Moving conveyors and cut his finger on left hand.
Employee was walking around the standard cone to the first van when the doors on the VSI 2100 came open and started throwing rock out. A rock flew out towards the employee and hit him in the mouth.
Employee was injured when he slipped and fell when he was climbing into his drilling equipment. Snow and ice conditions on the steps caused the employee to slip. He fell and hit his ribs and head when he fell. Employee was taken to a medical facility and was diagnosed with a fractured rib and bruising under the eye.
Fixing tear in conveyor belt left hand got caught - smashing it. Employee has been treated at Union County General Hospital.
Finished working on 988F, the loader pulled away employee continued picking up tools. As he reached down to get the tools the wind blew dust and fiber in to both eyes. Employee was taken to Union County General Hospital in Clayton New Mexico, 301 Harding Street, Clayton New Mexico.
EE WAS PRYING ROCK FROM JAW WITH 6' PRY BAR. THE BAR SLIPPED AND CAUGHT EE'S THUMB BETWEEN THE BAR AND THE HANDRAIL. SMASHED LEFT THUMB.
Cutting a piece of metal from the jaw crusher & it fell onto the chute & slid down the chute pinning the employee's rt. hand under the piece of hot metal. It burned the back of his rt. hand. EE could be away from work for 3 weeks.
EE cleaning mud from under conveyor slipped & fell back & hit his head. Cut to back of head. Bar EE was using hit him in chest. Foreman called Moriarty ambulance & EE was transferred to University Hospital (Albuq) to be checked out. It's Southway policy that any EE hurt be checked out by a Dr or ER. EE was wearing fall protection, taken off at hospital.
CLEANING JAW CRUSHER OUT FOR THE NIGHT, ROCK ROLLED-
WORKED ON A GUARD AND BOLT FELL OUT AND EE CUT MIDDLE FINGER ON LEFT HAND 5 STITCHES.
A ROCK ROLLED ON TO THE PLATFORM AND STRUCK HIM IN THE BACK OF THE LEG. EE HAD OF THE PPA ON. SAFETY GLASSES, DUST MASK, GLOVES AND SAFETY HARNESS WERE IN USE.
EE WAS HELPING CO-WORKER PUT ON A PATCH ON THE PRIMARY CRUSHER. THEY GOT READY TO MOVE THE BELT SO AS TO GET TO THE OUTER CLIPS. CO-WORKER TOLDEE AND OTHER EE'S TO WATCH OUT HE WAS GOING TO BUMP THE CONVEYOR. EE THOUGHT HE SAID TO PULL THE BELT BY HAND. WHEN CO-WORKER HIT THE BUTTON, EE GRABBED THE BELT AND THE BELT BROKE HIS RT. FINGER AND CUT HIS OTHER FINGER.
