Showing all 14Employee was in a kneeling position and when they got up they stumbled off of a muck pile and came down on their left leg hurting their left knee.
Employee was cleaning out from under a catwalk and was struck on right shin by an attachment from the skid-steer that was assisting in the cleanup process, and received a contusion to the shin.
Employee was directing an asphalt truck back to dump on entrance road into quarry and was overcome with a massive heart attach (cardiac arrest)
EE was performing maintenance on crusher. Stepped backward and strained left knee. Examined at St. Thomas More, released without restrictions.
States that he was exiting haul truck in a hurry to get to the men's room. His foot slipped and he stopped his fall by grabbing the side rail with his left hand, spraining his left wrist. Steps were clean and dry.
Employee was cutting crown rubber with utility knife. Knife slipped and cut his left thumb. Stitches were required. Able to return to work.
Employee was cleaning cone for crusher where it is seated with airplane stripper. He was wearing safety glasses and long sleeve gloves at the time. Employee believes that after reaching into the cone with his glove to clear trash that he must have touched his face with his glove.
Was picking some wood out of rock and he twisted his right knee when he stepped wrong on a rock.
A driver of a leaser's truck caught his sneaker on the truck as he was getting down to get his scale ticket and he fell and hit the side of the scale house. He injured both his right shoulder and his right cheek.
Employee was tightening bolt on crusher jaw. Bolt broke and wrench and spring hit him in the face. His nose was broken and stitches were required.
Employee was rushing to help injured employee and hit right shin on handrail.
ROCK WAS PUSHED OFF HIGHWALL & HIT THE EDGE OF HIS MACHINE & JARRED HIM, INJURING HIS SHOULDER. (WC PENDING INVESTIGATION, EE DIDN'T REPORT TILL MUCH LATER.)
