EE was shoveling out the bottom of a cup elevator when the blade of EE's shovel struck the side of the enclosure, with the impact sending a sharp pain up EE's left arm from the shovel.

While closing the door of end loader with left hand and bracing self with right hand on the door frame, employee closed the door on right hand causing crushing injury & fracture to end of right middle finger.

Operator was attempting to replace drivebelts on a de-energized fan assembly for the dryer dust collection system. EE attempted to roll the belts onto the pulley when EE's left pinky became pinched between the belt and the pulley, severing the tip of that finger.

Employee was assisting with manual installation of v-belt onto drive motor pulley assembly when fingers were pinched between the belt being installed and the pulley surface. Employee was not aware of hand position relative to the belt being installed. Employee had hand in an exposed position. This resulted in a crushing type injury middle and index fingers.

EE reported to work on 6/25/2019 complaining of a sore and stiff low back. EE felt that working at the gravel bagging station the previous day may have led to a strain of EE's low back.

EE was cutting bags apart for use in the bagging machine and cut right thumb with the box cutter.

Employee was standing in the warehouse when EE reached into the right pant pocket with right hand to grab the radio when EE had a low back spasm causing EE to go down to one knee.

Injured left shoulder while using a breaker bar to remove bolts during plant disassembly around 2/22-26/2017. Initially thought shoulder was achy from exertion and did not report an injury until March 10, 2017 when diagnosed with rotator cuff tear in left shoulder.

Employee went to the top floor of Dry Plant L to check the bin levels and tripped when entering the doorway. He lacerated the left thumb on the rotex during his fall. The rotex machine was inside the doorway.

The EE was manually lowering the dumper hopper attachment for the forklift and his index finger was pinched between the hopper handle and the forklift mast.

EE was lifting the end of a pipe to place it on the forks of the front end loader when he felt a strain in his back.

Employee was cleaning out a bagging machine. There was a hinge plate on the machine that fell on his finger causing a laceration.

When the operator, using a torch, attempted to burn an opening to install an idler on the conveyor belt, the flame apparently ignited soot build-up under the conveyor's rain hood from an oil burner that was used to heat the feed hopper. The flame then spread to a second conveyor. It appears both conveyors are destroyed as a result of the fire. Fire extinguished in 30-45 minutes.

Employee was filling and tying a 100# bag of sand. As he pulled on the top of the bag to lay it down on the conveyor, he felt a pain on the inside of his left elbow.

EMPLOYEE REPORTED PUSHING ON DUMP LEVER TO LOWER BOX AND A TINGLE WENT DOWN HIS ARM.

EE WAS LIFTING A SCREEN CLOTH WITHOUT ASSISTANCE AND EXPERIENCED PAIN IN HIS BACK.

EE CAUGHT FINGER BETWEEN TAILGATE AND DUMP BOX ON TRUCK WHILE REMOVING LOOSE MATERIAL.

EE REPORTED THAT AFTER FINISHING WORK ON 1-25-00 HE EXPERIENCED PAIN IN LEFT SHOULDER. EE DID NOT REPORT ANY PROBLEM UNTI 2-13-01.

EMPLOYEE SLIPPED AND FELL ON ICY GROUND TWISTINGRIGHT ANKLE.

EE REPORTS THAT WHILE TIGHTENING A BOLT, THE WRENCH SLIPPED CAUSING HIM TO FALL AGAINST A STEEL BEAM, INJURING HIS RIBS.

EE REPORTED FOREIGN MATERIAL IN HIS EYE THAT HE WAS UNABLE TO GET OUT AND SOUGHT MEDICAL ATTENTION.

EMPLOYEE HAD A DIABETIC REACTION AND WAS TRANSPORTED FROM SITE BY AMBULANCE.

EE LOST BALANCE WHILE LIFTING SCREEN FROM RAILS AND FELL ON LEFT HAND.

EE REPORTED PAIN IN HIS LOWER BACK AFTER LIFTING WOOD BLOCKS.

EMPLOYEE REPORTED THAT WHILE CUTTING STEEL PLATE, THE PLATE SWUNG DOWN AND STRUCK HIM IN THE FOOT AND LEG.

EMPLOYEE REPORTED LOW BACK PAIN AFTER LIFTING HEAVY MATERIALS.
