I was walking down a slight incline in the rock pit, and there was loose gravel that gave way under my right foot, resulting in me falling to the ground from standing height. When I fell I landed on my left knee, and my right wrist. My left knee had minor scrapes and cuts, and my right wrist received a hard impact and resulted in lots of pain and swelling.

Showing all 5Not sure exactly what may have caused injury. I pulled on a water pump pull start motor. Pulled around hose watering pit grounds, it's very heavy. Started running 970 loader and just noticed pain in right shoulder around 11:00 am. Noticed shoulder was worse by 3:00 pm.
On Wednesday 08/21/2019 this employee was working at crusher site picking rebar from conveyor. On Thursday 08/22/2019 employee told supervisor that it was painful to breathe/sore throat/stuffy nose. Employee said it was the dust from the crusher and wanted to go to the doctor.
