Instructions to all personnel if they witness any non-employee out of their vehicle instruct them to get in and stay in vehicle.

Miner was operating Cat haul trk when he ran off haul road striking a boulder berm that had been placed to protect trucks from a drainage ditch and culvert pipe. The truck crossed over a boulder hitting a dirt bank causing the operator to hit his head on cab frame

Miner was fueling and greasing D8N Dozer to start stripping burden. He was bending over to grease ripper on the back of the machine, as he straightened up, he pulled a muscle in his lower back.

Wet shot rock plugged up the Primary Crusher. I was inside of the crusher cleaning out the rock. I picked up a large rock to throw outside, the rock slipped off of the blow bar and fell on my left index finger, cutting it deeply.

THE AIR WAS NOT RELEASED FROM BRAKE CHAMBER ON CRUSHER. EE WENT TO TAKE BRAKE APART AND IT BLEW APART ON HIM HITTING HIS HAND.

INSTALLING A CABLE ON CA6 PLANT WHEN PRY BAR SLIPPED CAUSING A PIECE OF METAL TO FLY UP & STRIKEEE IN THE FACE

EE STATES HE WAS LOADING A HOLE AND THE HOSE BECAME STUCK IN SOME LOOSE ROCK. HE WAS LIFTING ANDTWISTING THE HOSE WHEN HE HEARD HIS SHOULDER "POP"

HAD CUMALONG HOOKED TO A CRANE AND OTHER END HOOKED TO EYELET ON PIPE. HE WAS WRATCHEDING UP THE PIPE WHEN THE WELD ON THE EYELET BROKE. CAUSING THE CUMALONG HOOK TO HIT HIM ON TOP OF HEAD.

BENT OVER GRINDING ON A PIECE OF METAL, AND WHEN STOOD UP HIT HEAD ON SHARP OBJECT.

SNASHED HAND WITH HAMMER WHILE TRYING TO REMOVE PINS FROM CUTTER HEAD.
