Showing all 12Co-worker was backing up the pickup and backed into employee who was on the rung going to operator station. This was caused due to the blind spot while in the pickup and the overhang of the boom.
EE WAS ON A WET & MUDDY SURFACE AND SLIPPED AND FELL, HE MISSED TWO STEPS ON THE CRUSHER SCREEN WALKWAY
EE WAS DRIVING STAKES INTO THE GROUND. HE FELT HE TWISTED AND STRAINED HIS BACK. (EE DOES NOT NORMALLY WORK AT THE MINE. HE IS A GENERAL MAINTENANCE GUY THAT TRAVELS FROM FACILITY TO FACILITY.)
Employee informed us on 2/10/05 that he believes that he may have somehow injured his knee at work. He went to the doctor on 2/7/05. He states he woke up on Sunday, February 6, 2005 with excruciating pain in his knee. He could not think of any specific thing that could have caused this pain.
EMPLOYEE WAS STEPPING OFF OF A CONVEYOR & TWISTED HIS RT. KNEE. IT WAS WET & A LITTLE SLIPPERY.
While leaving the pit on the haul road he forgot to lower his truck box. He was driving over the posted speed limit with the box in the air. His box hit the over land conveyer which caused the truck hydraulic box to land on the cab of the vehicle.
DRY DUST BLEW INTO EE'S EYE WHILE HE WAS WORKING ON THE TAIL PULLEY GUARD ON BELT #12 VSI.
