLLOWED.
WORKING ON DRAGLINE--WRENCH SLIPPED AND HIT HIS LIP.
CHANGING SCREENS ON SHAKER. FRACTURED ONE LEFT RIB.
