Showing all 38Operator was repairing skid steer attachment. EE was swinging a sledge hammer, and EE heard something pop in EE's bicep area.
Customer Truck was tarping the load in the employee driveway when the tarp came in contact with the high voltage power lines. No injury or impairment.
Removing the belt wiper on the conveyor feeding the gravel plant screen. While removing the 2nd bolt holding the wiper, the wiper fell down and pinned employee's hand by pinching it between the wiper and frame.
Operator was operating a 980H Caterpillar loader, EE was feeding the primary crusher. During the steering operation, EE's left shoulder popped. The operator is feeling pain and a popping sensation through normal movement. The operator is scheduled to have a surgery to repair two torn ligaments.
Employee was eating lunch and got choked on piece of meat. Once given Heimlich maneuver one of the pieces was unwedged allowing employee to breathe. Employee was taken to hospital and put under anesthesia for a throat scope to remove rest of blockage.
The employee was closing the gate to the entrance of the mine. It was raining hard and very windy, and the gate swung away from the employee while trying to grab the gate, EE lost EE's footing and fell. EE hit EE's arm on the ground/rock driveway. EE notified the supervisor of incident after clocking out.
Co-worker was prying on a roller bearing, EE was standing behind and aside co-worker. The roller freed making contact with EE's teeth.
Employee was performing a routine plant inspection. Employee was stepping over the railing into the screen deck area. Employee twisted knee while performing this task.
Installing transmission pump, pump slipped out of employee's hand and fell, striking left hand and lacerating hand and finger
Employee was working in shop area cleaning up the remnants/shavings from working with Poly Pipe. While sweeping shavings from shop floor feet became tangled and tripped and fell onto the concrete floor.
Buildup of material was preventing the return roller from rolling. A support soldier block was in close proximity to the return roller. While using a pry bar to clear the buildup off of the soldier block, the pry bar came into contact with the moving conveyor causing the pry bar to impact the palm of the hand. The palm of the right hand received a 1" long x ½" wide laceration.
We were doing a job at Brenda Kay Sand LLC, EE and co-worker were attempting to move a piece of grated steel that had been stacked on top of another. As ee lifted end up in order to place spud wrench, the sheet moved towards ee. Doing so trapped finger between the sheets and caused a tip amputation of the left middle finger.
Employee was replacing the wear plate on the dredge pump. The wear plate was attached to a chain hoist. Employee stepped down grabbing the handrail. The wear plate swung, hitting employee's thumb that was holding onto the handrail.
After adjusting of parking brakes, while climbing up stairs on a Caterpillar 980H wheel loader, operator jammed left hand into the handrail.
EE was changing our the screens on the gravel plant. The incident occurred while removing the 1/4" screen. He has continues to work, but was determined at a doctors appointment 5/7/15 that hernia would be repaired surgically in June.
Employee was lifting a pivot pin from a hydraulic hammer to separate the hammer from the 450LC track hoe for maintenance. Employee had a bone spur on his left heel. That bone spur influenced the employee to lift the pin in an awkward manner causing him to put strain on his left side lower back.
A 2 1/4" 3/4 drive socket was being used to tighten down lid bolts on primary crusher. The socket was being used with a 3/4 drive air ratchet. Miner was using his right hand to hold socket in place and his left hand to separate impact. Socket had a piece of chrome sliver peeling off of socket. It was about 1 1/2 inches long. The sliver went into the miners right hand.
While filling the fuel truck with fuel, the employee twisted his ankle on an eye bolt located in the walkway provided. The diagnosis was a sprained ankle.
Employee took the fan belt guard off of the 450LC trackhoe. When he started to get off his wedding ring got caught on the skid grate. The weight of his body caused the wedding ring to cut his finger.
While performing plant clean up, employee exited the bobcat and his right foot slipped off of the drag attached to the bobcat. When his foot slipped, he landed on his right knee.
The EE got off an end loader to turn off a conveyor belt. After running from the loader to the plant, he started to experience knee pain.
Employee was drilling with an 18 volt cordless drill. The drill bit caught and the drill twisted out of his hand and dislocated his little finger, right hand.
