While cleaning brush up an employee contacted poison ivy. EE went home and had a mild reaction. Today EE's reaction was severe enough to go to see a doctor. EE had reaction on EE's face and stomach.
Miner strained EE's arm shoveling underneath the feeder hopper. Miner did not fill out an 801 until 7-23-25 when EE's arm hurt enough to see a doctor. The 801 was tendered the end of EE's shift on 7-23-25.

Miner was helping to move a wall locker and the wall locker door / door frame smashed EE's hand.

Miner was grinding links on a feeder chain and got a metal shaving in EE's eye. EE said that it did not start irritating EE until 11/12/19 when EE went to the ER after work.

Scale Operator was stung by a bee. Scale Operator is allergic to bees. Scale Operator was taken to the Hospital for observation.

Miner was tightening bolts using a hand wrench and impact gun. Wrench slipped off a bolt hitting miner in the ribs. Miner did not go to the doctor; completing EE's shift. Miner worked a full shift on Saturday, 8/6/16 then went to the doctor after EE's shift. No lost time. Miner was not restricted from any duties.

replacing a bushing on a screen using a hammer to hit it; spacer slipped off while swinging and hit finger with hammer. Miner stayed the full shift. Finger began aching on Sunday; reported to EE's Doctor 7/11/16 then returned to work.

Miner slipped in mud and rolled ankle. Supervisor was informed but Miner indicated that he felt fine. However, by 11-4-15, Miner's ankle had swollen to the point that Miner sought medical attention.

Miner was driving haul truck on 4/7/15 and claimed the haul road was bumpy causing strain to lower back and hamstrings. He did not go to the doctor but reported to work on 4/8/15 (the next day); working as the Feeder Operator as a protective measure. On 4/9/15 Miner went to his Chiropractor who placed him on Light Duty.

Miner lifted a chain up a ladder, straining his lower abdominal muscles on Wednesday, 12-3-14 but did not report an injury. On 12-5-14 miner came to the office reporting soreness and then went to the doctor at 8:00 am the same day.

Sale Operator claims that on 11-13-14 she slipped on ice on the scale deck t-bar hurting her left heel. She did not report the accident and did not seek medical attention until 1-16-15 when she went to urgent care after work. She reported to work the next work day; she did not lose time / work day due to injury.

EE was stepping onto a belt and slipped and fell onto the belt, landing on his right knee. That evening his knee was hurting so he went to the doctor and was diagnosis with a Knee Sprain.

Employee was operator an Excavator when he told his Supervisor that he wasn't feeling good and needed to go to the Doctor. Employee was having stomach problems. Supervisor called for a ambulance because of employee's condition. Employee was transported to McMinnville Hospital. Company is waiting to hear what the problem his.

EE had been working for a few hours when he went to his Supervisor and stated that he was feeling lite headed and he thought he had Vertigo again. He stated that he had someone coming to pick him up and take him home. We were informed later in the evening that the Doctors thought that he might have had a mild stroke and they were admitting him to the Hospital.

Employee was holding a bar over his right shoulder while his Supervisor was hitting the bar with the hammer. The Supervisor didn't hit the bar square, which caused the hammer to kick to the left, and hitting the employee on the bottom part of his nose.

Standing on a step stool changing fuel filters on a haul truck when the stool gave way and he fell. He made contact with the upper part of his body and the ball joint on the haul truck.
Injured EE was helping another EE change screens. When they were moving the screen to the side, injured EE pulled on it for the fourth time and felt a sharp pain in his back.

EE WAS OPERATING BACKHOE, DIGGING OUT SUMP HOLE, WHEN THE HYDRAULIC LINE BLEW OUT. EE GOT OUT OF THE MACHINE TO FIND THE HOSE. HE SLIPPED ON OIL AND FELL, LANDING ON HIS BACK ON THE FUEL TANK EDGE.
