Showing all 23While standing on a pile of dirt and doing maintenance on a water line, employee reached over to cut a zip tie on the hose. While holding the hose in his left hand and cutting with a razor knife in his right hand, he accidently cut his left index finger which required 4 stitches.
EE originally injured his back on 6/22/07 & 7000-1 filed. On 11/5/08 EE was installing conveyor & hurt his back while turning as he moved conveyor. At the time he thought he had just pulled a muscle or something & didn't report it. On 12/3/08 he reported to our safety director that his back was bothering him & his Dr was taking him off work. EE thinks it is related to '07.
Employee was working with a come-along to repair a belt in the plant. The handle on the come-along tensioned back and struck and cut the ee over the right eye. He was wearing gloves, safety glasses and a hard hat at the time.
A conveyor on a crushing plant had iced up over the weekend. EE was holding a torch and twisting back and forth while applying heat to the head pulley to melt the ice. Suddenly he felt a sharp pain on the left side of his back.
Empoyee was performing plant maintenance and was carrying heavy parts. While stepping onto a catwalk he slipped on the edge of the catwalk and hurt his back.
Employee was preparing to get up on a loading machine. As he started to put his foot on the first step, he somehow missed the step which must have thrown him off balance. When his foot came down on the ground, the strain apparently caused a hernia in his groin area.
EE was cleaning a roller on a conveyor, contrary to his training, he was standing directly under the roller and some dirt fell from the roller. Although he was wearing a hard hat and safety glasses, some dirt got under his glasses and into his eye.
EE alleges that he tripped and fell while walking across the quarry. Nobody witnessed the alleged fall. EE has tried unsuccessfully (to date) to have a prior injury to the same shoulder covered under workers compensation.
EE was working in the quarry generator room and while attempting to pry a nail out of some wood, the pry bar slipped off the nail head causing some minor back strain as he apparently re-aggravated a pre-existing back problem.
EMPLOYEE WAS STEPPING ONTO A CATWALK AFTER GREASING A CONVEYOR. THE CATWALK GAVE WAY & HE FELL.

EE WAS WALKING ACROSS YARD WHEN LEFT KNEE GAVE OUT.
EE WAS CHANGING SCREEN CLOTH, WHEN HE SMASHED HIS LEFT FINGER #5 IN THE CLOTH.
EE FELL WHILE WALKING ACROSS THE YARD WHICH WAS ICY
EE WAS KNEELING ON FLOOR REPAIRING A TRUCK
PULLED A DISC IN BACK WHILE CLEANING OUT AROUND A CONVEYOR.
EMPLOYEE WAS MOVING WOOD PLANKS AND HURT/STRAINED BACK MUSCLES.
FELL FROM CONVEYER CARRYING MATERIAL.
TWISTED LEFT KNEE WHILE CLEARING ROCKS FROM CRUSHER.
EE LIFTED PALLET WITH LEFT HAND AND SPRAINED HIS SHOULDER.