Employee was getting off of a Komatsu 220 hammer when feet slipped and he fell. It was raining at this time. **Received 8 Sutures**
Employee was cleaning trash out of cab of haul truck when he felt something stick his finger. The next morning his finger was swollen up. Sent him to emergency room to have it checked out.
Employee was using a impac wrench to tighten bolts on lower roller on cat 245B shovel. a piece of metal flew into right eye.
WORKING ON TAIL PULLEY ON CLASS ONE BELT WHEN EMPLOYEE GOT A PIECE OF DEBRIS IN HIS LEFT EYE. THE WIND WAS BLOWING PRETTY HARD AT THE TIME. EMPLOYEE EYE WAS WASH OUT AND LATER ON THAT NIGHT HE HAD TO GO THE DOCTOR.
THE EE WAS GETTING OUT OF MAINTENANCE TRUCK. THE WIND BLEW THE TRUCK DOOR CLOSED, CATCHING THE EE'S SHIRT IN THE DOOR CAUSING HIM TO LOSE HIS BALANCE & FALL.
EE WAS CLEANING OUT A STOPPED UP CHUTE. HE WAS USING A FLAT BLADE MOUNTED ONTO A ROUND ROD. WHILE PUSHING OUT MATERIAL BUILD UP HE HIT HIS PINKY FINGER ON RIGHT HAND BETWEEN ROD & TOP OF CHUTE . HE CUT HIS FINGER REQUIRING 7 STITCHES TO CLOSE IT UP.
EMPLOYEE WAS HELPING TO INSTALL ROLLER ON CONVEYOR BELT. WHILE TRYING TO REMOVE THE BAD ROLLER HE WAS HOLDING WHILE ANOTHER EMPLOYEE WAS HITTING IT WITH A HAMMER. THE ROLLER TURNED AND THE HAM MER HIS HIS LEFT INDEX FINGER. THE FINGER NAIL WAS REMOVED AND TWO STITCHES PUT IN THE END OF THE FINGER.
EE WAS OPERATING A KON 420 LOADER. HE STARTED TO STAND UP & HAD PAIN IN LOWER BACK. HE WAS HELP DOWN FROM LOADER 7 TAKEN TO DOCTOR. THE DOCTOR SAID HE HAD LOWER BACK SPASM.
EMPLOYEE WAS HELPING TO DISLODGE A PIECE OF STEEL HUNG IN JAW CRUSHER. THEY WAS USING A DRIFT PIN AND A SLEDGE HAMMER. INJURY EMPLOYEE WAS WATCHING ANOTHER EMPLOYEE HIT THE DRIFT PIN WITH HAMM ER WHEN A PIECE BROKE OFF & FLEW INTO CORNER OF LEFT EYE. THE PIECE WAS HOT & BURN EYE.
EMPLOYEE WAS HELPING TO PULL SLEEVE OFF OF AN ENGINE. THE SLEEVE PULLER FELL & HIT EMPLOYEE ON LEFT HAND CAUSING A LACERATION ON RING FINGER AT THE SECOND JOINT. 4 STITCHES.
EE WAS REMOVING A BUSTED HYD. HOSE ON DRILL. THERE WAS OIL WHERE HE WAS WORKING. EE FEET SLIP AND HE GRAB HOLD TO KEEP FROM FALLING. WHEN HE GRAB HOLD HE STRAIN HIS BACK.
EE WAS HELPING TO TIGHTEN BOLTS ON CRUSHER WHEN HE TURN AND PULL MUSCLE IN HIS BACK.
EMPLOYEE WAS TIGHTENING BATTERY TERMINAL ON TRUCK WHEN THE WRENCH HE WAS USING MADE CONTACT WITHMETAL BOX. THE WRENCH WHEN SHORTENED MADE CONTACT WITH EMPLOYEE RING AND HE RECEIVED THIRD DEGRE E BURN ON RING FINGER.
EE WAS FIXING LEAKS ON WATER HOSE. WHILE ATTEMPTING TO COUPLE HOSE TO QUICK COUPLING THE COUPLING TWISTED & CUT LOWER RIGHT THUMB
EE WAS GETTING OFF OF TRUCK TO REMOVE A PIECE OF STONE FROM ROADWAY. HE SLIPPED FROM TRUCK LADDER AND FELL ON THE GROUND CAUSING A LOWER BACK SRPRAIN.
