Employee was cleaning the spirals of a sand screw with a wire grinding wheel. A wire from the wheel broke off and struck his eye.

Employee was lathing bushings in a loader bucket. A large metal shaving was stuck to the boring bar and the employee grabbed it with his gloved hand to break it off and cut his finger. The laceration required stitches to close.

Employees were removing steel plate and rebar from a pallet that was raised on a forklift. The supervisor instructed the forklift operator to lower the pallet. Instead, the operator moved the forklift back as the supervisor was removing a piece of rebar. The rebar dislodged a piece of plate which fell onto the assisting employee's foot.

On 8/27/12 the employee was cut on his elbow when the nut he was trying to loosen broke free & his arm hit the frame of a machine. He received 1st aid & returned to work. On 8/28/12, while using a wrench to turn a thread-tapper & compensating for the pain in his elbow, the wrench slipped, glanced off his safety glasses & struck him in the forehead, causing another laceration.

The employee was lifting bags of cement and felt pain in his back. He did not seek medical treatment at the time of the injury. On 10/14/2010 he saw a physician and was told to take a leave of absence to have back surgery.

While using a utility knife to cut rubber conveyor belt, the knife slipped out of a groove in the rubber causing a laceration to the employee's left thumb. The wound required three stitches, x-ray and tetanus shot.
