Two employees were loading items onto a trailer. Weather conditions were dry and very windy. Wind blew grit into one employee's eye which became embedded.
Employee was grinding steel. Wearing safety glasses but no face shield. Material got into his eye.
Employee was welding, repairing a rock screen. When he finished, he lifted his helmet to check his work. A piece of slag popped off and flew up under his safety glasses into his eye.
employee was stepping up onto the step on an excavator when he felt a pop and tear in his left calf.
Employee was cleaning the spirals of a sand screw with a wire grinding wheel. A wire from the wheel broke off and struck his eye.
Employee was lathing bushings in a loader bucket. A large metal shaving was stuck to the boring bar and the employee grabbed it with his gloved hand to break it off and cut his finger. The laceration required stitches to close.
Employees were removing steel plate and rebar from a pallet that was raised on a forklift. The supervisor instructed the forklift operator to lower the pallet. Instead, the operator moved the forklift back as the supervisor was removing a piece of rebar. The rebar dislodged a piece of plate which fell onto the assisting employee's foot.
On 8/27/12 the employee was cut on his elbow when the nut he was trying to loosen broke free & his arm hit the frame of a machine. He received 1st aid & returned to work. On 8/28/12, while using a wrench to turn a thread-tapper & compensating for the pain in his elbow, the wrench slipped, glanced off his safety glasses & struck him in the forehead, causing another laceration.
Two employees were shoveling trash into the back of a garbage truck. One employee lifted his shovel as the other was leaning down and struck him in the forehead with the shovel.
Employee was chipping slag off of a weld. A piece flew up under his safety glasses and lodged in his eye.
The employee was using a hammer to dislodge a rock from a grate. A rock fragment broke off and stuck into his hand between his index and middle finger.
The employee was lifting bags of cement and felt pain in his back. He did not seek medical treatment at the time of the injury. On 10/14/2010 he saw a physician and was told to take a leave of absence to have back surgery.
Employee picked up a can of welding rods. A sharp edge on the can caused a cut to the employee's thumb.
Employee was using a bar to lift a feeder into place. He strained too hard and injured his groin.
Employee was walking across the floor of the maintenance shop when he tripped on a gutter grate and fell down.
While driving a haul truck, the employee hit an overhead obstruction. The impact caused injury to his ribs. The employee did not initially lose any work time, but has since decided to retire.
Employee was attaching a chain to a piece of equipment, preparing to move it. He pinched his thumb between a pulley and the lifting chain.
While using a utility knife to cut rubber conveyor belt, the knife slipped out of a groove in the rubber causing a laceration to the employee's left thumb. The wound required three stitches, x-ray and tetanus shot.
EE was repairing a dredge bucket. He stepped onto the teeth of the bucket, where he twisted his ankle, causing a hairline fracture.
EMPLOYEE WAS DISASSEMBLING A CRANE BOOM WHEN THE SUPPORT SECTION FELL OFF OF THE BOOM, ONTO THE EMPLOYEE'S FEET.
Employee was guiding a pan onto a lowboy trailer when he fell through a hole in the trailer bed, injuring his knee.
EE was exiting his loader and twisted his ankle.
EE WAS GRINDING A PIECE OF METAL WHILE WEARING SAFETY GLASSES. A PIECE OF SLAG FLEW INTO HIS EYE.
EE WAS STANDING ON A CONVEYOR, REACHING AND EXTENDING HIS BODY TO LIFT CHAIN FALL TO JIB CRANE OVER THE CRUSHER BASE. THIS INCIDENT OCCURRED IN FEBRUARY, BUT THE EE DID NOT SEEK MEDICAL ATTENTION OR TAKE TIME OFF FROM WORK UNTIL MARCH 25TH.
DRIVER WAS WALKING FROM HIS TRUCK TO THE SCALEHOUSE. HE FELL BETWEEN THE SCALE AND THE END OF THE SIDEWALK, HIS LEFT FOOT WENT THRU THE SIDEWALK.
WHILE FUELING LOADER, THE HANDLE ON THE NOZZEL STUCK, EMPLOYEE TURNED AROUND TO SHUT OFF PUMP, TWISTING BACK AND HIP.
EMPLOYEE WAS SWINING A 10-16 LB SLEDGEHAMMER IN A BENT OVER POSITION WHILE HELPING TO CHANGE A BEARING ON THE WASH PLANT.
EMPLOYEE WAS PULLING WIRE THROUGH A CONDUIT IN THE SHOP, THE WIRE BROKE AND THE EMPLOYEE FELL ONTO HIS BACK.
EE WAS CHECKING THE WASH PLANT BEFORE START-UP, CLEANING THE FEED CONVEYOR AND ATTEMPTING TO REMOVE A PIECE OF WOOD FROM THE BELT. THE EE PINCHED HIS FINGER BETWEEN THE SIDE OF THE HEADSTOCK & THE FEED BOX.
EE WAS WIRE WHEELING A RUSTED FLATATION TANK OUTDOORS, WIND BLEW PIECE OF RUST BEHIND SAFETY GLASSES AND INTO RIGHT EYE. SENT TO HOSPITAL-DR USED NEEDLE TO REMOVE RUST BUT NO DAMAGE TO EYE OCC URRED. '
EE WAS WALKING ON A HORIZONTAL MANITOWAC BOOM TO PERFORM MAINENENACE DUTIES. HIGHLIFT OPERATORMOVED BOOM CAUSING EE TO LOSE BALANCE & FALL TO THE GROUND. HE HIT LOWER RIGHT BACK & LEGS ALONG WITH HIS CHEST & ELBOWS. MOST OF THE DAMAGE WAS DONE TO THE RIGHT SIDE, THERE ARE NO BROKEN BONES
EE WAS HELPING MOVE TOWERS WITH OVERHEAD CRANE IN FABRICATION SHOP. LOAD ON CRANE WAS NOT EQUAL& ABOUT 200LBS OF THE STEEL TOWER WAS BROUGHT DOWN ON LEFT FOOT. WENT TO ER AFTER WORK CRUSHED ARCH AND STRETCHED LIGAMENTS
