Employee #1 was clearing frozen material from the road runner feed hopper. Still not understand the grizzly on the end of the feeder began lowering. Two employee who were just a few feet away, immediatly warned the grizzly. Both ee stated that #1 ee wouldn't respond or react 10 to 20 second. he was pinned by the grizzly. due to his injuries. the investigation is ongoing.

The employee was retrieving a piece of metal (flipper tooth) from a jaw crusher by cutting the tooth with a torch. When most of the tooth was cut, the jae closed a little bit and shot the tooth into the face of the employee.

Blaster did not have good visual of area below: parked equipment blocked access into the blast area. After first blast, loader went into area to get a bucket of dirt. Second blast went off. There was damage to cab of loader. Injuries to ribs and head of operator.
